Abnormal Occurrence as 40-72-05 and confirming our telephone conversation -

During the af ternoon of September 21 with the reactor operating at approximately 151 power, the supply valve to the Off-Cas Sample Char:bar

[ was f ound to be c!nced. This prevented monitoring of the air ejector off-gau and constituted a violation of Technical Specifications, Section 3.8.A.1.

After discovery of the mis-pooitioned valve, it was-opened immediately and a tmnaurement was made using the installed cystem which ' indicated an

_I of f -ga.9 nctivity of 5 mr/hr. Additionally, a grab sample was taken which showed 145 yc/nec at the air ejectoro. The flow at that time was 15 cfm corresponding to a hold-up time of 300 minutes. The stack monitor during i

thin period indicated essentially at background.

The of f-gan monitoring nystem is currently operable in accordance with _

Technical Specif ications. A review of this incident is beirg conducted.
