McGuire Nuclear Station Reply to a Notice of Violation Violation 50-369. 370/94-23-01 10 CFR 50 Appendix B, Criterion ill, Design Control, as implemented by the Duke Power Company Topical Quality Assurance Program, (Duke 1-A), Amendment 16 & 17, Section 17.3.2.2, Design Control, requires that design changes shall be subject to design control measures commensurate with those applied to the original design and approved by the organization that performed the original design. These design control measures shall be applied to plant activities which include maintenance and repairs.
Contrary to the above, design changes were accomplished which were not subject to design control measures commensurate with those applied to the original design. Seven examples were identified in which equipment or system design was changed during maintenance or repair without appropriate design control measures. Two of these examples were as follows:
Problem Identification Program Report (PIP) 1-M941300 identified on September 27, 1994, that unqualified arc welding cable was installed in safety-related 15 KVA inverters, 1EVIA,1EVID,2EVIA, and 2EVID. PIP 0-M94-0493 identified on April 4,1994, that the power terminal box upper seismic mounting brackets on three safety-related pump motors had missing or damaged bolts. The resulting mounting configuration deviated from the design mounting specification.
This is a Severity Level V violation (Supplement 1).

Reason for the violatisfr The reasons for the violation are failure to follow procedures and failure to recognize that maintenance or repair activities were performed without using design control measures commensurate with those applied to the original design.

Corrective steps that have been taken and the results achieved:
a.
Conceming PIP 1-M94-1300 (Example 1), cable replacement for 15 KVA inverters 1EVIA and 1EVID was completed during the 1EOC9 refueling outage. Item 3.a addresses cable replacement for inverters 2EVIA and 2EVID.
b.
Concerning PIP 0-M94-0493 (Example 2), the power terminal box upper seismic mounting bracket bolts were replaced for containment spray pump NS1 A. This corrective action was completed on April 15,1994.

'c A written communication was issued to all Maintenance personnel that revbwed the Notice of Violation and reinforced the need for personnel to strictly adhere to established design control measures during all maintenance activities. This corrective action was -
completed on December 9,1994.
l No similar events have occurred since implementation of these corrective actions.

Corrective steos that will be taken to avoid further violations:

Conceming PIP 1-M94-1300 (Example 1), cable replacement for 15 KVA inverters 2EVIA and 2EVID will be completed by the end of the 2EOC9 refueling outage.

Concerning PIP 0-M94-0493 (Example 2), the power terminal box upper seismic i
mounting bracket bolts will be replaced for containment spray pump NS2A and decay heat removal pump NDI A by March 31,1995, c.
The Notice of Violation and the need for personnel to strictly adhere to established design control measures during all maintenance activities will be addressed during i
scheduled maintenance shift training. This corrective action will be completed by May 1, 1995.
d.
The McGuire Safety Review Group will perform an in-Plant Review of McGuire's maintenance and repair process to determine the extent of plant equipment being altered without use of the appropriate design change process, the root cause of identified examples and to recommend process improvements. The in-Plant review will be completed by April 18,1995. McGuire management will review recommended process improvements and determine the improvements to be implemented by May 18,1995.
