Light Company Provisional Operating Licence No. DPR-16, Section 3. c.1..,

the following abnormal occurrence disclosed a condition that prevented a nuclear system frer performing ene of its safety functions.

On ch 25, 1970 during the ccurse of performing a curveil3nnce test on the f

reactor n.q;4 pressure scram pressure switches, it van observed that the sensing

[

line to the high pressure scrcm precoure switch had developed a leak at a.

" Swage-Lok" fitting, which caused a level indicator to fail-up scale. At this time, an attempt was made to tighten the fittJng and the 3eak increased, causing the excess flow check valve in the primary pressure sensin6 line to close.

Tne result was a zero pressure signal to the pressure sensors mounted on this rack.

(IIigh Pressure Scram, High pressure Isolation Condenser Actuation, Condenser Lov Vacuum Scram By-pass, Core Spray Valve permissive, Triple Lov Level Auto Lepressurination, Level Transmitter to Feedvater Control System, Reactor Pressure Indicator' Transmitter and Auto-Relief Valvo Pressure), At first investigation, it was noted that the Protective Instrumentation Liniting Conditions for Operation could not be mot, the system,re.stora vore notified to prepare for a plant shut-dovn and repairs to the tubin, fitting vere immediate3y started. ReFairs were completed prior to inid ating the shut-down.

(It was approximately 50 minutes from time of occurrence unLil system was again normal.)

-On March 25, 1970, AEC Compliance was notified of this incident and informed that notification was made upon the basis of the inabilitf to meet a Limiting Condition for Operation.

Upon review of thic incident by the Operations Supervisor and the P3nnt frating Review Ccanittee, it was detennined that the single _ failure of this O
