SUBJECT:
AIT CHARTER - QUAD CITIES UNIT 1 SCRAM WITH EQUIPMENT FAILURES Enclosed for your implementation is the final Charter developed for the inspection of the events associated with the Quad Cities Unit 1 scram and equipment failures which occurred on February 6 and 7, 1992.
This Charter was prepared in accordance with the NRC Incident Investigation Manual and the Manual Chapter 0325 AIT implementing procedure issued for use on April 18, 1991.
As stated, the objectives of the AIT are to communicate the facts surrounding this event to regional and headquarters management, to identify and communicate any generic safety concerns related to this event to regional and headquarters management, and to document the findings and conclusions of the onsite inspection.

Ouad Cities Unit 1 Scram with Eculpment Failures Auamented Inspection Team (AIT) Chartqr You and your team are to perform an inspection to accomplish the following:
/
1.
Determine and validate the sequence of events associated with the Quad Cities Unit 1 scram and equipment failures that occurred on February 7, 1992.
2.
Evaluate the equipment failures at Quad Cities Unit 1 that v
were identified on February 6 and 7, 1992, including:
/
a.
The' failure of HPCI stop valve H01-2317.
/ b.
The failure of "C"
relief valve.
/ c.
The apparent failure of the reactor feed pumps to automatically trip at the appropriate vessel level and related operator actions.
/ d.
Anomalies associated with the main steam line flow instruments.
/ e.
'The cause of the reactor scram.
f.
The reason for the toxic gas monitor being inoperable.
/ g.
Any other significant equipment problems with safety related or balance of plant equipment that could have interfered with the ability of the operators to safely operate the plant.
In evaluating these failures, strongly consider root cause and determine if these failures are indicative of any broad maintenance weaknesses.

Determine'if-the components that failed are examples of repetitive failures.

Review the adequacy of the licensee's program for evaluating these events.
Oversee troubleshooting, testing and analysis of any quarantined equipment.

. Interview plant personnel-involved in the scram and the equipment failures to determine if personnel actions and procedural guidance were adequate.
/ 6.
Evaluate licensee managerial performance related to this event including shift supervision, management response, and maintenance supervision of activities involving failed components.
