Dear Mr. O ' Leary:

The purpose of this letter is to inform you of the details concerning the Unit i 2B torus to core spray pump room watertight door which was found open, thus i exceeding Technical Specification 3 7.C.2. This abnormal occurrence was previ-ously reported to you by telegram, and to Region til, Directorate of Regulatory "

Operations personnel by telephone and telegram on May 25, 1974.

PROBLEM AND INVESTIGATION At 1400 on May 24,1974, with Unit 2 having all rods inserted and reactor water

- temperature > 212 F, the Unit 2B torus to core spray pump room watertight door _

was discovered open with a water hose in the doorway. This rendered the 2B core spray loop inoperable in accordance with Technical Specification 3.7.C.2. The hose was removed and the door was closed, thus making the 2B core spray loop operable. l t

EVALVATION AND CORRECTIVE ACTION  !

Safety implicatiens a

The 2B core spray loop was only technically inoperable. At all times it was actually able to perform its intended function. This, in conjunction with the redundancy of design, causes the safety implications of this open door to be ,

minimal. ,

Determination of Cause and Corrective Action A water hose in the 2B core spray room to torus watertight doorway caused the door to be open. The hose was removed and the door was closed. A meeting was held with supervisors of contractor personnel working in the area and the re-quirements for maintaining closed watertight doors were emphasized. Provisions <

were made to use an alternate route for required hoses.
