i Office of Nuclear Reactor Regulation Items of Interest Week Ending November 5, 1993 Brunswick Units 1 and 2 The licensee is preparing the setup for the electric discharge machining (EDM) equipment that will be used to machine the bolt holes for the core shroud clamp modification. The EDM will start on November 6, 1993, and the modification should be completed by December 6, 1993. The core shroud repair, control blade installation, and core reload remain as the critical path activities for the restart of Unit I scheduled for early January 1994.

Palo Verde The Unit 1 eddy current inspection has been compieted. Tube plugging is almost complete.

Steam genera +. 21 has 56 tubes plugged this outage; steam generator 12 has 131 (these numbers include tubes plugged for preventative reasons). No freespan axial indications were identified (similar to the numerous indications found in the " arc" on the most recent Unit 2 inspection).

However, numerous tubesheet circumferential cracking indications (almost all in the sludge pile) were identified.

The licensee's eddy current report was received on November 3, 1993. A meeting is planned for November 9 with APS ar.d Region V to discuss the results.

The licensee is still preparing its eddy current inspection plan for the upcoming Unit 3 outage (plant shutdown expected in late November). The Unit 2 mid-cycle inspection is scheduled for early January 1994.

i Palisades This highlight is an update of the Palisades activities regarding the pressurizer nozzle crack.

At 11:15 a.m. on November 3,1993, the Palisades reactor reached criticality.

This is the first time the Palisades reactor is at power since their shutdown i

for a refueling outage on June 5.

Due to problems during the outage with fuel handling activities and a crack in a pressurizer nozzle, the outage was extended several months.

The staff is continuing its review of the licensee's engineering analyses regarding the pressurizer nozzle crack and plans to issue a Safety Evaluation later this month.

System 80+ SGTR lssue r

On October 27, 1993, the staff met with ABB-CE to discuss the System 80+ issue of containment bypass that may result from a steam generator tube rupture.

l The discussion focused on the enhanced features of System 80+ over the t

System 80 designs regarding mitigation of Steam Generator Tube Rupture (SGTR) events, especially the design change to ensure continuous operation of the Turbine Bypass Control System (TBCS) upon a Safety Injection Actuation System (SIAS) actuation, and the installation of N-16 radiation monitors in the main t

2 steam lines. These and other enhancement features provide the operator with better diagnostics for the SGTR events, and provide the operator with extended time before the main steam safety valves (MSSVs) are challenged in the event of a SGTR.

The intent of these changes is to reduce the probability of operator errors in the SGTR mitigation and recovery actions.

The staff concludes that these improvements should result in a significant reduction in challenges to the MSSVs; and, therefore, this issue is technically resolved.

As a follow-up action, ABB-CE will provide more detailed information for staff review related to the TBCS operability after an SIAS actuation, a comparison of system response to a SGTR event for the System 80+ design with and without these enhancements, and updated emergency procedure guidelines that properly reflect the new design features.

NUMARC-Industry Thermo-lea Workshop On December 1 and 2, 1993, the Nuclear Management and Resources Council (NUMARC) will hold an industry workshop on Thermo-Lag fire barriers in Baltimore, Maryland.

NUMARC will present the results of Phase 1 of its fire test program and will discuss future plans.

NRC staff will attend the workshop.

Emergency Preparedness Counterpart Teleconference On November 2,1993, PEPB conducted the monthly telephone conference with the Emergency Preparedness groups in all five Regions.

The following topics were discussed:

impending FEMA reorganization, guidance on emergency worker dose limits, and experience with accountability drills at power plants.

Poor performance in accountability drills is on the list of issues for restart at South Texas.

It was determined during the conference that some other plants in other Regions are having performance problems in this area.

PEPB agreed to examine the issue of acceptance criteria for accountability drills.

ACRS Briefina on ABWR instrumentation and Control System desian On November 2, 1993, GE Nuclear and the staff of the Instrumentation and Controls Systems Branch (HICB), NRR briefed the ACRS ITAAC Subcommittee on the final ABWR instrumentation and control (I&C) system design, associated design certification material and the staff's acceptance of the design for certification under 10 CFR Part 52.

GE representatives described the basic 1&C system architecture, functional design requirements, and the associated ITAAC for implementation of the design in accordance with an approved digital system design development process. The HICB staff described the criteria and basis for its acceptance of the ABWR design including the features provided to meet the Commission approved digital 1&C system diversity position.

Material Control and Accountino Insoection at ABB Combustion Enaineerina During the week of October 25-29, 1993, the Inspection Section of the Operations Branch performed a material control and accounting (MC&A) inspection at ABB Combustion Engineering Inc., located in Windsor, Connecticut. Manufacturing activities have ceased at the facility and clean-out operations have been initiated. The primary purpose of the inspection was j

to determine the current status of the plant and the material quantities on i

hand.

In addition, the MC&A activities of previous operations were critiqued.

Areas of the facility toured included the production area, the laboratories and the storage vaults. Six inspection modules were addressed.

Claiborne Enrichment Center i

The NRC staff has completed the manuscript for the Draft Environmental Impact-l Statement (DEIS) for the Claiborne Enrichment Center, to be built and operated by Louisiana Energy Services, L.P., near Homer, Louisiana. The DEIS is now in the publication process and is expected to be printed and distributed for public review and comment before Thanksgiving. A notice of availability will subseruently be published in the Federal Reoister.

(The DEIS will be published as NUREG-1484.)

Included in the DEIS are assessments of the impacts of construction, normal operation, potential accidents, and the eventual l

decontamination and decommissioning of the site.

Technical Assistance Contractors Travel to Licensee Facilities During the period October 13-28, 1993, the Nuclear Regulatory Commission sponsored travel by two members of the Savannah River Technology Center (SRTC) of the Department of Energy to three NRC-licensed fuel fabrication facilities.

The SRTC is currently under contract with the NRC to develop, among other 1

things, a guidance document on Integrated Safety Analysis.

The travel was intended to provide an opportunity for the SRTC members to familiarize themselves with typical fuel cycle facilities, to obtain first-hand information on the methods of safety analysis used by the facilities, and to assess their capabilities for performing Integrated Safety Analyses. The i

facilities visited were those of Westinghouse Electric Corporation, Columbia, South Carolina; Babcock and Wilcox Company and B&W Fuel Company, both at j

Lynchburgh, Virginia; and General Electric Company, Wilmington, North Carolina. An NRC staff member accompanied the SRTC members, and plant tours and discussions with the facility staff were held at each stop.

Public Meetina on Part 70 Standard Review Plan A public meeting was held at Nuclear Regulatory Commission Headquarters on November 1,1993, for exchange of information between the NRC and fuel cycle 4

licensees and other interested parties relevant to the NRC's development of the acceptance criteria sections in the Standard Review Plan (SRP) document.

The SRP document provides guidance to NRC staff who review applications for licenses, under 10 CFR Part 70, to produce enriched uranium or to use it in 4

2 fabricating reactor fuel.

Specific topics discussed at this meeting were the i

SRP chapters related to Fire Protection and Emergency Preparedness. This j

meeting is the first in a series of public meetings that will be held for licensees and other interested parties to provide information on the various acceptance criteria sections in the SRP document. The next meeting is scheduled for November 8-9, 1993, at NRC Headquarters.

f Shieldallov Metalluraical Corooration On November 3,1993, the Division of Fuel Cycle Safety and Safeguards met with i

representatives of Shieldalloy Metallurgical Corporation (Shieldalloy) to discuss Shieldalloy's September 2,1993, decision to file under Chapter 11 of the U.S. Bankruptcy Code. Although environmental issues such as decommissioning work, had an impact on their decision to file, they were not 1

the driving force. However, unknown decommissioning costs are a major factor hindering Shieldalloy's attempts to create a reorganization plan. Under-bankruptcy law, Shieldalloy must present the bankruptcy court with a reorganization plan within 120 days of the filing.

The NMSS staff notified Shieldalloy that no decision on the ultimate disposal of the material could be made within the 120 day period, but that the staff should be able to notify Shieldalloy whether or not they would consider any specific alternative before the end of the period.

Acoroval of the YNPS Steam Generator Transport Packaae

On October 28, 1993, the Storage and Transport Systems Branch issued i

i Certificate of Compliance No. 9256, for the Yankee Nuclear Power Station 4

(YNPS) Steam Generator as a transport package for low specific activity j

radioactive material. Yankee Atomic Electric Company (YAEC) is planning to i

decommission Yankee Nuclear Power Station near Rowe, Massachusetts.

plans to transport the four steam generators by rail for disposal at Barnwell (South Carolina). The YNPS steam generator is filled with low density l

concrete on both primary and secondary sides, and nozzles and penetration are covered with welded closures. The package weighs approximately 275,000 pounds.

On November 1-2, 1993, a regularly-scheduled meeting of the Advisory Committee on Medical Uses of Isotopes was held in Reston, Virginia. Topics at the meeting included:

a discussion of 10 CFR 20.1301, regarding patients as j

members of the public; the impact of fees on medical programs; medical i

indications associated with calibration of Strontium-90 eye applicators and' their use in brachytherapy; the Memorandum of Understanding between the Nuclear Regulatory Commission and the Food and Drug Administration regarding the medical use program; an overview of the management plan for the medical use regulatory program; a discussion of the term " referring physician"'as used-in 10 CFR Part 35; and status reports on High Dose Rate Afterloader programs, implementation of the quality management rule, the NUREG on Management of Radioactive Material Safety Programs at Medical Facilities, the National Academy of Sciences study on medical regulation, and rulemaking. efforts f

Status of Exploratory Studies Facility (ESF) Construction Activities l

The NMSS On-Site Representatives (0Rs) conducted an on-site review of the f

starter tunnel activities at the ESF North Portal on Wednesday, October 27, i

1993. Two kinds of construction activities in the starter tunnel will I

continue through the first quarter of FY94:

(1) construction of the first i

test alcove and (2) installation of instruments for monitoring and recording effects of construction. The test alcove is advancing by the drill and blast method and reached a penetration depth of about 40 ft.

The ors were briefed in the field on the implementation of DOE's construction 2

monitoring (CM) program for ESF operations, in accordance with Study Plan

8.3.1.15.1.8, "In Situ Design Verification" and Job Package 92-20D, Rev. O, l

" Construction Monitoring in the Starter Tunnel." CM methods to be installed

to detect rock instabilities include multi-point borehole extensometer l

measurements, steel-tape extensometer point measurements and borehole

seismometers. The CM principal investigator and crew from Sandia National Laboratories demonstrated calibration tools and walked through their f

scientific notebook and methods for performing and documenting their i

scientific investigations.

Meetina with Envirocare on License ADolication Analyses On November 4,1993, Office of Nuclear Material Safety and Safeguards (NMSS) staff met with Envirocare of Utah, Inc. (Envirocare) to discuss possible changes to operations at its proposed 11e.(2) byproduct material disposal facility from those presented in its current license application. These changes are to allow more operational flexibility due to customer i

considerations. During the meeting, Envirocare discussed possible actions it could take that would justify changes in the operating parameters incorporated i

in the license as conditions.

It was emphasized to Envirocare that the license conditions were based on the information presented in its license application, and that if Envirocare wanted to obtain more operating flexibility, it needed to present additional analyses establishing new i

operating bounds. Envirocare stated that the information presented in the application was intended to demonstrate expected operating situations, and was not intended to set bounds on operations.

NMSS management explained that because the situations presented in the application are the basis for analysis of the facility, these become the bounds placed on operations.

Envirocare stated that it understood this position, and agreed to determine what analyses would be needed to justify different operating parameters.

4 Office of Nuclear Regulatory Research Items of Interest Week Ending November 5, 1993 Meetina of JCCCNR$ W.G.12 on Nuclear Plant Aoina and Life Extensips During October 25-29, 1993, the Joint Coordinating Committee for Civilian Nuclear Reactor Safety (JCCCNRS) W.G.12 on Nuclear Plant Aging and Life Extension conducted its 4th (93-1). meeting in Bethesda/Rockville. Seven Russian delegates with technical expartise in strength of materials, erosion-corrosion, cable diagnostics, advanced VVER reactor designs, residual lifetime evaluation, designs and problems w th Russian steam generators contributed to i

the activities of the W.G.12.

The Russian side presented twelve (12) technical papers related to plant &ging and life extension. Dr. Bougaenko, the Russian co-chair and the Director of the newly formed Engineering Center of Atomic Equipment Strength, Reliability and Lifetime of the Russian Minatom discussed the Russian National Research Programs on NPP Equipment Lifetime Assurance. The major functions of the Engineering Center include: Addressing issues related to strength, material science, and corrosion; support the development and coordination of appropriate regulations essential for NPP components design, production, and operation; conduct necessary research and develop technical basts in support of regulations; develop data base including codes and material properties; integration of Russian activities, domestic and foreign.

Four out of the original planned six tasks for W.G.12 have been completed.

The working group considers the tasks on Cable Agings and Residual Life Prediction important and continuing. The working group proposed a new task on Translation and Comparative Analysis of the selected articles of the ASME Boiler and Pressure Vessel Code. The level of effort, costs, and specific details of this task are being reviewed by the U.S. side prior to approval.

The Russian side cor F iers this task extremely important to support the activities of GAN,.he Russian regulatory authority.

Furthermore, this comparative analys'; will provide the basis for developing uniform guidelines and regulations to implement them at plant level and maintain safety of the operating nuclear power plants.

At the joint meeting of the JCCCNRS W.G.3 and 12 Dr. V. Kovyrshin represented i

Ukraine.

He anticipated more active participation by the Ukrainian delegates in future meetings, both the W.G.3 and 12.

JCCCNRS Workina C-up 3 The 5th meeting of the JCCCNRS Working Group 3 on Radiation Embrittlement, Structural Integrity, and Life Extension of Reactor Vessels and Supports was held in Bethesda and Rockville on October 26-29, 1993. St.ven Russian delegates from three organizations and one Ukrainian participated in this meeting and were led by A. D. Amayev, of RNRC-Kurchatov Institute. Nineteen U.S. representatives, led by L. C. Shao, Director, Division of Engineering, RES, participated in this meeting. The working topics of this meeting included (1) radiation embrittlement of pressure vessel steels--evaluations and predictions, (2) radiation embrittlement--analysis and mechanisms, (3) radiation embrittlement mitigation using thermal annealing, (4) studies of fracture toughness of reactor vessel material, (5) reactor vessel integrity NOVEMBER 5, 1993 ENCLOSURE C

i evaluation, and (6) elastic-plastic fracture toughness testing. The working group discussed details of-a cooperative exchange programs on irradiation and 9

annealing studies and a round robin testing program on fracture testing. The l

working group has proposed that several Russian scientists work at Oak Ridge National Laboratory on advanced studies on irradiation embrittlement and the-study of microstructures using advanced microscopy equipment and techniques.

i Advance Notice of Proposed Rulemakina on Desian Certification Rules 5

On November 3,1993, an Advance Notice of Proposed Rulemaking (ANPR) entitled

"Rulemakings to Grant Standard Design Certification for Evolutionary Light j

Water Reactor Designs" was published in the Federal Reaister (58 FR 58664) with a public comment period expiring on January 3,1994. This ANPR seeks public comment on the staff's proposal for providing through rulemaking i

standard design certification of evolutionary light water reactor designs.

The Commission has received four applications for design certification pursuant to the requirements in Subpart B of 10 CFR Part 52.

It is expected that two of these applications will be at the proposed rulemaking stage of r

design certification by June 1994.

This ANPR has been published to provide i

the opportunity for early public participation in the development of design

certification rules.

Diaanostic Evaluation and Incident Investiaation Branch (DEIIB)

i On November 2, the Quad Cities Diagnostic Evaluation (DE) Team Leader briefed the EDO and other NRC senior managers on the team's findings, conclusions and l

root causes of performance issues. The formal exit meeting to discuss the DE results is scheduled for November 8, 1993, 12:00 p.m. at the Quad Cities site.

The meeting will be open for public observation.

Technical Trainina Center (TTC) 1 The Shoreham simulator (BWR/4 design) has been acquired through a contract l

with Long Island Lighting Company (LILCO). The simulator has now been moved from the LILCO site to the S3 Technologies Company's facility in Columbia 1

Maryland. The simulator move contract with S3 Technologies provides for i

3 months storage while space at the TTC is being acquired and includes the provision for additional. storage time, if needed.

Efforts continue with GSA l

to acquire suitable space within the TTC building. Once the Shoreham simulator is operational at the TTC, the major reactor technology training programs in the General Electric design will be shifted from the-BWR/6 to the BWR/4 base.

TTC staff members provided technical support to the Human Factors Branch of RES by providing simulation data for a research project to study ways of measuring cognitive workload that may be associated with operator needs to navigate through hierarchical display screens that may be presented through i

j advanced control room interfaces. The simulation data obtained will be used to enhance the realism of the testing scenarios which will be accomplished through a series of laboratory studies using a part-task simulator at ORNL.

On November 5, management from AE0D briefed the ACRS'on the existing NRC Technical Training Program. The briefing described the training resources, current needs reevaluation, TTC facilities, staff qualification and training programs, existing curriculum areas under the TTC purview, simulator programs, and high technology training enhancements being used or developed.

In addition to providing information for members new to ACRS since the last-similar briefing, AE0D management was seeking a more detailed understanding, future comments, and continued dialog with the academic members of ACRS and l

ACNW. A similar briefing was provided to the ACNW on October 29.

Incident Response Branch (IRB) l On November 1, the Department of Energy (DOE) helicopter, under an NRC

]

contract, commenced an aerial survey of Neutron Products, Inc. (NPI), in.

Dickerson, Maryland. The aerial survey was initiated in response to Cobalt-60 contamination in and around the NPI site. The survey of the Dickerson site is about 90 percent complete as of November 4, The NRC is in the process of evaluating the need to conduct additional surveys at locations where NPI may NOVEMBER 5, 1993 ENCLOSURE D

have dumped waste and at the Blue Plains sewage treatment plant which serves l

l Neutron Products facility. The NRC in conjunction with the State of Maryland has surveyed effluent from the Blue Plains plant and has measured no 3

contamination likely to have originated from the NPI facility.

On November I, an IRB staff' member and the IRM Office Director participated in i

the Communications Emergency Preparedness Seminar sponsored by the National a

Communications System (NCS). The meeting was attended by high level officials from agencies with major roles in response to an emergency.

It was clear that there is a tremendous increase in the activities and capabilities associated with communication support during an emergency. This is in direct response to l

the lessons learned during recent natural disasters. The communications capabilities available are vast and complex.

IRB will continue to monitor i

these activities and assure that the NRC response program takes advantage of these advances.

Office of Administration items of Interest Week Ending November 5, 1993 Federal Fire Forum On November 1, 1993 employees from the Division of Contracts and Property Management attended the Federal Fire Forum in Gaithersburg, Maryland.

Representatives from Federal, State and local agencies participated in the forum which was sponsored by the National Institute of Standards and Technology. The lectures included information on the status of technical fire issues and the systematic approach for fire risk assessment.

Enerav Conservation Awards NRC has been awarded two energy conservation awards sponsored by the Department of Energy and the Federal Interagency Energy Policy Committee.

These awards were in recognition of conservation efforts by the Office of Administration in the following areas at OWFN:

(1)

The reduction of energy use at One White Flint North by installing high efficiency lighting, using heat exchangers for free cooling and incorporating energy conservation requirements into our commercial facility nanagement contract; and (2)

The reduction of water consumption at OWFN by reducing the lawn watering schedule, installing water saving devices in restrooms and upgrading the float values in the cooling towers.

These awards were presented on October 29, 1993 at a ceremony held at the Dirksen Senate Office Building. NRC was one of fifteen organizations receiving the energy efficiency award and one of five organizations receiving the water conservation award. NRC was the only organization to be the recipient of two awards.

Crime Prevention presentation The Division of Security has scheduled a crime prevention presentation to be held on November 18, 1993 at One White Flint North. Representatives from the Montgomery County Police, GSA's Federal Protective Service and Metro Transit Authority Police will be on hand to address crime prevention and personal safety issues.

Office of Information Resources Management Items of Interest Week Ending November 5, 1993 IRM Customer Support Center (CSC)

The Office of Information Resources Management (IRM) CSC began operations on November 1,1993. During the first two days of operation, the CSC received over 330 calls for service. Reminder notices were sent via e-mail to NRC Headquarters staff advising them to call the new IRM CSC for service.

In addition, a notice was published in the NRC Weekly Announcements on November 2, 1993.

ADP (Automated Data Processing) Coordinators were briefed on the new CSC during the week of October 25, 1993.

Internet Access The Office of Information Resources Management has completed the implementation of a "firewall" security scheme that will provide NRC users with full access to Internet mail, file transfer, and connectivity services while safeguarding the NRC network environment from unauthorized access.

NRC users will be able to access the Internet from Agency Upgrade of Technology for Office Systems (AUTOS)

I and high-performance engineering workstations. An initial pilot group has been i

implemented to begin testing Internet mail access from AUTOS.

A larger pilot group will be implemented in December.

Computer Security Activities The Codes and Standards Section staff reviewed six Certification and Accreditation packages for sensitive unclassified microcomputer systems and sent acceptance memoranda to the submitting offices.

Sianificant Enforcement Actions A Notice of Violation and Proposed Imposition of Civil Penalty in the amount of 52,500 was issued on November 3,1993, to Memorial Hospital of Cambridge, Minnesota, for one Severity Level III problem associated with a substantial failure to implement the Quality Management Program (QMP) as demonstrated by a failure, on multiple occasions, to develop written directives for the administration of radiciodide 125 and/or 131 in quantities in excess of 30 microcuries and a failure to instruct supervised individuals in the content and use of the QMP. The base civil penalty for a Severity Level III problem is 5 2,500. The penalty was escalated 50 percent for both the identification and corrective action factors.

Full mitigation (100%) was warranted for good past licensee performance.

The other factors were considered and no other adjustments were deemed appropriate.

A Notice of Violation and Proposed Imposition of Civil Penalty in the amount of $25,000 was issued on November 5, 1993, to Richardson X-Ray, Inc. The action was based on violations found in operations conducted on federal property under reciprocity, including failures to perform adequate surveys, to properly post high radiation and radiation areas, to provide training to an assistant radiographer, to block and brace material during transport, and failure of an assistant radiographer to wear an alarm ratemeter.

The improper survey and ratemeter violations were assessed at Severity Level III and were repeat violations.

Both violations were escalated for NRC identification and for poor past performance.

In addition, Demands for Information are being issued to the licensee and to the radiographer.

conference room cabinets, glass lobby doors, vending area and coffee station sinks and cabinets.

On November 2, the developer delivered a signed Supplemental Lease Agreement j

between White Flint North Limited Partnership and the Government covering the 18,000 square feet of " retail" space on the Plaza Level (Floor 1) for use by the NRC.

On November 3, the developer's representative announced that he had preliminary discussions with interior design firms and mechanical / electrical j

systems designers concerning the pending lease and build-out of the TWFN Plaza level for the NRC and hoped to have selection of the design contractors l

accomplished by December.

On November 3, at the weekly progress meeting, the developer's representative-announced that he had revised the OMNI completion date for floors 4 through 10 to November 22 and the date for floors 2 and 3 to December 15.

Also on November 3, the Director notified GSA and OMN1 that card reader wiring conduits from the plenum junction boxes to the wall mounted card-reader boxes l

throughout the interior should be 1" conduit instead of the 1/2" flexible conduit being installed. This item will be checked during the punch-list j

review before acceptance by the government.

On November 4, milling of the perimeter driveway commenced in preparation for l

applying a final layer of asphalt on a weekend as soon as weather permits.

Utility Radioloaical Safety Board of Ohio Meetina Roland Lickus, Region III State. Liaison Officer, participated in a Utility Radiological Safety Board (URSB) on Ohio meeting on November 1,1993 in Columbus, Ohio.

Mr. Lickus discussed the recent Perry management meeting on improving plant performance, dry cask storage at Davis-Besse and the status of NRC activities related to the review of the 2.206 petition process, the enhanced participatory rulemaking on decommissioning criteria and planned workshops with States on license renewal issues.

The URSB received reports on its NRC inspection observation program, and updates on its Citizen's Advisory Committee 1

(CAC) recommendations on NRC's standard design certification process for advanced i

nuclear reactors and spent fuel dry storage cask temperature monitoring. The CAC also provided comments on NRC's 2.206 petition process. The URSB also received an update on the Blue Ribbon Commission to site a low-level radioactive waste disposal facility in Ohio and reports from utility representatives on activities at Perry and Davis-Besse.

Well Loaoina Course The Office of State Programs sponsored a course entitled " Gas and Oil Well Logging for Regulatory Personnel" on November 1-5, 1993 in Houston, Texas. The course was held at Schlumberger Well Services. The purpose of the course was to provide regulatory personnel with background training regarding operations and regulatory practices associated with gas and oil well logging, with particular emphasis on the health and safety aspects of the use of radioactive materials in these activities. Nineteen State representatives and five NRC staff participated in the training.

Election Results A State-wide referendum was held in Maine on whether to join the low-level radioactive waste compact with the States of Texas and Vermont. The measure was approved by a 73% to 27% margin.

In addition, a local initiative to not allow any radioactive emissions from the Perry Nuclear Power Plant in Ohio was i

defeated.

Roland Lickus, Region III State liaison Officer, attended the Wisconsin Radioactive Waste Review Board meeting on November 4,1993 in Menasha, Wisconsin.

Rocky Mountain low-level Radioactive Waste Board Charles Hackney, Region IV State Liaison Officer, attended the Rocky Mountain l

Low-level Radioactive Waste (LLW) Board meeting on November 5, 1993 in Denver, Colorado.

The Board held hearings on alleged Compact violations by Ramp Industries, Inc. for importing waste into the region without authorization and by ADC0 Services, Inc. for exporting waste from the region without authorization.

The Board also received an update on negotiations with the Central Midwest l

and considered applications from Public Service Company of Colorado for export i

Visit by Ecuadorian National On Friday, November 5, Ms. Susanna Buitron, Director of Biophysical Sciences at the Ecuadorian Atomic Energy Commission (EAEC), visited NRR staff to discuss radiation emergency response. She is visiting the U.S. under an IAEA technical cooperation program to assist Ecuador in developing and implementing national programs related to the inspection of governmental and private medical and dental facilities, quality control in nuclear medicine, transport of radioactive materials and individual personnel dosimetry. Because of the increase in the use of radiation sources in medicine, industry and research in Ecuador, the EAEC is in the process of establishing adequate procedures in case of accidents and is also developing a national radiation emergency plan.

Ms. Buitron also visited FEMA and observed the Waterford emergency exercise.

Hahnemann University Enforcement Conference 4

The Division of Radiation Safety and Safeguards held a closed enforcement conference on November 3,1993 with Hahnemann University. The conference was conducted to discuss the apparent absence of an adequate Quality Management Plan for their various medical diagnostic and therapeutic modalities.

Also discussed were an apparent security violation and several minor violations which indicated that the licensee, through its Radiation Safety Officer, failed to ensure that radiation safety activities are being performed in accordance with approved procedures and regulatory requirements. Enforcement action is pending.

Region II Items of Interest November 5, 1993 1.

University of Viroinia-Research Reactor The University of Virginia has detected a leakage of water from the pool of water surrounding the research reactor. The rate of leakage is estimated to be 100 gallons per day as determined by pool level decrease. The licensee has pool water makeup capacity exceeding this rate.

The pool is a concrete structure 32 feet long,12 feet wide and 26 feet j

deep containing approximately 75,000 gallons of water. The licensee has i

not identified the exact location of the leakage, but is obtaining the services of a contractor to repair the source of the leakage. The licensee indicates that if water leaks outside the building, the pathway would be into a retention pond next to the reactor building. Measured radioactivity levels in water samples from the pool and pond are well below 10 CFR Part 20 limits for release to unrestricted areas. The licensee has an alarm if the pool water level drops to a predetermined level which is 19 feet above the top of the reactor fuel. The alarm system activates an automatic telephone notification of designated licensee individuals. The licensee will be analyzing water samples from the pool and pond on a daily basis until the cause of the leakage is identified and corrected. The licensee will also be conducting surveillance to note status of pool water level during periods when the reactor is not operational and reactor operators are not at the facility.

The Region has notified the Commonwealth of Virginia and is continuing to follow the licensee actions. The Region and the Office of Nuclear Reactor Regulation requestad the licensee to analyze water from the reactor pool and the pond for radioactivity on a daily basis.

An ordinance that had been placed on the North Perry Village ballot to ban all radioactive air emissions from the Perry plant was defeated by a vote of 284 to 57. The ordinance was placed on the ballot earlier.this l

year by an anti-nuclear group from outside the village after they were able to obtain a sufficient number of signatures from residents of the village.

Subsequent to the ordinance being placed on the ballot the Village Council campaigned against its passage.

Region III dispatched two radiation specialists to the LaSalle Generating Station on November 2, 1993 to examine the events regarding an administrative overexposure of a worker. The worker received approximately 655 mrem (6.55 mSv) whole body dose while performing post maintenance valve testing. The worker's daily dose limit was 100 mrem (1 mSv).
