Notification Under 10 CFR Part 21 Regulations

Reference:

Your Purchase Order X-03411 Item 01 Gentlemen:

Rosemount has determined that a notification to you, our customer, is required under the regulations called out in 10 CFR Part 21.

This notification is for the following situation.

Recent testing on our Model 1153 Series B indicated that there could be a functional problem with amplifier boards using components from a particular determined lot and manufacturer. Rosemount has identified all boards in this suspect lot with the exception of one. This board may have been supplied to you either in a complete transmitter or as a spare board ordered as our part number 01153-0123-0003.

The amp board or transmitter would have been supplied to you against the above referenced purchase order.

Ths suspected board can be identified visually by inspecting the circuit side of the board, and specifically the three operational amplifiers shown in the enclosed board layout print. The suspect board has Op Amps marked with the letters "PM", in addition to the date code on the top of the can. If your inspection finds this board, we are requesting you replace it with the one we have enclosed and then call Mr. R. Volsted at A/C 612-828-3473 for disposition of the replaced board.

If your inspection effort does-not find the suspect board, we would like you to keep the enclosed amplifier board as a spare against '

your above referenced order. This is offered as the compliments of Rosemount for the inconvenience caused by this notification. .
