- 1. NARRATIVE
SUMMARY
OF OPERATING EXPERIENCES l
Limerick Unt 1 began the month of September at a nominal 100% of rated thermal power, l
On September 7, at approximately 1830 hours0.0212 days <br />0.508 hours <br />0.00303 weeks <br />6.96315e-4 months <br />, a reactor power decrease was begun. The reactor was manually scrammed at 2221 hours0.0257 days <br />0.617 hours <br />0.00367 weeks <br />8.450905e-4 months <br />, from approximately 32% power, to the third refueling outage Operational Condition (OPCON) 4 was entered on September 8 and 6was entered on SepteW2er 13. ON September 24, OPCON 5* was entered and the first fuel bundle was offloaded from the core. Limerick Unit 1 ended the month of September in OPCON 6* with approximately one-quarter of the core off-loaded, in day 23 of a scheduled 75 day refueling outage, j

t Operational events that occurred during the month of September included:

On September 16, D13 Diesel Generator (D/G) was run for its LOCA4.OOP test. As part
.i of the LOOP testing, the diesel was started on a bus under voltage signal. It automatloally closed onto the bus and loaded, however, a bus voltage of greater than 5.7 kV was noted.
Following voltage verification, the diesel was secured. The D13 sieguard bus and associated loads were declared inoperable. D13 safeguard bus and assochied loads were -
_l declared operable following various testing and receipt of an engineering evaluation on -
September 18. Following 013 D/G voltage regulator troubleshooting, repair of a suspect l
rectifier selector switch, and replacement of a shutdown relay, a test run was successfully performod on September 19. The LOCA/ LOOP test was next performed, resulting in a bus t
over voltage condition similar to the 9/15 event. The diesel was run several times during the following week for System Engineer and vendor troubleshooting of the voltage regulator, The D13 D/G was run satisfactorNy on September 28. The diesel was then started on -
t September 29 for performance of the LOCA-LOOP test. With the #1 rectifer bar* selected,
-l a bus over voltage condition occurred and the diesel was shutdown. The diesel was restarted using the #2 rectifier bank and the LOCA-LOOP testing was completed i
antisfactorly, The D13 D/G was declared operable on September 30.
II. CHAT i FNGES TO THE MA!N STEAM SAFETY RELIEF VALVES l
There were no challenges to the Main Steam Safety Relief Valves during the month of September.

SUMMARY
OF OPERATING EXPERIENCES Limerick Unit 2 began the month of September at a nominal 98% of rated thermal power. On j
September 1, reactor power was reduced to 00% to repair an Electrohydraulic Control (EHC) system i
leak on #4 Main Turbine Control Valve. Retum to power was begun on September 2 and reactor -
power achieved 100% on Septomber 5 following completion of LPRM onlibrations. On ember 8, reactor power was briefly reduced to 90% to perform Main Turbine Control Valve test
. On September 10, at 0911 hours0.0105 days <br />0.253 hours <br />0.00151 weeks <br />3.466355e-4 months <br />, the reactor automatically scrammed due to a Nuolear Steam upply Shutoff System (NSSSS) Group l isolation signal created during survet:ance testing.. On September 13, at 1815 hours0.021 days <br />0.504 hours <br />0.003 weeks <br />6.906075e-4 months <br />, the reactor achieved criticailty, and the main turbine generator was synchronized.-

Power ascension was continued and the reactor reached 100% power on September 16. On September 17, reactor power was briefly reduced to 90% to perform a Control Rod Pattom adjustment. On September 20, reactor power was briefly reduced to 95% to perform a Control Rod Pattom adjustment. Umerick Unit 2 ended the month of September at a nominal 100% of rated thermal power, Operational events that occurred during the month of September included:
i On September 10, a full NSSSS Group 1 MSIV isolation occurred due to an instrument failure. This occurred when a plant operator was reading the Turbine Enclosure Steam Tunnel Temperature D!V 4 switches in the Auxiliary Equipment Room, concurrent with l&C

testing of the NSSSS Main Steam line DIV 1 temperature instrumentation. Following the MSIV isolation, the reactor scrammed with the reactor vessel leve' 'xreasing to 48' and.
pressure rising to 1112 psig. HPCI and RCIC both started, but due to the short duration of I
the initiation signal, only RCIC injected to the reactor vessel. HPCI was run in the full flow test mode. The 'N' SRV opened initially on the pressure spike and the K, M, H, S and E SRV's were cycled to contral reactor pressure. The plant entered OPCON 4 at 2209 hours0.0256 days <br />0.614 hours <br />0.00365 weeks <br />8.405245e-4 months <br />.
On September 11, l&C replaced the faulty temperature switch (TTS-025-216D) and satisfactorily performed a cal / functional test.
