. . 1 P.EPORT NUMBER: A0 50-254/75-12 REPORT DATE: May 15, 1975 OCCURRENCE DATE: May 5, 1975 Quad-Cities Nuclear Power Station FACI LITY :

Cordova, Illinois 61242 IDENTIFICATION OF OCCURRENCE: ,

Unit 1 RCIC system inoperable. j CONDITIONS PRIOR TO OCCURRENCE :

Unit I was in the run rode at a core thermal power of 1291 MW and an elec-trical output of 393 MJ.

DESCRIPTION OF OCCURRENCE:

During the monthly surveillance. test of the RCIC system, on May 5,1975 at 5:37 p.m., the RCIC condensate pump was observed to have a high amp reading. The pump was taken out of service to be inspected and repai red,

. thus rendering the RCIC system inoperable.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Equipment Fai lure The ap' parent cause of this occurrence is designated as equipment failure.

The equipment failure was caused by defective motor windings that mal-functioned causing a high pump amp reading and in turn rendering the RCIC condensate pump motor inoperable.

ANALYSIS OF OCCURRENCE:

The RCIC system was inoperable whre the RCIC condensate pump motor failed.

During the period of the RCIC. system inoperability, the HPCI system was tested daily to insure its operability. This testing insured the ability to safely shutdown and cooldown the reactor. Thus the health and safety of the public were not Jeopardized by this occurrence.

A0 50-254/75-12 May 15, 1975 CORRECTIVE ACTION:

The RCIC condensate pump motor was tagged out of service and a work request (1625-75) was initiated to inspect and repair the malfunctioning motor.

The motor was sent to the General Electric maintenance shop in Bettendorf, Iowa for repairs. There the motor windings were rewound. The rewinding included using a higher grade of insulated wire than that requi red to be in the motor. The motor was re-installed and returned to service on May 8, 1975 at 7:00 p.m. The RCic system was subsequently tested satisfactorily.

FAILURE DATA: ,

No similar occurrences have occurred in the past. There are thus no safety implications based on cumulative experience related to this occurrence.
