DESCRIPTION OF OCCURRENCS:

In preparation for running S/R $.6.16-SA, (Sinulating Loss of Outside I

Pouer and Main Turbine Trip) a nalfunction of contacts in one Diesel-Generator nain breaker cauced the 252 TX relays (load shedding) not to operate, and prevented any further auto action.

ANALYSIS OF OCCURRENCE:

The malfunction of contacts on the "B" Diesel Generator uain breaker, was caused due to loose screws holding the auxiliary switch ncchanisn; there-Both fore the switch raised, irstead of actuating the rotor of the switch.

generators started and cane to speed and voltage but did not load automatically.

in the diccci-generator brec.hers. To cliniate the retnon nade failure, a redundent para 11cl circuit sensing bus voltage has been proponed to and approved by the Directorate of Licensinh.
