Diesel generator no. I cooling water pump motor failure.

CONDITIONS PRIOR TO OCCURRENCE:

Unit 1 operating at 163 MWe.

DESCRIPTION OF OCCURRENCE:

On July 22, 1974, at approximately 7:00 p.m., when attempting to start the Unit One diesel generator cooling water pump, the breaker tripped repeatedly thereby preventing a pump start. The coolir.g water pump was to be started in order to provide water for the Unit 1 HPCI room cooler during HPCI flow rate operability surveillance and vibration testing.

pressure core cooling and containment cooling systems was done in accordance with Technical Specification 3.9.E.1.

A work reques: was written to investi-gate and initiate repairs.

A unit power reduction to the hot standby condition had been scheduled to commence following the HPCI surveillance in ord'er to change control rod pat-terns as part of the startup program following the refueling outage. Since the Unit I diesel ger.erator had been declared inoperable, an orderly shutdown to the cold condition was therefore initiated.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The initial investigation and inspection of the diesel cooling water pump re-vea led that the notor windings had shorted out. The high currents drawn by the shorted windings caused the breaker to repeatedly trip.

Further investigation revealed that shaft seizure had occurred due to failure of the notor bearings. Failure of the motor bearings was thus designated as the cause of this occurrence.

v generator being inoperable was performed immediately and was successful. All ECCS. systems were operable at the time of the occurrence.

Since the redundant systems were operable, the safe operation of the reactor was not threatened, and there were no effects on the health and safety of the public as a result of this occurrence.

CORRECTIVE ACTION:

Af ter the diesel and~ cooling water pump were rem'oved from service, the motor was removed and sent to a General Electric Company service shop for detailed inspection and rewinding.

A new 100 HP, 3525 rpm encapsulated motor was purchased and installed on July

23. The Unit One diesel generator was returned to service early on July 24.

No abnormalities were observed on 'the cooling water pump or the diesel itself.

Unit One startup continued following the operability test.

FAI LURE DATA:

This is the first instance of a diesel generator cooling water pump motor failure at Quad-Cities Station.

The Unit One diesel generator was previously run on July 10, 1974, and r.o prob-lems were experienced with the cooling water pump motor at that time.
