SUMMARY

I Examinations were administered during.the week'df.-Septhmber,17,.

1990. This examination differed from a normal:requalification exam in that no simulator crew operating tests were administered.

Five additional Job Performance Measures; (JPMs) were administered to each individual in lieu of the simulator exams..

Requalification written examinations and operating tests were administered-to nine Senior Reactor Operators (SRos) and three l Reactor Operators (Ros). All of the operators : passed these examinations.

.5 Based upon the above results, the Crystal River Requalification Program is assigned an overall' rating of Satisfactory.

During the~ week of August 27, 1990, theLexamination team met with facility personnel to prepare the final version of the administered exam. This examination differed-from.a normal- 4 requalification exam in that' no: simulator crew operating tests were administered. Five additional JPMs were; selected-in lieu of the simulator exams. The' examination. team conducted a thorough review of all-test items and:made o recommended changes:to the' facility. These changes / comments I were' incorporated into the final version of the examination-by the facility.

4. Exit Meeting l= At the conclusion of the site visit,.theLChief Examiner met I with the plant staff and management' representatives to-discuss the results of the. examinations. Significant items--

which were identified by the= examination--team during the development and implementation of.the exam were shared withi plant management. l Specific concern was noted regarding the numerous changes -

that had to be made to the examination. materials by the:NRC l examination team.' hajor revisions were required on both the'  !

written tests and the JPMs to achieve.an acceptable levelJof quality. It is anticipated that future endeavors-in the development of examinations will yieldisignificantly.. improved'. i results.  !

No generic weaknesses were noted during the performance of the JPMs. Additionally, the' cooperation and effort devoted j towards.every aspect of-the examination process'by the-  !

REQUALIFICATION~ PROGRAM: EVALUATION-Referen.ce Material Reauirements The reference material supplied by.the facility was reviewed to determine its adequacy for the' examination. The facility-supplied an adequate number of open> reference questions. The Section "A" Bank contained 100 questions and was' required to remain unpublished'. The Section "B"' Bank: contained.the required minimum of 350 questions. .The' facility also-submitted ~75 Job 1 Performance Measures. A sufficient amount of' additional: '

reference.and'requalification training material was provided to the examination team members. In addition, the facility submitted a complete examination package containing=the proposed exams and sampling plan. This package was reviewed and" modified; byt the NRC Examination Team prior to. exam administration.

l Examination Preparation I NRC Examiners met with the facility training staff during-the week of August 27, 1990, to develop the final draft-of the exams'. ,

1. The facility sampling plan submitted with.the examination:  !

package did not meet the requirements'of'ES-6011(Revn5)'. The l sample plan indicated the emphasis: which each topic received  :

during the requalification. cycle, however, there was.no correlation between this-and the selection of questionstfor 1 the exam.

Furthermore, there was no system.in' place to ensure that different Knowledge and Abilities (KAs)ohad been selected for testing, to ensure a balanced.and comprehensive-coverage of topics. This was. evidenced by the factothatEone written test contained three~ essentially identical-questions on Diesel Generator trip interlocks testing.theisame:KA.

Furthermore, the actual breakdown of; topics;for the~ exam did-not agree with the' listed percentages on the plan. EIn addition, there had been no effort to correlate 1 topics on1the wr.tten exams with those covered -in the JPMs or the 'JPM' '

2. The written exam consisted of 100! percent multiple choice ._

questions. Extensive. changes were made_to these.questionsDin order to ensure proper content and validity'of the examina-tion. Many of the test items had only a tenuous relationship to the referenced learning objective.- This may be- due to- the -

fact that the learning objectives were very broad in scope and few in number. Also, numerous editorial changes were  ;

made involving rewording for clarity ~and incorporating the-  !

guidelines set ~forth,in NUREG/BR-0122. The time validations for both exams appear to'have,been reasonably accurate.

3. The NRC reviewed, those JPMs that' had- been selected- by the .

~

t facility and-made numerous changes / comments-to-upgrade:the l JPM quality._ The team also substituted'several JPMs because i those that had been proposed were eithar too' simple or-overlapped topics covered in the: written portion of.the exam; or in other JPMs. JPMs.were selected to broaden the! spectrum-of job tasks sampled. Generic problems found and' corrected within the JPMs were:

- JPMs lacked statements of. initial; plant conditions which should be given to the examinee.

- Performance standards were not specific and, in some cases, did not refer to actual equipment nomenclature.

i L - Several JPMs differed significantly from the currently .[

l- approved procedures. JPMs had not been revised ~to reflect: -i procedural modifications.

- Sequence critical steps were not' identified where.

applicable.

- Critical steps were not well defined and shoulo not include i

" observe" or " verify" steps since their. performance is difficult to ascertain.

- Cues given to the examinee were generally unsatisfactory.

The cues should be performance based ~and give only the; information that the~ operator- would physically observe- if he were to actually perform the task.

- JPM questions needed many' revisions. The questions, in many cases did not elicit the answer given in the JPM.

Additionally, the questions did not. emphasize knowledge-required for task performance that was not' demonstrated during the execution of the JPMs.

i L Examination Administration The facility's administration of the examination was satis-factory. The facility evaluators performed. adequately with"no major generic problems noted.. Therefwere, however, minor instances of evaluator performance that were inappropriate and.

not in accordance with NUREG 1021.

1. On several occasions evaluators were observed to have given

, excessive verbal cues when administering the JPM questions.-  !

l Making-statements such as "anything else", or "are you'sure",' I' assists:the examinee in delivering the answer. Evaluators.

. are there to evaluate, not assist.

2. Evaluators were not always. familiar with the JPMs and j experienced 2 difficulty in locating indications that werespart i of the cues for the examinee. Evaluators need to know the- 1 JPMs so that they can deliver'the cues efficiently, and-not j' appear unprepared.
3. Evaluators need to be more cognizant of their non-verball [

cues. For example, the examinee should be the first to make '

a move towards another location or piece of equipment, not' >

the evaluator. i

4. Evaluators should corroborate with each other to:make sure that they are asking the same' number of questions-(usually-two) per JPM to 'each individual in order to 'make the1 exam more equitable. Evaluators were inconsistent in.that some l would ask three questions and others onlystwo for.the same JPM. In addition,. follow-up. questions to= clarify an answer for a previous question should not be counted as-a separate-question. Additional documented questions to probe a

~

weakness identified during the performance of a.JPM should be-counted as additional questions.

Recualification Procram Evaluation l The Crystal River requalification program was evaluated based on the applicable criteria set forth inLNUREG 1021, ES-601. Since no simulator evaluations were conducted, only the following-  ;

criteria applied:

1. Ninety percent pass / fail decision agreement between the NRC and facility grading of the walk-through and written examinations.

2. At least 75 percent of all operators' pass the walkthrough and written examinations.

Analysis of the examination'results indicates'that the:above criteria were met satisfactorily. 'Accordingly,.the Crystal-River--
