SUBJECT: NRC INSPECTION REPORT 50-382/93-32 (NOTICE OF VIOLATION)

This refers to the inspection conducted by Mr. E. J. Ford and

Ms. J. L. Dixon-Herrity of this office on September 19 through

October 30, 1993.

The inspection included a review of activities authorized

for your Waterford Steam Electric Station, Unit 3, facility. At the

conclusion of the inspection, the findings were discussed with those members

of your staff identified in the enclosed report.

Areas examined during the inspection are identified in the report. Within

these areas, the inspection consisted of selective examinations of procedures-

and representative records, interviews with personnel, and observation of

activities in progress. The purpose of the inspection was to determine

whether activities authorized by the license were conducted safely and in

accordance with NRC requirements.

Based on the results of this inspection, certain licensed activities appeared

to be in violation of NRC requirements, as specified in the enclosed Notice of

Violation (Notice).

The violation is of concern because the continuous

failure to follow a system operating procedure and' return a deviated valve to

its normal locked closed position resulted in the failure of the automatic

makeup for the-ultimate heat sink, allowing the ultimate heat sink water level

to fall below the Technical Specification requirement.

The valve had been

deviated open and not questioned since August 17, 1993.

A second violation was identified that involved the failure to promptly

identify and document the failure of a valve to stroke as required by the

Technical Specification. This violation is considered to be a second example

of the apparent violation identified in NRC Inspection Report 50-382/93-33,

which involved the failure to promptly identify and correct the inoperability

of containment spray Valve CS-125A. This violation is of concern because your

failure to promptly identify the inoperable status of Valve MS-120B and to

enter this inoperability into your corrective action system is. indicative of a

continuing problem to properly implement your corrective action system.

With respect to the first violation, you are required to respond to this

letter and should follow the instructions specified in the enclosed Notice

In your response, you should document the

specific actions taken and any additional actions you plan to prevent

recurrence. After reviewirg your response to this Notice, including your

proposed corrective acticas and the results of future inspections, the NRC

will determine whether further NRC enforcement action is necessary to ensure

compliance with NRC regulatory requirements.

With respect tc the second violation, you are requested to address your

corrective %.tions as a part of your response to the violation identified in

NRC Inspection Report 50-382/93-33.
