ATTN: Mr. J. A. Hancock
Vice President Nuclear Operations
P. O. Box 14042, Mail Stop C-4
St. Petersburg, FL 33733
Gentlemen:
Subject: Report No. 50-302/82-09
This refers to the routine safety inspection conducted by Mr. T. Stetka of this
office on March 27 - April 26,1982, of activities authorized by NRC Operating
License No. DPR-72 for the Crystal River facility.
Our preliminary findings were
discussed with Mr. T. Lutkehaus, Plant Manager, and other members of your staff,
at the conclusion of the inspection.
Areas examined during the inspection and our findings are discussed in the
enclosed inspection report. Within these areas, the inspection consisted of
selective examinations of procedures and representative records, interviews with
personnel, and observations by the inspectors.
During the inspection, it was found that certain activities under your license
appear to violate NRC requirements. These items and references to pertinent
requirements are listed in the Notice of Violation enclosed herewith as
Appendix A.
Elements to be included in your response are delineated in
Appendix A.
Item A in the Notice of Violation is a violation of 10 CFR 50.59.
As indicated
in Supplement I of the NRC Enforcement Policy, significant violations of this
type are normally classified at Severity Level Ill.
However, because the
reactor was not operated under conditions exceeding Limiting Conditions for
Operations, we have classified the violation at Severity Level IV.
>
One new unresolved item is identified in the enclosed inspection report.
This
item will be examined during subsequent inspections.
In accordance with 10 CFR 2.790(a), a copy of this letter and the enclosures will
be placed in the NRC's Public Document Room unless you notify this office, by
telephone, within ten days of the date of this letter and submit written appli-
cation to withhold information contained therein within thirty days of the date
of this letter.
Such application must be consistent with the requirements of
2.790(b)(1).
The responses directed by this letter and the enclosures are not subject to the
clearance procedures of the Office of Management and Budget as required by the
Paperwork Reduction Act of 1980, PL 96-511.
