September 24, 1982 Rev. 1, October 7, 1982 Rev. 2, January 10, 1983 PLAN FOR PARTIAL LIFT SWA 10 FOR REFUELING BELLOWS In response to S&L's request per their letter SLS-I-3599 Attach-ment 1, dated August 26, 1982, we propose the following plan be implemented.
1.
To allow S&L to further technically evaluate the weld defects in the top & bottom horizontal flange plates and the bottom vertical skirt perform an ultrasonic inspection to determine location and size of defects.
See sketch Attachment 2.
2.
The ultrasonic testing will be performed by Southwest Research Institute under their existing QA program using their approved procedures which were used for IPC's pre-service inspection.
These procedures meet the specifications provided in S&L's letter.
(ASME Section XI and Section V).
3.
SWRI will prepare and submit to IPC and S&L a report which will provide the examination results plotted in the form of a weld map showing X,Y, and Z dimensions (size) of the indications and the X,Y &Z coordinates to the center (location) of the indications.
4.
To allow SWRI to properly perform the Ultrasonic examination, some welds will require minor grinding to achieve acceptable contours.
B.A. will be directed to do this work using appropriate Q/TS surveillance and paperwork.
IPQA will also perform random surveillance of the grinding & UT examination.
5.
Stop Work 10 will be amended to allow necessary work to perform UT examination.
6.
The plan for S&L and IPC evaluation and disposition of the results determined by the above non-destructive examination is found in Attachment 3.
This " flow chart" characterizes what step by step action will be followed after completion of the NDE.

Attached for your use in the recovery effort is the plan for dispositioning the drywell refueling bellows. As you will note from the plan, the " completion" date is dependent on the results of S&L's analysis and, if required, IP PMD's l
decision to repair or replace. The estimated dates for activities outside of engineering's control are based on my discussions with you.
