While doing the special weekly surveillance test on LIS-2-263-72D as per o'ur letterc of July 11, 1974, the switch failed to actuate.
The switch was lightly tapped and sdbsequently operated normally.
Special weekly surveillance of this switch will continue until a new switch is received and installed.
The switch has functiened normally on two previous special tests performed since July 11, 1974.
