SUBMITTAL OF INFORMATION REGARDING DR. SAENGER AND UNIVERSITi 0F CINCINNATI In response to the January 6, 1993, letter from Bob Alvarez, we are providing you with the information requested by Mr. Alvarez which we were able to locate in Region III.

This includes contractual information pertaining to Dr. Saenger's role as a medical consultant to the NRC, a summary of licenses in Region III which Dr. Saenger consulted for, and copies of documents we were able to locate pertaining to consulting services accomplished by Dr. Saenger.

Please note that we were only able to provide you with information pertaining to those cases involving active licenses, since due to time constraints, we were not able to retrieve from the archives those specific retired licenses.

In response to Mr. Alvarez's request for information regarding documents from Peter Crane, our review of our official materials files for the University of Cincinnati did not identify any documents generated by Mr. Crane.

The only document which we are aware of generated by Mr. Crane is his memorandum of August 27, 1986, which is part of the 01 investigative file on the University of Cincinnati.

Regarding your additit,adl questions to Mr. Axelson regarding the University of Cincinnati, we are providing you the following information:

1.

We were unable to find any documentation indicating that the patient or referring physician was notified of the misadministration involving the leaking iodine-125 seed. We do believe, however, that the patient was aware of an event since she was required to have additional followup care.

2.

It appears that the University was never officially notified by the NRC of the fact that after evaluation we did determine that the event was a misadministration.

In addition to the above information which we were able to locate, we are also providing you with three other documents pertaining to ths University of Cincinnati event.

These include an October 29, 1990 memorandum from Kenneth Carr to Commissioners Rogers, Curtiss and Remick; a November 13, 1990 memorandum from Charles E. Norelius to Glen Sjoblum and a January 16, 1991 memorandum for Robert Bernero from A. Bert Davis.
