SUBJECT:

SHNPP Unit No. 1 10CFR50.55(e) and 10CFR21 raportable item - Welding Defects in Pipe Support Frame C-1-195 ITEM:

Shop welding of pipe support frame.

SUPPLIED BY:

Bergen-Paterson Pipesupport Corp., Laconia, New Hampshire NATURE OF DEFICIENCY:

Frame C-1-195 was fabricated by PX Engineering in Boston, Massachusetts working under subcontract to Bergen-Paterson.

The frame was shipped in several large, shop-welded subassemblies.

In order to install these fabricated portions of the frame, it was necessary for us to cut through some of the shop welds. This revealed that weld preparations had been improperly done with torch cuts through the base metal, which had been concealed by the attaching members.

DATE PROBLEM OCCURRED:

DDR-1562 was written on May 23, 1983.

DATE PROBLEM REPORTED:

On June 6, 1983, Mr. N. J. Chiangi notified the NRC (Mr. A. Hardin) that this item was reportable.

SCOPE OF PROBLEM:

Large Pipe Support Frame C-1-195 SAFETY IMPLICATIONS: The damaged base metal which was revealed could have significantly affected the ability of the frame to provide necessary support during a seismic event. Other joints not investigated may have had similar or more serious defects.

CORRECTIVE ACTION:

The frame fabricated by PX Engineering will not be used, and no additional shipments from PX Engineering will be accepted.

Frame C-1-195 will be refabricated using certified material with welding performed on site.
