BELLEFONTE NUCLEAR PLANT UNITS 1 AND 2 - BOLTS AND NUTS FAIL TO MEET ASTM SPECIFICATIONS - BLED-50-438/81-64, BLRD-50-439/81 THIRD INTERIM REPORT The subject deficiency was initially reported to NRC-0IE Inspector R. V. Celenjak on September 24, 1981 in accordance with 10 CFR 50.55(e) as NCR's 1597 and 1598. This was followed by our interim reports dated October 22 and December 8, 1981. Enclosed is our third interim report. We expect to submit our next report by June 29, 1982. We consider 10 CFR Part 21 applicable to this QPiciency.

ENCLOSURE BELLEFONTE NUCLEAR PLANT UNITS 1 AND 2 BOLTS AND NUTS FAIL TO MEET ASTM SPECIFICATIONS BLRD-50-438/81-64, BLRD-50-439/81-62 10 CFR 50.55(e)
THIRD INTERIM REPORT Description of Deficiency 10,000, 3/4" stainless steel nuts (furnished by Chattanooga Bolt and Screw, Chattanooga, Tennessee) and 10,600, 3/4" x 3-1/2" stainless steel bolts (furnished by Challenge Enterprises, Chattanooga, Tennessee) were received not marked in accordance with Afi'A specifications. Subsequent testing of the subject material by Singleton Materials Engineering Laboratory revealed the material failed to meet ASTM specifications. The associated

documentation received with the material erroneously shows that the nuts and bolts do meet ASTM specifications. Approximately 450 of these bolts and 1300 nuts have been issued to the field for installation.
Nonconformance repcet (NCR) 1597 pertains to the bolts and NCR 1598 pertains to the nuts.
Interim Progress Construction Mechanical Engineering Unit (MEU) QC has performed inspection in the areas designated as areas of usage for the describec hardware and the results are as follows:

22 bolts issued to steamfitters are believed to have been used in unit 1 spent fuel pool temporary flush piping, and cannot be

120 bolts issued to electricians for lighting standards cannot be accounted for by the electricians.

16 bolts are believed to have been used on the traveling water screens at the Intake Pumping Station but were not located.

16 bolts issued to steamfitters for the Decay Heat Removal System cannot be accounted for.
5.
100 bolts issued to m111 wrights are believed to have been used in Heat Rejection System motor bases and are possibly embedded in concrete.
6.
25 bolts and nuts are installed in waste tank and waste holding tank agitators. These have been located and are being removed.
7.
20 nuts were used for an alignment /fitup jig which is to be scrapped.
8.
22 bolts and nuts were returned to MEU by the Instrumentation Engineering Unit.
All items returned to MEU will be sent. to the warehouse for disposition.
TVA is continuing the searon for unaccounted for nuts and bolts.
