Delay in Construction of NUMARC Fire Barriers for Testina j

During a telephone call of October 8,1993, B. Bradley, NUMARC, intormed S. West of NRR/DSSA/SPLB that NUMARC has delayed the construction of the phase 2 test specimen fire barriers from the last week of October until mid-November 1993. Allowing for cure time, NUMARC expects the fire endurance tests to begin mid-January 1994. These tests will be conducted at Omega Point Laboratories' new test facility.

As a result of the poor performance of the 3 hour3.472222e-5 days <br />8.333333e-4 hours <br />4.960317e-6 weeks <br />1.1415e-6 months <br /> Thermo-Lag fire barriers under phase 1, NUMARC is restructuring phase 2 to replace some of the previously planned baseline tests with additional upgrade tests.

NUMARC is now also planning a third phase of tests. NUMARC is also actively pursuing l

Thermo-Lag barrier upgrades using barrier materials manufactured by vendors other than Thermal Science Incorporated (TSI). NUMARC plans to provide additional information during the meeting of October 18, 1993, with the staff.

The phase 2 delay and the addition of phase 3 will delay the development of the industry application guide.

ABB-CE System 80+ ITAAC Review On October 4,1993, the Probabalistic Safety Assessment Branch (SPSB) participated in a meeting with ABB-CE on the System P0+ ITAAC review. Risk important insights and assumptions selected by SPSB before the trip were discussed with ABB-CE's PRA team. The objective was to use this information to identify PRA-based items that should be included as ITAACs or other requirements in the System 80+ design c~ tification. The product of the meeting was an agreed-upon list of items with recommendatioso for dispositioning of each of these items (e.g., as ITAAC, COL, Tech Spec etc.).

i Palisades - Update of the Palisades Activities Regarding the Pressurizer i

Nozzle Crack The pressurizer (PZR) nozzle safe end and the power operated relief valve (PORV) line were repaired by removing the cracked weld and the heat affected u

zoae of the safe end, rewelding the stainless steel PORV line to the Inconel 600 safe end, and performing the non-destructive examination (NDE) of the repair work.

l The licensee also performed NDEs on the welds associated with the PZR spray, safety / relief valves, and the surge line. The results showed no flaws in those locations.

Existing piping stress analyses associated with the PORV l

lines, safety / relief valve discharge lines, spray and surge lines were i

reviewed to evaluate their impact on the Inconel safe end. All stresses were found to be acceptable. The licensee's repair work, engineering analyses and NDE results were documented in an October 7,1993, submittal. Subsequently, two requests for additional information were issued to the licensee to support continued staff review. A management meeting was held between the NRC and 4

Palisades on October 12 to review the licensee's progress to date and identify additional work to be completed by the licensee.

On October 9,1993, while the system was at 250 psi and 85'F, a small leak (less than 1 ounce per minute) from the PZR vapor RTD well was found during a planned walkdown of the system. The licensee depressurized the system and reduced the water level in the PZR.

On October 12, while the system was at atmospheric pressure, a second leak (2-3 ml per minute) was discovered from the water RTD well near the bottom of the PZR. The licensee has scheduled Combustion Engineering to perform eddy current tests on these RDT wells to accurately characterize the cause of the leaks. A detailed repair plan will be developed for NRC review.

NRR and Region III staff members continue to closely monitor licensee activities regarding these primary system leaks.

Browns Ferry Unit 2 Unit 2 continues to operate at full power and has been online for 132 days.

The site record is 189 days.

The Browns Ferry Unit 2 SALP Board meeting was held on October 7, 1993.

This is the first SALP for a Region 11 plant performed under the recently revised procedure. NRR is responsible for the Operations functional area. The SALP presentation to the licensee is scheduled for a public meeting on November 15, 1993.

On Monday morning, October 11, 1993, with Unit 2 at 100% power, operators noticed a 5 inch difference in vessel level indication between NR instruments fed off of the "A" and the "B" reference legs. The licensee entered a 2 hour2.314815e-5 days <br />5.555556e-4 hours <br />3.306878e-6 weeks <br />7.61e-7 months <br /> TS LCO (for RPS) at 10:15 a.m. CDT. The level deviation was caused by leakage (1 drop every 20 to 30 seconds) from the "A" reference leg and was not due to a vessel depressurization event. The sources of the leakage were identified to be valve packing leaks from the two isolation valves and the equalizing valve associated with one of the level transmitters (LT-3-306).

The licensee believes that the reason that the rate of condensation in the condensing pot for the reference leg was insufficient to make up for the leakage was the presence of non-condensible gases in the condensing pot. The error in level was increasing at an accelerating rate, possibly due to the fact that the condensing pot had emptied (same amount of leakage causes a greater change in reference leg level when the top of the water column is in the reference leg).

The leakage was stopped by tightening the valve packing, and the proper level was restored by backfilling the reference leg. The licensee exited the TS LCO at 12:00 noon, CDT, on Oct 11.

BFN has completed the engineering for the constant reference leg backfill modification and has all the necessary material onsite. The licensee has committed to complete the modification during the next cold shutdown.

4 Office of Nuclear Material Safety and Safeguards Items of Interest Week Ending October 15, 1993 Meetina With Louisiana Enerav Services (LES)

On October 12, 1993, an open public meeting was held with Louisiana Energy Services (LES). The meeting was requested by LES for the purpose of discussing the status and schedule of the review of LES' application for a commercial uranium enrichment facility, to be located near Homer, Louisiana.

LES' representatives discussed the submittals they would be making in the next few days to update their application to reflect additional information.

NMSS staff indicated that the projected publication dates for the Draft Environmental Impact Statement (DEIS, November 30,1993) and the Safety Evaluation Report (SER, January 28, 1994) remain the same as reported to the Atomic Safety and Licensing Board in mid-September's status report.

Pre-Anolication Meetina with Amersham Corporation On October 7, 1993, Cask Certification Section staff met with representatives of Amersham Corporation to discuss their Model No. 3300 shipping package. The package is designed to ship large irradiation sources containing up to 500,000 curies of Cobalt-60. A discussion was held on the general package design, information to be included in the drawings, the plans for model testing, and results of tests on a similar package design. An application is expected in mid-1994.

Technical Exchance With the Department of Enerav on the Exploratory Studies Facility The Division of High-Level Waste staff met with the Department of Energy (DOE) in a technical exchange on the issues related to the exploratory studies facility (ESF) at Yucca Mountain, on October 4-5, 1993, in Las Vegas. The main topics of the DOE presentations centered around issues raised by the NRC staff in its August 20, 1993, letter, in which the DOE was asked to explain how it is assuring that ongoing design and construction work will not be adversely affected by problems related to the design, design inputs, and design control processes identified during DOE quality assurance audits.

During the technical exchange, DOE described how the Management and Operations (M & 0) contractor was trying to develop a new requirements hierarchy from the existing documents developed by earlier DOE contractors.

Presentations also clarified the flow-down of the various applicable regulatory requirements to design requirements. The phased approach to the ESF design and the current status of the ESF were also described.

Following the technical exchange, the NRC staff discussed with DOE and M & 0 staff actual examples of requirements flow-down in some detail. The staff did not identify any flaws in the examples discussed. The staff also visited the ESF construction site and observed the drill-and-blast operation of the new test alcove under construction. Based on this exchange, the NRC staff expects a thorough response to its August 20 letter.

2 Meetino of the Licensina Support System Advisory Review Panel On October 5-6, 1993, staff from the Division of High-Level Waste Management, along with staff from the Offices of Information Resources Management, General Counsel, Secretary of the Commission and Atomic Safety and Licensing Board Panel, participated in a meeting of the Licensing Support System Advisory Review Panel (LSSARP) in Las Vegas, Nevada. The purpose of the meeting was to inform panel members of the NRC's revised approach to develop and operate the Licensing Support System (LSS) within DOE's HLW document management system, known as InfoSTREAMS. The panel includes representatives from NRC, DOE, the State of Nevada, affected units of local government, the National Congress of American Indians, and industry.

Panel members did not object to using the InfoSTREAMS approach from a technical point of view but representatives of the State, local governments, and Indian Tribes expressed serious concern with giving DOE direct responsibility for operating the LSS.

They suggested other possible approaches for retaining NRC oversight of the operation of the LSS that will require further study.

Division of low-level Waste Manaaement and Decomissionino Staff Meet With Representatives From Envirocare to Discuss License Conditions On October 13, 1993, personnel of the Division of Low-Level Waste Management and Decommissioning met with Envirocare of Utah, Inc. (Envirocare). The purpose of this meeting was to review the draft license with Envirocare to assure that each license condition was clearly understood.

The main discussion of the meeting centered on the condition requiring Envirocare to complete several specific actions prior to acceptance and/or disposal of lle.(2) byproduct material.

As a result of the discussion during the meeting, Envirocare indicated its preference to receive a license as early as possible and supported the issuance of the license in this manner. The earliest this license could be issued would be 30 days after the U.S.

Environmental Protection Agency (EPA) issued its September 24, 1993, Notice of Availability (NOA) of the Final Environmental Impact Statement, which would be October 24, 1993.

Nuclear Reaulatory Commission Staff Meet with Conaressman Gilchrist On October 8,1993, Nuclear Regulatory Comission staff met with Representative Wayne T. Gilchrist (R, MD) at the Curtis Bay Depot near Baltimore, Maryland. The property adjacent to the Curtis Bay Depot, which was released by NRC for unrestricted use in the late 1970's, is currently listed on the Site Decommissioning Management Plan.

Recent radiological surveys revealed spotty thorium contamination above current release limits in soils and abandoned structures at this site. Also present at this meeting were representatives from the Defense Logistics Agency (DLA), the Maryland Department of the Environment (MdDE), and Anne Arundel County.

Congressman Gilchrist indicated that his main purpose in requesting the meeting was that he was concerned that potential disagreement between DLA and Anne Arundel County over the responsibility for removal of the asbestos siding from the abandoned buildings was slowing the remediation of the site and wanted to ensure that the remediation of the site moved forward. 3 Arundel County representatives indicated that they had reached agreement on how the removal and disposal of the asbestos siding would be accomplished and that the cost of these activities would be shared by both agencies, with Anne Arundel County reserving the right to seek repayment from DLA or the General Services Administration after the site was remediated. During the meeting DLA provided NRC and MdDE staff with an evaluation of the site groundwater prepared by DLA's remediation contractor.

Staff committed to Congressman Gilchrist to review the report and provide coments back to DLA within one week.

Office of Nuclear Regulatory Research Items of Interest Week Ending October 15, 1993 Hydroloav Research Coordination Meetina for HlW The Center for Nuclear Waste Regulatory Analyses (CNWRA) hosted an NMSS-RES hydrology research coordination meeting for high-level radioactive waste (HLW)

October 5-6, 1993. The objective of the meeting was to review research by the University of Arizona (UAz) and the CNWRA in support of resolving technical issues of regulatory significance (Key Technical Uncertainties (KTUs)) in the area of hydrology. The meeting was a focal point of an ongoing coordination process to assure a common base of understanding and coherent direction towards closure of the research.

NMSS staff and management presented an overview of the relevant KTUs and actively solicited technical information from the RES staff and its contractors in attendance (CNWRA and UAz) to be used to further develop and refine the discusseo KTUs.

Presentations of research by CNWRA and UAz focused on their recent accomplishments and ongoing computer and field studies related to addressing the KTUs (NRC staff drafts of the KTUs were transmitted to the contractors one month prior to the meeting).

Research coordination opportunities were discussed in the areas of testing conceptual models using the CNWRA developed SUFLAT code and datasets from experiments and measurements I

conducted by UAz at their Apache Leap Tuff site.

Following the meeting, i

separate discussions were held between RES and CNWRA staff and management to explore more direct cooperation between UAz and CNWRA, focusing on technical opportunities suggested at the previous January 1993 hydrology research coordination meeting.

Office for Analysis and Evaluation of Operational Data Items of Interest Week Ending October 15, 1993 Incident Response Branch (IRB)

On October 13 & 14, IRB participated with FEMA and the other Federal agencies who are signatories to the Federal Radiological Emergency Response Plan (FRERP) to develop a revision to the FRERP. The revision will address inconsistencies between the FRERP and the Federal Response Plan (FRP) and will provide the necessary guidance for coordinating the technical and non-technical aspects of a response to a radiological event.

On October 14, IRB staff met with Shiftwork Systems, Inc. of Boston, MA concerning a six-month joint project involving RES and IRB. The project will investigate how extremely bright lights can be used to improve circadian responses to shift work.

It is being conducted with IRB's Headquarters Operations Officers.

Technical Trainino Center (TTC)

Members of the TTC staff conducted a National Media Seminar at the TTC facility on October 13-14, 1993. The seminar material is titled Nuclear Power and Radiation and is given in coordination with the NRC public affairs function. Classroom presentations and discussions were conducted in addition to demonstrations on the GE and Westinghouse full scope simulators. The nine individuals who attended the seminar represent the following organizations:

newspapers or radio stations in eight different cities (Chattanooga, TN; Dayton, TN; Knoxville, TN; Lexington, NC; Jackson, MS; Myrtle Beach, SC; Syracuse, NY; and San Antonio, TX) and McGraw-Hill Publications. The seminar was well received by attendees.

Public affairs support for the media seminar was provided by the Public Affairs Officer of Regions II.

Meetina on Trends in Industry Risk A working meeting to exchange ideas on ways to analyze trends in industry risk using operational data and PRA methodology was held October 13-15 in Gaithersburg, Maryland. This meeting involved a number of consultants with expertise in PRA and PRA applications, representatives from a number of national laboratories, and cognitive staff from AE00, NRR, and RES.

Currently, the NRC uses the results of the Accident Sequence Precursor (ASP)

Program as a principal indication of the level of risk associated with operating nuclear power plants. The meeting focused on broadening this approach to incorporate other sources of information regarding risk such as hardware and human performance, safety issue implications, existing PRA results (e.g., NUREG-1150), IPE and IPEEE results, as well as ASP results.

Information from these sources would be used to characterize industry aggregated core damage frequency (CDF) trends, " peer group" CDF trends, how changes in design and operational factors have affected industry risks, and would be used in a consistent treatment of statistical issues including uncertainty.

This meeting covered the oojectives for industry risk trends and appropriate measures for trend %g industry risk, along with their associated shortcomings, including potentia, a:su.ee. Areas of discussion included:

core damage frequency, containmu t events, consequences, aging, shutdown and low power events, and external events such as earthquakes, fires, and floods, and statistical issues. The results of this meeting will be used to prepare a program plan to produce an aopropriate method for trending industry risk.

This plan will involve a coordinated interoffice and contracted technical assistance effort.

Raymond Brady, Director, Division of Security serves as the NRC representative on this government / industry security Committee to oversee the future implementation of E. O.12829 (NISP) and the publication of the NISP Operating Manual (NISPOM).

In an October 15, 1993 letter to the Director, Information Security Oversight Office, Mr. Brady concurred in a NISPPAC recommendation to amend E.0. 12829 and to delay the NISPOM publication from January 6,1994 to June 30,1994. This will allow the NISP implementation organizations to consider the impact of other national security initiatives currently underway, i

Contract Awards A contract was awarded to FirstPage, Inc.,

on October 1,

1993 for the

" Purchase / Lease, Airtime and Maintenance of Pagers." The Contractor shall (1) provide unlimited airtime for all pagers, (2) provide on-call maintenance on all pagers, and (3) as required by the NRC Project Officer, provide for lease and/or purchase of new pagers. The contract was awarded for $31,410 and has a one-year period of performance.

A contract was awarded to Knowledgeware, Inc., on September 30, 1993 entitled "ADW Case Tools." The heart of the Information Workbench (IEW) Case tool is an integrated central data repository or data dictionary where all the information is collected.

The repository ensures consistency among all the modules by l

providing error checking as the information is created.

The contractor shall upgrade the NRC's 10 existing IEW Case tools to the Application Development Workbench (ADE) Case tool version 2.7.

The contractor shall also provide 16 additional copies of the ADW CASE tool version 2.7 as well as maintenance on all 26 modules.

The contract was awarded for $123,857 and has a period of performance from September 30, 1993 through October 30, 1994.

Armed Response Personnel. and Guards at Cateaory I Licensees A proposed rule that would amend the regulations applicable to Tactical Response Team members, armed response personnel, and guards at fuel cycle facilities i

possessing formula quantities of strategic special nuclear material (Category I licensees) was published in the Federal Register on October 6, 1993 (58 FR 52305).

The proposed rule would require these personnel to participate in a continuing physical fitness program and to pass an annual performance test. The proposed requirements are intended to ensure that the covered personnel are able to perform their assigned duties under conditions of strenuous tactical engagement. The comment period for the proposed rule closes December 20, 1993.

was published in the Federal Register on October 8, 1993 (58 FR 52406).

The final rule conforms current regulations to reflect the new whistleblower protection provisions of the Energy Policy Act of 1992. The final rule becomes effective November 8, 1993.

Office of Information Resources Management Items of Interest Week Ending October 15, 1993 Advisory Committee on Reactor Safeauards/ Advisory Comittee on Nuclear Waste (ACRS/ACNW) SuDDort ACRS Comittee members were given a demonstration of software for remote access to the Agency Upgrade of Technology for Office Systems (AUTOS) LAN (Local Area Network).

The software and instructions for its use will be provided to ACRS/ACNW staff for review and testing.

The Office of Information Resources Management will be working with ACRS/ACNW over the next month to provide dial-in access for Comittee members.

Initial work has been completed to provide ACRS/ACNW staff with access to the Internet from their UNIX workstation.

Records Review On October 13, 1993, the Information and Records Management Branch issued 82 memoranda to Records Liaison Officers requesting their review of 719 boxes of records before we authorize their destruction. Sixty of these boxes are stored at the Federal Records Center in Suitland, MD.

Disability Workshop Held On October 14, 1993, in keeping with National Disability Awareness month, OP staff presented a r.ew course to the agency, " Working with People with Disabilities." Employees attended two half-day sessions, which were conducted by an organization which specializes in the training and job placement of people with disabilities. Participants discussed the new Americans with Disabilities Act and learned about a variety of disabilities among employees t

such as physical, learning, and communication. The next session of this.

course will be held in February 1994. Managers are encouraged to attend this l

course and to urge their employees to attend.

Sionificant Enforcement Actions A Notice of Violation and Proposed Imposition of Civil Penalty in the amount of $80,000 was issued October 13, 1993, to Public Service Company of Colorado l

(Ft. St. Vrain). This action was based on a DOL complaint by a radiation protection technician who alleged that Public Service of Colorado had terminated his employment when he reported radiological safety violations.

The action was discussed in SECY-93-263 and approved by the Commission on October 7, 1993. (EN 93-101)

A Notice of Violation and Proposed Imposition of Civil Penalties in the t

cumulative amount of $200,000 was issued on October 12, 1993, to the Nebraska Public Power District. This action was based on the violations involved in three separate regulatory concerns which have each been classified at Severity Level III. The first concern involves a number of corrective action violations which have collectively been categorized as a problem. The second concern involves a violation for the inoperability of both trains of the containment hydrogen / oxygen analyzer system. The third and last concern involves a violation for the failure to include required portions of the service water and reactor equipment cooling systems in the inservice inspection program.

i This action was based on: (1) a Severity Level I problem for violations that led to a misadministration as a result of the Licensee's failure to prepare a written directive as required by the Licensee's quality management program and follow departmental procedures, and (2) a Severity Level III problem for violations associated with misadministration reporting requirements.

Office of Consolidation Items of Interest Week Ending October 15, 1993 Second Buildina Status During the week, the landscape contractor replaced azalea and evergreen plants that had not survived the initial plantings in June. On Floor 4, carpet tile installation was completed in the office areas and lobby.

Installation of wall covering in corridors, case-work cabinets in conference rooms and special work areas, and final painting is underway on Floors 4 and 5.

Joann Manzek of the Office of Consolidation staff completed a review of Floor 5 final take-off drawings for Haworth furniture and sent them to Baltimore Stationery for preparation of the orders for that floor. Mrs. Manzek also completed the initial review of Floor 5 Transwall demountable wall take-off 1

drawings and returned them to Transwall for editing. Mrs. Manzek completed the design modifications of Floor 7 floor plans to accommodate a reorganization of the Office of Nuclear Material Safety and Safeguards and sent them to Baltimore Stationery for production of CAD-generated drawings.

On Octcber 13, General Services Administration (GSA) Regional Office space l

planning staff provided copias of GSA's space assignment analysis to the Office of Consolidatica. These "As-ignment Take-offs" provide the basis for GSA's determination of the rent payment assessment to the NRC. Office of Consolidation staff are reviewing the analysis.

Region IV Administrator, and other NRC staff to River Bend on October 12, 1993 for a public meeting between NRC and Entergy Operations Inc./ Gulf States Utility (GSU) management.

The purpose of the meeting was for Entergy to brief NRC on recent management changes at GSU and short and long term plans to increase productivity, decrease cost, create better management / employee relations, increase NRC awareness of plant activities and better response to NRC concerns.

CRCPD Technical Plannino Committee Meetina Mindy Landau, State Programs, participated in the Conference of Radiation Control Program Directors, Inc. (CRCPD) Technical Planning Committee meeting on October 12-14, 1993 in Frankfort, Kentucky. The purpose of the meeting was to develop an agenda for the 1994 Annual CRCPD meeting which will be held on May 22-26, 1994 in Williamsburg, Virginia.

Maria Lopez-Otin, Federal Liaison, attended a meeting of the Subcommittee on Water Resources (SWR) on October 13, 1993.

This working session focused on developing a framework for agency submission to the upcoming SWR report. In this regard, an example paper on " water availability" was distributed and the members were requested to provide their agency's submissions in that format.

In addition, the SWR was informed that as a result of the prompt work it had done responding to the OSTP request to provide a list of available agency " flood experts" the OMB and the Office of Environmental Policy will include the SWR when they send the memorandum announcing the formation of an interagency " Scientific Assessment and Strategy Team" to provide scientific advice and assistance regarding flood recovery in the Mississippi Basin. The SWR members who are also i

members of the Private Enterprise / Government Interaction (PEGI) were strongly urged to attend the upcoming PEGI workshop on November 3 and 4, 1993.

Meetinas with Massachusetts Officials Marie Miller, Region I State Liaison Officer, attended the Massachusetts Low-Level Radioactive Waste (LLRW) Management Board meeting on October 13, 1993 in Boston, Massachusetts. The purpose of the meeting was to resolve comments from the public on major portions of the LLRW management plan. No decisions on site selection or disposal options will be made until the plan is completed.

Panel Discussion on Pilarim Environmental Monitorina On October 13, 1993, Dr. Robert Bores, Chief of the Region I Effluents Radiation Protection Section, participated in a panel discussion in i

Duxbury, MA on the monitoring of radiation releases from and the environment around the Pilgrim Nuclear Power Station. The public meeting was held at the request of the selectmen of the Town of Duxbury, in response to a request from the Duxbury Nuclear Advisory Committee.

The panel was moderated by Dr. Nancy Ridley, Director of the Massachusetts Bureau of Environmental Monitoring, and in addition to Dr. Bores included l

Robert Hallisey, Director of the Massachusetts Radiation Control Program, a representative from the Boston Edison Company (the Pilgrim licensee),

1 and a Duxbury citizen who runs an independent radiation monitoring program for the community.

South Carolina Electric and Gas Company On October 13, the Deputy Regional Administrator was at the Sunrner facility to preseat operator certificates to newly licensed operators.

Fire at Nuclear Fuel Services. Inc.

On October 13 at 4:13 p.m., the licensee declared an Alert because of a fire on the roof of Building 234. This building, which was formerly used to process plutonium, has been undergoing decontamination. The fire was extinguished by 4:20 p.m. by the licensee's staff.

Region II activated its Incident Response Center and entered the Monitoring Mode until 5:25 p.m. when it was established that the facility was in a safe condition.

The NRC resident was onsite and responded to the fire.

The fire was caused by contract roofers, who were heating roofing material while placing a new roof on the building.

The fire caused a hole of approximately one square foot in the roof.

There were no personnel contaminated.

The licensee issued a press release and the event was reported in local media.

Evaluations of stack sample results for the building show that the releases during the fire appear to have been within normal ranges and air samplers down wind at and beyond the site boundary showed normal background concentrations.

The licensee will not resume roofing operations until the licensee's investigation team as determined the causes and corrective actions are implemented.

i Sequoyah Unit 2 has entered hot shutdown condition (Mode 3) and is currently conducting maintenance work prior to requesting NRC approval to proceed with the startup of the unit.

NRC continues around-the-clock coverage onsite.

The unit has been shut down since March 1, 1993.

Summit Testina and Insoection Company On October 12, 1993, a telephone enforcement cor:ference was conducted in the Region III office between representatives of Summit Testing and Inspection Company, and members of the Region III staff to discuss the circumstances surrounding reported damage to a moisture / density gauge which occurred on July 8, 1993.

Reaulators Meetina On October 14, 1993, the Director, Division of Radiation Safety and Safeguards, Region III and other members of his staff participated in a Regulators Meeting in Newburgh Heights, Ohio, to discuss remediation plans for the Chemetron Bert Avenue site. Also participating were representatives from NMSS Low Level Waste Division, Ohio EPA, Ohio Department of Public Health, and local officials from the Newburgh i

Heights community.

Bio Rock Point On October 12, 1993, the Deputy Regional Administrator and Deputy Director, Division of Reactor Projects, conducted an enforcement conference with the Chief Operating Officer and Vice President, Nuclear for the Consumers Power Company to discuss two events involving a breach of containment integrity while shut down, and an overpressurization of the primary coolant system during a hydrostatic test.

Meetina with Kennecott Uranium Comoany On October 12, 1993, the Director, URF0, and members of his staff met with representatives of Kennecott Uranium Company and their consultants in the URF0 Office. This meeting was held at the request of the licensee to discuss the proposed design of a new tailings disposal area for the Sweetwater uranium mill in Wyoming. The Sweetwater mill is currently in standby;; however, Kennecott believes uranium market conditions will permit resumption of operations. A new tailings disposal area will be required for future operations.

Manaaement Meetina at River Bend Station l

The Regional Administrator and other NRC staff members met with Gulf States Utilities senior management on October 12, 1993.

The purpose of the meeting was to discuss the performance improvement plan implementation for the River Bend Station. The President and Chief l

- Entergy Operations, Inc. attended the meeting. This meeting was open to public observation. Members of the public and media attended the meeting.

Personnel Chanaes at River Bend Station On October 11, 1993, Gulf States Utilities announced additional management changes for the operation of the River Bend Station.

It was announced that: Mr. Joe Venable, previously the Operations Supervisor, will be the Assistant Plant Manager - Operations and Rad Waste; Joe Schippert, previously the Plant Manager, will be Assistant Plant Manager

- System Engineering; and Mr. Early Ewing, previously an Entergy employee in the corporate office, will be the Assistant Plant Manager -

Maintenance.

Cooper Nuclear Station The Regional Administrator and members of the Region IV staff toured the Cooper Nuclear Station during the evening of October 14, met with the I

resident inspectors, and interviewed selected licensee managers on l

October 15, 1993. The visit was intended to provide a current l

i perspective on the plant and licensee improvement initiatives.
