Emergency Response Organization and Control Room (CR)
STATE OF OREGON: State Emergency Response Organization, Oregon Health Division Agency Operations Center (OHD-AOC), Columbia County EOC, Field Team, and CR Representative.
1.3 PURPOSE The purpose of the 1993 Annual Exercise is to demonstrate the ability of PGE emergency response personnel to adequately respond to an emergency at the Trojan Nuclear Facility.
This exercise also provides the State of Oregon and Columbia County the opportunity to test their plans and procedures.
The conduct of this exercise satisfies the requirements of Title 10, Code of Federal Regulations, Part 50, Appendix E.
i 1.4 DEFINITIONS Drill-A supervised instruction period aimed at testing, developing, and maintaining skills in a particular operation. A drillis often a component of an exercise.
Emeroency - An off normal situation at Trojan that involves or could potentially involve the release of radioactive material warranting actions to safeguard Plant personnel and equipment. Such situations are categorized into two classes: UNUSUAL EVENT or ALERT.
Emeroency Action levels - Radiological dose rates; specific levels of airborne, waterborne, or surface-deposited concentrations of radioactive materials; or specific instrument indications that are used to designate a particular class of emergency. Each class of emergency requires prescribed actions by PGE.
Emeroency Coordinator - The PGE individual responsible for directing on-site actions during an emergency at Trojan. The Shift Manager serves in this position.
2 Emeroency Ooerations Centers - Facilities from which County emergency direction / coordination is provided.
Emeroency Worker - PGE, Federal, State, County and other personnel who execute presenbed assessment, corrective, protective or recover actions.
Exclusion Area - Area surrounding a nuclear reactor, in which the reactor licensee has the authority to determine all activities including exclusion or removal of personnel and property from the area, as defined in Title 10, Code of Federal Regulations, Part 100.3(a).
Incestion Exoosure Pathway The potential pathway of radioactive materials to the public through consumption of radiologically contaminated water and food products.

Protective Actions Those emergency measures taken for the purpose of preventing or minimizing radiological exposures to persons.
Recovery - A period of time lasting until the Facility is restored to as nearly as possible its pre-emergency condition.

l 3.0 GUIDELINES 3.1 GENER AL GUIDELINES 1.
The Exercise will be conducted in real time.

The scenario is confidential.

If Exercise participants identify a need to terminate play due to f acility conditions or unsafe play, they shall immediately notify the Facility Controller or the PGE Lead Controller.
4.
The EC may also stop the Exercise.
j 5.
Exercise Controller Organization a.
The Controller Organization is directed by the PGE Lead Controller. The overall conduct of the Exercise is the responsibility of the Lead Controller, b.
All other Controllers report directly to the Lead Controller. If Exercise play does not follow the Exercise Scenario, Controllers shall immediately report play status to the Lead Controller.
c.
State and county Facility Controllers are responsible for controlling play within their assigned facility. Facility Controllers are responsible for periodically reporting to the Lead Controller the status of the Exercise play.
6.
The public address system will be used. All Exercise related announcements shall begin and end with the statement "THIS IS A DRILL *.
7.
Onsite actions will be demonstrated in as realistic a manner as possible based on scenario events. Exercise players will don anti-contamination clothing, use respiratory equipment and set up radiation barriers as needed to demonstrate actions that would be taken if a real emergency existed. All exercise participants will follow Facility procedures.
8.
Facility controls, equipment and systems will NOT be manipulated. DO NOT do anything that affects or could affect the plant operation or equipment. DO NOT change any switch positions, open/close valve, start /stop equipment, change fire hoses, etc.
9.
DO NOT do anything to endanger personnel.
10.
Observe all normal Facility security requirements and all posted signs: RP, Security, Traffic Control, etc.
11.
Arm bands will be used to identify Exercise Controllers (yellow), Observers (green) and Evaluators (orange). Red arm bands will be distributed to players by the controllers only when they are leaving the Control Room.

3.3 PLAYER GUIDEUNES 1.
Controllers serve an active role in the Exercise by providing messages or j
instructions to the players. Controllers may also prompt or initiate certain actions to assure continuity of the events described in the Exercise scenario.

Evaluators assess and keep notes of players actions in each facility.

Controllers may act simultaneously as evaluators.

Play out all actions, as much as possible, in accordance with plant procedures as if I
it were a real emergency.

Verify that a Controller sees and hears your actions. Play out your actions unless j
directed to simulate them.

If you are in doubt, ask a Controller for clarification.
7.
All messages transmitted on communication lines or radios shall be preceded and followed by the statement that "This is a Drill".

Play as if radioactive materials are actually present, in accordance with scenario data.
9.
Trojan Facility Procedures shall be followed throughout the course of the Exercise.
10.
Facility and radiological parameter data may be made available by inputs from the controllers.
11.
DO NOT become overly concerned with the mechanics or the cause of the accident.
This exercise is designed to test plans and procedures and is not concerned with establishing the probability, feasibility or detailed mechanics of the simulated accident.

There will be one or more Controllers at each important location.

13.
If, during any part of the Exercise, you are having trouble accomplishing your required duties, confusion anses, or clarification is necessary, ask a Controller, Controller assistance or clanfication does not necessarily imply failure on your part.
The Controller will know the limitations of information that may be provided to you, and will assist you only to the extent necessary.
14 Document the items which you feel will improve plans and procedures. Participant Evaluation Forms should be completed at the end of the Exercise and turned into i
the Lead Controller.

Any Players who encounter members of the news media during the Exercise should i
not respond to questions regarding the simulated events or the success of the j
Exercise. All press inquiries should be directed to the Media Representative.
16.
Do not take actions that would result in actual alterations of valve and switch positions in response to scenario simulations. Any event or operation outside the scenario that results in an actual or potential danger to Facility operation or safety will take precedence over exercise activity. Should an actual operational requirement for action occur during the Exercise, the Exercise activity will be suspended or canceled at the discretion of the Shift Manager.
17.
Phone numbers to be used during the Exercise are to be those as printed in the l
Emergency Telephone Directory. Exceptions to those numbers will be provided as a handout by the exercise controllers on the day of the exercise, i
l 3.4 CONTROLLER GUIDELINES 1.
All Controllers shall pre-position themselves at least thirty (30) minutes prior to the beginning of the exercise.
2.
Prior to the start of the Exercise, Facility Controllers shall call the Lead Controller to verify communications availability.
3.
All Controllers will comply with instructions from the Lead Controller.
4.
All Controllers shall synchronize their watches to ensure that messages and time-related plant and radiological data are delivered on time. The goveming time is that shown on the clock in the CR.
5.
Each Controller will have copies of the messages controlling the process of the Exercise scenario.
6.
Controllers responsible for giving players cue cards (messages) will be assigned a j
set of colored cue cards. These colored cue cards are the only messages that are given to the players. If you are not given a set of colored cue cards, you do not give messages to the players unless otherwise specifically directed by the Lead Controller.

I 7.
Unless otherwise instructed by the Lead Controller, all messages, plant parameter data and radiological data shall be issued at their designated times. Verify that players understand the information provided to them.
8.
Issue contingency messages only as directed by the Lead Controller.
9.
Call the Lead Controller immed;ately for advice or if in doubt about your responsibilities.
10.
Allow the Players reasonable flexibility to perform their functions and demonstrate their skill, knowledge and initiative.
11.
Call the Lead Controller immediately for advice if the Players depart significantly from scenario script. If necessary, intervene with Player action and put play back on track.
12.
Stop play and notify the Lead Controller if Facility or personnel safety is jeopardized.
13.
Do not prompt Player actions by offering suggestions, reminders or instructions.
14.
Controllers shall not divulge information concerning the scenario events or scenario data prior to the times noted on the cue cards or data sheets, unless directed by the Lead Controller.

15.
Each Controller will take detailed notes regarding the progress of the Exercise and l
the responses of Participants. All notes should be retained for the purpose of preparing a written critique of the Exercise.
16.
Non-Participants are exempt from acting appropriately to simulated radiation levels specified for the emergency Exercise. However, pfl normal radiolooical control oractices shall be followed throuohout the course of the Exercise.
17.
Controllers should ensure that contact between Players and Observers is minimized.
An Observer area may be established in the Control Room.
18.
If it appears that an Exercise objective will not be met, immediately notify the Lead Controller.
19.
Following the termination of the Exercise, the Lead Evaluator will hold a critique of the Exercise activities at the Facility. Facility Controllers will also hold critiques at their designated facility. This critique should last no more than thirty (30) minutes.
20.
All Controller notes, completed evaluation forms and attendance sheets shall be handed in to the Lead Controller at the end of the Facility Critique.

4.5 CONTROL CER 4.5.1 FUNCTION The Control Cell serves two functions during the exercise: (1) to receive telephone calls made by PGE/ Trojan to offsite agencies or PGE corporate groups which are not participating in this year's exercise: (2) provide telephonic media and public rumor control questions to state exercise participants.
The following offsite agencies will not be participating in the 1993 Annual Exercise:

4.5.2 RUMOR CONTROL The individuals assigned to the Control Cell will periodically call the number state and county participants designated as the contact point for rumor control. These calls shall be randomly spaced within each one hour exercise period starting at exercise timeline time T+ 1.00 hours0 days <br />0 hours <br />0 weeks <br />0 months <br />. The Control Cell will initiate a minimum of two calls to each participant each hour.
The caller shall:
(1) decide whether to simulate being a media or public caller; (2) review the questions listed under that heading on the following pages; (3) place the call to the state or county office; (4) specify to the individual answering the phone that the call is exercise related by stating "THIS IS A DRILL MESSAGE";
(5) identify self as being a member of the media or public using an appropriate alias; (6) ask the selected question (s);
(7) document the call using a " Controller Observation Sheet" and list the question (s) used, by type and number, and the answers provided.

Note that it is not intended that all of the questions provided be asked during the exercise. Rather it is left up to the discretion of the Control Cell Caller to ask those questions that seem most relevant to (1) the conditions as provided in the exercise and (2) the answers provided by the participants.
4.5.3 MEDIA QUESTIONS 1.
Can the Trojan Visitors information Center Heliport be used by Helicopter 6 to set up a news team at Trojan? How close can we get to the Facility?

I'd like to know what is going on at the Trojan Facility. What exactly has the State done so far, and what is being done to protect the people?

Can you give me some general information about the accident at the Trojan Facility?
What started it? What is the situation now? How long before the situation is under control? What are the states and counties doing?

I heard that something unusual is going on at a nuclear plant in your area. Can you confirm or deny and give me the details? I need to know where the Facility is and the population of the area.

I understand people have been asked to evacuate from an area surrounding the Trojan Facility because of a release of radioactive materials. How many people are we talking about here and where are they going? Is there help available for people i

5.0 EXERCISE SCENARIO i
5.1 NARRATIVE SUMM ARY The scenario for the 1993 Annual Exercise involves a flammable material spillin the RadWaste Building. The spillis followed by an extensive fire. At some point during the j
fire, a propane tank explodes. ripping a hole in the ceiling of the building. The extent of the j
fire is such that radioactive waste stored in the C-Vans for transport to a high density l
compactor burns. This results in a contaminated fire with an unmonitored release path.

annual PMs on the "A" EDG. It is anticipated that the generator will be unavailable i
for the entire day. The Radiation Protection Department is coordinating a C-Van i
shipment of radioactive waste this morning.
A semi-tractor / trailer is in the process of leaving the flatbed trailer in the building for the j
radwaste filled shipping container to be loaded onto. The propane powered forklift is in the i
RadWaste Building to assist in the loading to be conducted later in the morning, i
j As the truck is starting to pull away from the trailer, the forklift operator simultaneously 4
begins to move the forklift around the front of the trailer. The forklift operator is distracted l
by a technician in the RadWaste Building as the truck stalls and rolls back into the forklift's l
path. The forklift collides with the truck, rupturing the right side fuel tank on the truck as i
the truck is restarted. The forklift operator rapidly backs the forklift up to approximately
}
the lift stands on the trailer as he notices the fuel pouring out onto the floor.

The forklift operator abandons his vehicle at that point, with the forklift still running, and yells for everyone else in the building to evacuate. The truck operator abandons the truck 3
in the RadWaste Building, also with it still running. All other personnel successfully j
evacuate the building.

The fuel continues to leak onto the floor for about 10 minutes and will spread north through the building, under the loaded container and toward the stored polyvinyl high integrity j
containers (HICs). During this time it is expected that the technicians and operating shift j
will be discussing what actions should be taken. Any attempts to shut the roll-up door or
]
cover the fuel with foam or chemicals will be unsuccessful. An UNUSUAL EVENT will be
]
declared at this point due to a flammable material release affecting areas within the EAB. A j
contingency card will be issued at 8:20 if this declaration has not been made by that time.
At about 8:30, the fuel ignites with a large " whump" noise. The building remains intact.
Visual inspection through the roll-up door will show the truck fully involved in the fire. The 4
j fire brigade should be dispatched at this time. At 8:40 a loud snapping noise will be heard j

and visual inspection will show that the left side fuel tank has split and its fuel is now also pouring out onto the floor. The fuel on the floor is burning and has spread the fire to the trailer and the forklift. At some point the fire department will be called but the arnval of the response will be delayed by an automobile accident on Highway 30 (simulated).
At 8:43 the propane tank on the forklift explodes. The missile effect from this explosion results in a hole in the roof of the RadWaste Building and a number of smaller holes in the east wall. An ALERT will be declared based upon an explosion affecting the structure of the RadWaste Building. A contingency card will be issued at 8:50 if the ALERT has not been declared.
The force of the explosion helps spread the fire throughout the building. The radwaste in the container is fully involved by 8:45. The radioactive release starts at this time.
At approximately 9:00, the ERO is activated and assuming their emergency responsibilities.
Ne cetual assembly or avacuation of site personnel or evacuation of the Exclusion Area shall take place.
At about 9:30 the fire will be out due to the combination of fire fighting efforts and the lack of further flammable material in the RadWaste Building. Clean-up operations and recovery planning should be discussed. Radiological surveys should be completed outside of the RadWaste Building by 11:00.
The Trojan ERO should determine that no offsite protective action recommendation (PAFl) is needed. The State of Oregon field team will take environmental samples.
No shift change or reentry is to be demonstrated.
At 11:00 the exercise terminates onsite except for key positions that liaison with offsite.
Those individuals and the offsite participating locations will terminate exercise related activities as objectives are completed. It is anticipated that this will be no later thaa 1:30.

6.0 MESSAGES Message to players concerning scenario related conditions are printed on cue cards. Cue cards to be given to players are printed on colored paper. Prior to the exercise these colored cue cards are given to the appropriate controller.
Each controller is issued a full set of cue cards printed on white paper. These controller cue cards have not only the message given to the players but also the anticipated actions of the players in response to the message.
Cue cards are numbered numerically from one to eighteen.
Cue Cards with a capital "C" fc!!owing the number are Contingency Cards. The 1993 Annual Exercise does not include any postulated events that would affect plant parameters as read in the CR. No plant parameter data or cue cards are used in the 1993 Annual Exercise. All plant conditions will be *as read" with the exception of radiological j
and meteorological conditions which are covered in the following sections.
8.0 RADIOLOGICAL DATA i
8.1 SOURCE TERM DEFINITION J
The scale of radiological effects from the postulated accident in this year's exercise were developed based upon the participation in this exercise by the OHD's Field Team.
The source term used in RPC-93-022, Ingestion Dose Projection from a Radwaste Fire, j
contained both resin / filter waste and dry activated waste (DAW). As this scenario involves j
the only the burning of DAW, that component of the source term was used. The DAW component was evaluated and any nuclide for which five half-lives would have passed on i
or before the scheduled exercise date (November 17,1993) were deleted from the release mixture. The remaining nuclides were decayed according to their half-life as listed in the NRC's Response Technical Manual, NUREG/BR-0150, Vol.1, Rev. 2, dated October 1992, j
using a decay period of 54 weeks.
The total release was further reduced by 75% to more closely approximate the amount of DAW that could be stored in the Radwaste Building under Trojan's permanently defueled condition.
The numeric value of the final exercise source term. fire release fraction and initial deposition values are shown on Table 1.
8.2 RADIATION MONITORS Due to the magnitude of the release, and the unmonitored release path, area radiation monitor and process radiation monitor readings will be "as read" in the CR.
8.3 ONSITE RADIOLOGICAL DATA Onsite radiological data (dose rates, surface and airborne contamination levels) are listed on Tables 2 and 3. Controllers use this Table to verbally relay the appropriate information to the players when required.

0 8.4 OFFSITE RADIOLOGICAL DATA The plume is postulated to ground north-northwest of the exclusion area. Offsite radiological dose rate measurements and airborne contamination sampling will be "AS READ" due to the calculated levels from the given source term being below background.
Contamination levels (smears) will be 1500 dom /cm at the nearest garden which is located 2
794 meters to the NNW of the fire.
Controllers will relay this information verbally to the players when required.
The Field Team will be taking samples and preparing the samples for transport to the OHD Laboratory in Portland. These samples will not be actually delivered to the laboratory.
9.0 METEOROLOGICAL INFORMATION Today's weather is mild and clear with morning temperatures in the upper thirties and a high in the upper fonies. The wind is from the South-Southeast at four to five miles per hour and will remain steady all day. A low pressure area is expected to reach this area Friday evening, bringing colder weather for the weekend.
Meteorological data as would be observea in the Control Room is detailed on Table 4, Meteorological Data. This information may be provided by any controller in the CR when requested.
