Dear Mr. Keppler:

The purpose of this letter is to provide you with notification of an Abnorsal Occurrence as required by Technical Specification 6.6.B.I.a within 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> of the occurrence.

With Unit 1 in the cold shutdssn condition core verification procedures revealed that fuel assembly CX90, at core co-ordinates 29-16, was rotated 180 from its proper orientation; and f uel assembly CX692, at core co-ordin:tes 53-12, uns not scated properly in the lower core grid. Following consultation with representatives of the General Electric Company, asserably CXC0 was, replaced by assembly CX505, which had similar exposure and was proved sound by sipping during the Unit I refueling outage in the spring of 1974 Assembly CX692 was removed from the core, inspected for damage, its spring clip replaced, and returned to the proper core location.

Iraproper loading and misverification at the conclusion of the last Unit I refueling 1

outage are the apparent causes of this occurrence.

This occurrence was discussed by telephone with Mr. Feicrobend of your of fice on February 12 and 13, 1975 Further information will be provided by Abnormal Occur-rence Report A0 50-254/75-4 which will be submitted within 10 days of the date of this occurrence.
