Shop Welds of Seismic Pipe Supports SUPPLIED BY:

Bergen-Paterson Pipe Support Corp., Laconia, New Hampshire NATURE OF DEFICIENCY:

During June and July of 1982, a number of shipments of pipe supports for SHNPP Units 1 and 2 sent from Bergen-Paterson Pipe Support Corporation were inspected by CP&L receiving inspection personnel. A significant number of welds were found to have deficiencies ranging from excessive undercut to missing welds. These deficiencies were detailed in a number of " Deficiency and Disposition Reports" (DDRs).

Bergen-Paterson and Ebasco were notified since the affected pipe eupports had been accepted on site based on a quality release signed by Ebasco personnel and the welds were assumed to have been inspected by Bergen-Paterson personnel. On July 6 and 7, 1982, Bergen-Paterson QA Manager Robert Stephens and Ebasco VQA Assistant Chief James Newell visited the site to discuss the problem and review the identified weld deficiencies. After this visit, it was agreed that a number of the reported deficiencies did indeed differ from the design drawings and exceed the allowable limits of Bergen-Paterson/s acceptance criteria.

DATE PROBLEM OCCURRED:

DATE PROBLEM REPORTED:

On August 13, 1982, CP&L (Mr. N. J. Chiangi) notified the NRC (Mr. H. Hardin) that this item was reportable under 10CFR50.55(e) and 10CFR Part 21.

SCOPE OF PROBLEM:

The total number of affected pipe supports has not been determined since inspection is not complete.

However, in a sample of three (3) inspection reports, 142 of 316 inspected supports had defects which were rejected by receiving inspection personnel.

SAFETY IMPLICATIONS:

Although analytical verifications were made on a number of the supports and none were identified which would have prevented a safe shutdown in the "as-inspected" condition, there was no assurance that a support not analyzed might not create an unsafe condition.

REASON DEFICIENCY IS REPORTABLE:

Breakdowns in Bergen-Paterson and Ebasco QA programs which allowed supports to be shipped with welds which were not in accordance with design criteria, mandated this incident be identified as reportable under 10CFR50.55(e) and 10CFR21.

CORRECTIVE ACTION:

Corrective actions involve two distinct efforts; 1) to assure that inspection at the Bergen-Paterson shop provides adequate assurance that welds are in accordance with design criteria; and 2) to identify and repair any welds on supports on site which do not meet design requirements.

To date, Bergen-Paterson has taken steps to increase the time spent on inspection and to provide additional training to welders to assure that welds are adequate.

In addition, they have revised their inspection criteria and some design practices to eliminate problems of interpretation.

Those supports which had been received on site and found deficient by CP&L inspection are being returned in truckload lots to Bergen-Paterson for reinspection and repair. Those supports which have been installed in the plant are being reinspected by CP&L inspectors and repaired. We are reviewing our weld inspection criteria to assure that any defects identified are significant with respect to design criteria.

FINAL REPORT:

A final report will be issued when the corrective actions described above are complete. We expect that the final report will be complete by July 1, 1983.
