Abnormal Occurrence The following occurrence was reported to Mr. Walt Baunack, A.E.C. Region 1 Regulatory Operations Office on December 19, 1974.

Written notification was made to Mr. James P. O'Reilly, Region 1 Regulatory Operations Office on December 19, 1974.

In accordance with Section 6.7.2.A of the Technical Specifications, Appendix A of DPR-56 for Unit 3 Peach Bottom Atomic Power Station, the following report is being submitted to the Directorate of Licensing as an Abnormal Occurrence.

Peach Bottom Atomic Power Station R.D. 1, Delta, Pennsylvania 17314 Identification of Occurrence:

Manual isolation valve from the "B" Core Spray Discharge Header to the " Stay Full system accumulator was found ' closed; this valve should be open.

Conditions Prior to Occurrence:

i Unit 3 at approximately 73X power.-

I Description of Occurrence:

1 While conducting ST 1.5 the "B" Core ' Spray' Logic System Func-tional Surveillance Test, it was not possible to pressurize between MOV-3-14-118 and MOV-3-14-128 to stroke the later valvo.

Inve stig at ion determined that the stay full system. vent solenoid, SV-5224, which opens -

on-low level,.was open. Further investigation found that the " Stay Full"

Mr. A. Giambusso Occcmber 28, 1974 Page 2 accumulator was valved out of service. The " Core Spray Line Vent Accumulator Low Level" alarm was also found to be inoperabic.

Designation of Apparent Cause of Orcurrence:

The last checkoff list shows that the val ve was open, and the last calibration on this accumuis ter was prior to the checkof f list.

The reason for the valve being closed was investigated, but was not determined.

Analysis of Occurrence:

The fact that this valve was closed has very littic safety implication because the system is pressurized continuously by the con-densate transfer system. When the system was returned to service it was judged that the piping system was full except for the accumulator.

Corrective Action:

The Unit 3 Core Spray System Valve Check-Of f List was redone; no other valves were found to be "off-normal".

The " Stay Full" system valving on both Unit 2 and Unit 3 RHR and Core Spray systems were veri-ficd to be alioned properly.

The icvel annunciator microswitch which failed to alarm the low level was repaired.

Failure Data:

Nonc previous on the " Stay Full" system.
