Fabrication of Piping Subassmblies having Wall Thickness Icss than Required by Design Specifications Gentlmen:

Initial notification of Southwest's failure to cmply with design specifications was discussed by telephone with Mr. William M. McNeill, Region IV R& CPS on October 14, 1983 at approximately 1:00 P.M. 'Ihis deviation was reported to me on October 7, 1983 by Mr. Peter Ganguly of Carolina Pwer & Light Company at approximately 3:00 P.M. During the 'Ibicon with Mr. Ganguly, he stated that two piping subassmblies had been found at the jobsite that were fabricated from 3" Schedule 40 material (.216 wall) and should have been 3" Schedule 161 (.438 wall) .

The two subassemblies were identified as PC. Mk. A5-236-1-CS-265-1 and A5-236-1-CS-265-2, which were fabricated on SF&W Sales Order Q3302-3%

Sheets 21 and 22, respectively.

A review of applicable drawings on bbnday, October 10, 1983, at 8:30 A.M. , revealed that the two pieces had in fact been fabricated frm 3" x .216 wall material in lieu of 3" x .43b wall, as raIuired by Ebasco's Line Specification #SS-1.

Agrecment veas rer.ched on Wednesday, October 12, 1983 between Mr. Peter Ganguly (CP&L) and John Harris (SF&WOD) that the two pieces should be 8402170005 831017 )

returned to Southwest. Drawings Q3302-AK-21X and-22X were released to our shop,- on Friday, October 14,1983, .which will cause both sub -

assenblies to be fabricated and furnished frun the correct materials.

Southwest personnel are currently reviewing records to determine the cause and extent of this incident. mis review has revealed to date-the following pertinent facts:

1. W e isometric drawing for these pieces, #1-CS-265, was properly drawn on 8/6/79. We drawing was checked on 8/6/79 and was back-checked on 8/24/79 by our engineering personnel.
2. mis drawing shows a line spec break at a 1500# valve. Line Spec. {

SS-1, cxxning into the valve requires Schedule 160 material, while '

Line Spec. SS-3, on the cpposite side of the valve, calls for ,

Schedule 40.

3. Upon receipt of an Ebasco revision, SF&W personnel reviewed this isometric and added a note to install a transition piece, Sch. 160 to tratch the valve on one end, taper bored to Sch. 40 to match adjoinire pipe on the other end. At that time, an arrow was in-advertently drawn to show a transition piece required on both sides of the valve.
4. We isometric was subsequently checked and approved by SF&W, and subnitted to Ebasco for approval.
5. h e Resident Ebasco Engineer approved the drawing via Transmittal Hou-CAR-153A.
6. Detailers subsequently prepared ' fabrication drawings Q3302-AK-21 and 22, requiring the use of Sch. 40 pipe to match the transition piece shown in error on the SS-l' side of the 1500# valve.
7. l Consequently, both pieces were fabricated in accordance with Shop Detail Sheets and shipped to the jobsite on March '31,1980.

Additional significant information is that the Southwest personnel-involved April in this: incident were enployed from-January 1,1979 'through 10, 1981 employed at SF&WCO.

Southwest engineering personnel are in the process of reviewing all nuclear ismetric drawings on this contract; this revicw will be completed on or before October 28, 1983. Those drawings that show spec breaks where the pipe sdiedule changes will be reviewed care-fully for empliance.

Your offico, and our client, will be promptly notified of any other incidents discovered, and a supplemental report will be issued upon ccrnpletion of our investigation.
