International Visit - Canadian Radioactive Waste Regulators Staff from the Division of High-level Waste Management, the Division of Low-level Waste Management and Decommissioning and the Office of Governmental 2ad Public Affairs met with representatives from the Canadian Atomic Energy Control Board
'(AECB), the nuclear regulatory body of Canada, on October 17, 1990.
Topics discussed at the meeting included the U.S. Environmental Protection Agency standard and problems being encountered in the United States, and similar issues in Canada.
Extreme-interest was expressed by the Canadians on the effort being undertaken by the staff to develop its performance assessment evaluation capability.
In addition, there were discussions on the waste vitrification experience in Canada as well as ongoing work being undertaken in the Canadian exploratory shaft J
facility. With respect to low-level waste (LLW), topics discussed included the U.S. LLW program and the staff's role, Greater-Than-Class-C waste, mixed waste and Agreement State activities.
Indian Point 3 On the afternoon of October 16, 1990, the Region I Regional Administrator ap-proved the licensee's plan for the safe and controlled retrieval of the two fuel assemblies attached to the upper core internals. The two fuel assemblies had been suspended four feet above the top of the core since October 4, 1990, when the licensee discovered that two fuel assemblies had been inadvertently withdrawn from the core while ' lifting the upper core internals as part of the preparations for refueling the reactor.
At 1628 on October 16, 1990, the licensee began retrieval of the fuel assem-blies by lifting the upper core internals vertically an additions 1 eight feet with the polar crane. The upper core internals were then manually rotated 180 degrees,'which moved the fuel assemblies away from-the reactor vessel and over the refueling cavity. The upper core internals were then moved horizontally with the polar: crane trolley approximately 10-feet into its recovery basket below.
In accordance with recovery procedures, all retrieval attempts were halted and the containment was evacuated. No increase in radiation-levels was detected'inside containment atmosphere and samples of the reactor coolant system were. also normal.

A combined Region 1/NRR inspection team, managed by Jack Durr, Region I, has been cnsite since October 11, 1990, monitoring the licensee's activities and performing an independent technical review of the adequacy of the licensee's engineering evaluations, safety assessments and recovery procedures for the fuel span of recovery activities up to and including transfer of the two fuel assemblies to the spent fuel pool,

Office of Nuclear Reactor Regulation items of Interest Week Ending October 19, 1990 Grand Gulf Unit 1 The plant was shutdown on September 30, 1990, for the fourth refueling outage.
During this outage, two hundred eighty-four (284)(8X8 fuel assemblies will be replaced with Advanced Nuclear Fuels Corporation ANF) 9X9 fuel assemblies.
With this reload, the entire core will contain ANF 9X9 fuel assemblies.
The reactor vessel head was removed and efforts were underway to remove the steam dryer and separator assemblies in preparation for refueling the reactor.
On October 4, during the lif ting of the steam dryer it became lodged approxi-mately five feet above its seated position.
It had rotated slightly out of alignment with its guide rods preventing further lif ting of the dryer.
On October 7, the dryer was realigned with the guide rods and was successfully removed. Upon removal of the dryer, an inspection of the guide rods and steam bolts of the separator assembly were damaged preventing immediate removal of the separator.
Damaged components are being removed with special underwater tools. Replacement components are available on-site. As of October 10, one damaged hold down bolt was removed.
The work has delayed start of core refueling operations by 4 days to date.
Some work previously planned to occur near the end of the outage was moved up to reduce the impact on the outage length.
The resident inspector is following this item.
TM1 Unit 1 GPU Nuclear announced on October 10 the following organizational changes which will be effective on December 31, 1990.
Mr. T. Gary'Broughton was named Vice President and Director of. Three Mile Island Unit 1.
He will succeed Mr. H. D. Hukill who is retiring. Mr. Broughton is currently Director of Operations and Maintenance for TMI Unit 1 and was pre-viously GPU Nuclear director of System Engineering.
Mr. Michael J. Ross will become Director of Operations and Maintenance. re-placing Mr. Broughton. Mr. Ross is currently Operations Director for-TM1 Unit 1.
Vogtle Units 1 and 2 Yesterday,. October 11, 1990, the licensee informed Region 11 that G. Bockhold had been transferred from his position as Plant Manager of Vogtle to a new position in Southern Services Company.
Bill Shipman, more recently the General Manager of Support Services in Birmingham, has been named as new Plant Manager on a temporary basis.

Waterford 3 On October 8, 1990, Waterford 3 was placed in cold shutdown to repair the pressurizer safety valves that had been leaking.
During the shutdown, the licensee performed a general area visual inside the containment and identified boric acid buildup on Reactor Coolant Pump (RCP) 2-A.
Further investigation revealed that a reactor coolant leak existed between the flanges that connect the RCP casing to the driver mount bottom flange.
The flange is designed with two concentric flexatallic gaskets and the leakage was through both gaskets. As a result, boric acid had accumulated around the flange studs, which are made of carbon steel.
The licensee has performed ultrasonic testing (TV) of the studs and has determined that no wastage has occurred due to boric acid corrosion.
NRR and Region IV personnel are currently in the process of reviewing the results of the UT of the studs. A conference call is planned with the licen-see today.
The licensee does not plan to start up the plant until this issue is resolved.
H. B. Robinson 2 On October 12, 1990, the heavy rain caused the plant cooling lake level to rise 6-inches in 6 hours6.944444e-5 days <br />0.00167 hours <br />9.920635e-6 weeks <br />2.283e-6 months <br />. The cooling lake dam gates were opened which caused flooding downstream.
Prior to the opening, the licensee notified the local authorities. About 2,000 to 3,000 residents were evacuated and a number of homes sustained flood damage.
The licensee stated that the procedures to open the gate is a Unit No. 1 (which is a fossil plant at the same site) operation.
There is no Technical Specification procedural requirement of Unit No. 2 on the control of the~1ake level.
NRR and Region 11 are pursuing this issue with the licensee to resolve questions related to the cooling lake (Unit No. 2 ultimate heat sink) level requirements.

GE Wide Rarge Neutron Monitoring System On October 3,1990 NRR issued a Safety Evaluation Report for General Electric Topical Report NEDO 31439, " General Electric Nuclear Measurement Analysis and Control, Wide Range Neutron Monitoring System" (NUMARC WRNMS). The SER con-cluded that this digital microprocessor based equipment is acceptable for use in nuclear power plant safety systems and meets all of the criteria of Regula-tory Guide 1.97.
General Electric expects that a typical application of the NUMAC WRNMS would be a replacement of the existing non-qualified Source Range Monitor and Intermediate Range Monitor equipment with a 10CFR50.49 environ-mentally qualified WRNMS.
NRC/NUMARC Meeting on Procurement 1
On October 16, 1990, VIB management and staff met with representatives of NUMARC to discuss the current status of licensee actions to implement the NUMARC procurement initiatives. The meeting focused on licensee and NUMARC
. actions taken since the NUMARC Board of Directors action on June 28, 1990, to approve the comprehensive procurement initiatives for licensee implementation by July 1992.
NUMARC reported that all licensees currently have efforts-underway to review, and revise as appropriate, procurement programs consistent with the initiatives. Two NUMARC/ licensee workshops were held in mid-1990 to discuss implementation issues.
During the meeting, NUMARC and VIB representatives agreed that periodic meetings on this program topic to discuts the status of licensees' actions to implement the initiatives and NRC actions to monitor and assess licensee improvements in this area would be appropriate. NRC also discussed general plans to conduct assessments of selected licensees during the inspection pause to observe directly licensee improvements and actions.
Reports of these assessments will be provided to the licensee visited, the public (PDR) and to NUMARC.

NUMARC requested that the next meeting be scheduled for mid-November and that the topics for discussion include traceability, documentation, and distributor /
supplier issues.
NUMARC also invited NRC to participate in the next NUMARC/
111censee workshop which is planned for early 1991.
Massachusetts Cancer Study
- The Director, DREP, assembled a task group consisting of members of his staff, RES, and Region I, in order to review a report prepared by the Massachusetts Department of Health of persons having leukemia and who reside near the Pilgrim Nuclear Power Plant.
This study differed from the recently published NCI Study in that it focused on a single facility and examined the incidence of leukemia (the NCI Study focussed on cancer mortality) in specific persons living (or deceased) in 22 towns near Pilgrim. Tne study claims that the incidence of leukemia for those living and working closest to the plant was four times higher than for those at more distant locecions during the period 1978-1983.

Office of Nuclear Material Safety and Safeguards items of Interest Week Ending October 19, 1990 Approval of the Pathfinder Reactor Vessel Transport Package On October 11, 1990, the Transportation Branch issued Certificate of Compliance No. 9238 for the Pathfinder Reactor Vessel as a transport package for low specific activity radioactive material.
Northern States Power Company is cur-rently decommissioning the Pathfinder plant, and plans to ship the reactor vessel intact from the Pathfinder site near Sioux Falls, South Dakota, to Richland, Washington. The Pathfinder Reactor Vessel contains irradiated reactor internals, and is filled with gravel and solidified grout for radiation shielding. The package weighs approximately 580,000 pounds and is to be shipped by rail.
International Atomic Energy Agency (IAEA) Insnection at the General Electric Facil g
.On October 15, 1990, an IAEA_t'.4m commenced a short-notice random' inspection at-the General Electric Fuel Fabrication Plant in Wilmington, North Carolina.
The team audited the facility's ledger, weighed and measured the U-235 content of UFg cylinders by nondestructive assay, and performed attribute testing on addit'fonal UF cylinders.
6 Nuclear Material Categorization Tables for Physical Protection Representatives of the Department of State (DOS), Department of Energy and Nuclear Regulatory Commission met at DOS on October 11, 1990, to review and coment on a draft contractor's report which provides recommendations for increasing the consistency among the categorizat?on tables presented in INFCIRC/225/ Revision 2,-INFCIRC/254 and INFCIRC/2/4/ Revision 1.
A revised draft will be issued, and a second meeting on this topic will be held at DOS on October 29, 1990. A preparatory conference is scheduled at the IAEA '
in Vienna in June 1991 to review possible modifications to the Convention on Physical Protection of Nuclear Material., including the Material Categorization Table. This is in preparation for a review conference on the Convention to be
-held at'the IAEA in June 1992.
Rocketdyne Division, Canoga Park, California On October 9-10, 1990, members of the Fuel Cycle Safety-Branch accompanied the Region V inspector to Rockwell's Santa Susana Field Laboratory site.
Rockwell is currently in the decomissioning phase. The tour included several buildings in various stages of decomissioning which are outsidt. the Department of Energy (DOE) controlled area, but-are not under the Nuclear Regulatory Comission license. _ Discussions included the decomissioning plan, ceganizational changes, l
financial assurance and status of the buildings outside the COE area.

General Atomics, San Diego, California On October 11-12, 1990, members of the Fuel Cycle Safety Branch accompanied the Region V inspector.to the General Atomics (GA) site.
GA is currently decommissioning the SVA Building. Topics discussed included the decommissioning plan for the SVA and the release of laboratories in Building 2.
Sequoyah Fuels Corporation, Gore, Oklahoma During the week of October 8-15, 1990, a member of the fuel Cycle Safety Branch participated in daily surveillance inspections at the Sequoyah Fuels Corporation site. This inspection supplements the Augmented Inspection Team and followup inspections previously conducted by Region IV. The Fuel Cycle Safety Branch staff will provide input to the forthcoming inspection report.
NRC-DOE Technical Exchange on Calico Hills Risk-Benefit Analysis On October 12, 1990, representatives of the Nuclear Regulatory Commission (NRC) and Department of Energy (DOE) held a technical exchange on the Calico Hills Risk-BenefitAnalysis(CHRBA). The NRC-DOE technical exchange focused on the Calico Hills data needs identified by DOE and how those data needs are incorporated into the decision analysis being done as part of the CHRBA.
Another major topic of discussion was DOE's approach to the analysis of waste isolation impacts that could result from excavations into the Calico Hills.
This technical exchange, a precursor to a more in-depth NRC-00E interaction on the CHRBA to be held within a few months, allowed NRC to provide DOE with some early feedback to the approaches being utilized in the CHRBA. The State of Nevada and Clark County, Nevada, also participated in this exchange.

1 E90-09' Additional Factors Affecting the Lift Setpoint of Pressurizer Safety Valves - AEOD Licensee root cause analyses of unexpected shifts in the lif t setpint of pressurizer safety valves at V. C. Summer and other nuclear pir.nts identified factors that may not be fully recognized or considered.
The reduction in lif t setpoints of pressurizer safety volves could result in primary system leakage, and cause unnecessary challenges to plant safety systems.
Conversely, an increase in lif t setpoint causes a reduction in safety margin in the protection of the RCS against overpressure S ansients.
s Accordingly, these factors affecting the lift setpoint of pressurizer safety valves may warrant'further attention, t
T90-14 Seal Problems in Boric Acid Transfer Pumps Problems with seals and the seal water system in the boric acid transfer pumps at Turkey Point were reviewed for potential generic implications.
SCSS and NPRDS databases were queried to identify other plants with boric acid pump problems. Contact with seven other plants disclosed that they did not use seal water systems for their pumps because they use single mechanical pump seals rather than the double seal used at Turkey Point. Seal failure does not appear to degrade the pump performance, but it does require corrective action.
Discussions with plant personnel indicated chronic housekeeping and maintenance problems with the 12.w/o boric acid in the system. _ One t.tility is changing over to sealless pumps as part of their modification to 4 w/o boric acid in the system.
This would eliminate the housekeeping and maintenance needed for these pumps.
Diagnostic Evaluations-A team planning meeting was held during October 16-18, 1990 in preparation for-the Oyster Creek Diagnostic Evaluation scheduled to begin on. November 5,1990.
Attendees included personnel from AE00, NRR, Regions 11 and III~'in addition to three contractors who will provide expertise in the areas of engineering and' l
management.- On October 18, Oyster Creek training personnel administered-site specific GET training to all team members to speed the badging process.

Incident Response l
On=0ctober 15, 1990, the IRB staff demonstrated the computer based operator aids utilized in the NRC Operations Center to the Halden Reactor Project Group.
This Group, based in Norway, consists-of representatives from several foreign countries who conduct research on computer systems and fuels research. The presentations-included a demonstration of the Operations Center communications system including the Emergency Notification System (ENS), the Emergency Response
_ Data System (EROS) and RASCAL,-the computer dose assessment model.

On October 17, 1990, an exercise was conducted with San Onofre.
The participants included, from Headquarters, Chairman Carr as ET Director and Hugh Thompson, Ed Jordan, Jack Heltemes, Bob Burnett and Tom Murley as the other Executive Team members and members of the Protective Measures, Reactor Safety and Administrative Support Teams; the licensee and State of Cal'fornia; and the Region V Base and Site Teams.
Other Federal agency representatives participated with the Protective Measure; Team in assessing protective actions.
FTS-2000 was used for emergency comnrunications as a pilot demonstration of its capability.
During the week of October 8-12, 1990, an IRB staff member chaired an IAEA Advisory Group to approve the final report on exchange of data and assistance in the event of an international nuclear accident.

Office of Governmental and Public Affairs Items of Interest Week Erding October 19, 1990 Society of Nuclear Medicine, Mi3-Eastern Chapter James Myers, State Programs, participated in the Fall Meeting of the Technologist Section, Mid-Eastern Chapter, Society of Nuclear Medicine, on October 13, 1990 in Old Town Alexandria, Virginia. Mr. Myers spoke on " Quality Assurance in Agreement State Prcgrams".
'New York State Radiation Control Programs Review A consolidated review of the New York State radiation control programs wes conducted on October 15-19, 1990 in Albany and Brooklyn, New York.
NRC staff from the Regions, NMSS and State Programs conducted the review of the State Departments of Labor, Health and Environmental Conservation. The Nr.w York City radiation Control program was reviewed on October 1-5, 1990. This is.the first time since New York became an Agreement State in 1962 that a formal consolidated review has been performed. The preliminary staff findings will be reviewed by Regional and Headquarters offices and the results of the review will be discussed at a meeting with the State scheduled for November 19, 1990 and will be confirmed in a' letter to the State.
Nevada Radiation Control Program Review Visit Jack Hornor, Region V' State Agreements Officer, conducted a review visit.. the Nevada Radiation Control Program on October 15-19, 1990 in Carson City, Nevada.
James Shaffner, LLW, assisted Mr. Hornor on thu LLW portion of the visit. The purpose of a review visit is to assess the status of the State's program and to assist the State in preparing for a routine review.
Meeting with Wisconsin State Liaison Officer Roland Lickus, Chief, Segion III State and Government Aff6ks, met with Leroy Conner, Acting Administrator, Wisconsin Division of Emergeacy Government, and-
-State Liaison Officer, on October 15, 1990 in Madison, W.sconsin. The. purpose.
of-the meeting was to discuss the NRC regulatory program, the State Liaison Officer program and other issues of mutual interest.

LLW Forum Meeting Stephen Salomon, State Programs, attended the Low-Level Waste Forum quarterly meeting on October 17-19, 1990 in Miami, Florida. The Forum held a special information session on liability and financial assurance. The Forum participants also received an update on constitutional challenges to the Low-Level Radioactive Waste Policy Amendments Act filed by the States of New York and Michigan and the Concerned Citizens of Nebraska. Site specific challo ges in North Carolina, Texas and Michigan were also discussed, as well as th ilabama hazardous waste case. The Forum also received an update on DOE technical assistance and on the Technical Coordinating Committee activities.
James Kennedy, LLWM&D, briefed the Forum on NRC activities, including the database rulemaking, BRC and Agreement State guidelines. Dr. Salomon gave a status report on the import / export of waste rulemaking, Part 20 and the NRC transportation rule, in addition, the Forum was briefed by its Working Group on Federal Waste, the Working Group on Mixed Waste and discussed State plans for interim storage, including a report from the New York State Energy and Development Authority on its study in the area and the NRC storage policy.
Finally, the Forum received an update on State plans for environmental assessments, medical community involvement in LLW disposal facility siting, materials & waste working group and the impact of new Department of Transportation regulations on LLW. The Forum unanimously adopted a resolution to request the NRC to provide a copy of SECY-90-318 " Low-Level Radioactive Waste Policy Amendments Act Title Transfer
. and Possession Provisions" to the Forum and-to allow Forum representatives to address the Commission at the meeting scheduled for October 29, 1990.
International Visitors On Wednesday, Mr. Cheng Youjeng, First Secretary for Science and Technology at the Embassy of the People's Republic of China, met with James Shea, Ronald Hauber and Kevin Burke to introduce his successor Mr. Zhao Kangyuan. The Chinese representatives expressed. appreciation for the safety assistance that NRC has provided to the Chinese nuclear power program and hope that the exchange program can return to its previous level of activity.
On Wednesday three members of the Nuclear Waste Directorate of Canada's AECB visited NRC. The visitors, Mr. John Beare (Director General, Fuel Cycle and Materials Regulation Directorate), Mr. George Jack (Director, Waste Management Division) and Mr. Ken Bragg (Supervisor, High-Level Waste Section) met with members of NMSS High-and Low-Level Waste Divisions to exchange views on a range of regulatory questions pertaining to the handling of waste disposal. in the two countries.

On Friday, Dr. Hans Blix, Director General of the International Atomic Energy Agency, met with Commissioners Rogers and Curtiss, Harold Denton,_ Director, GPA, and James Shea, Director, IP, to discuss IAEA programs. The current lAEA proposals for projects with advanced reactors, Operational Safety Review Team visits to the U.S., the September 1991 International Nuclear Safety Conference, the IAEA's assistance with VVER 440/230 reactors, and international safeguards efforts were some of the topics discussed.
On Friday, Mr. Jjublo Fabjan, Reactor Engineering Department, Jozef Stefan Institute in Yugoslavia, met with Mr. Richard Lobel, NRR, to discuss technical specification standardization.

Office of Administration items of Interest Week Ending October 19, 1990 OPH Security Director's Session The Director of Security and a member of the Personnel Security Branch attended a Security Director's Session sponsored by the Office of Personnel Management (OPM) on October 18, 1990, at OPM Headquarters in Washington, D.C.
Security Officials from most Government Agencies were in attendance. The Director of the OPM Office of Investigations provided a briefing on the proposed revision of forms used by OPM to conduct background investigations and provided updates on the status of several lawsuits that have been brought against OPM's use of the current forms.
Request for FAA Assistance in an October 15, 1990, letter to its Baltimore office, Hugh Thompson, Deputy Executive Director for Nuclear Materials Safety, Safeguards and Operations Support,requestedtheFederalAviationAdministration(FAA)totakeactionto revent the recurrence of helicopters flying close to the One White Flint North p(0WFN) building. NRC pointed out to FAA that erratic wind currents surrounding OWFN make it particularly dangerous for aircraft flying close to the building.
FAA advised NRC they have received many similar complaints in the Bethesde /Rockville area. The NRC staff will continue to attempt to obtain identi', cation numbers so that future aircraft may be identified for appropriate FAA enforcement action.

Telephone Service Interruption Telephone service in the Nuclear Regulatory Commission's offices that are located on the 7th, 8th,10th and lith floors of the Maryland National Bank Building, Bethesda, Maryland was interrupted on October 18.
The interruption was caused by water leaking into the telephone riser cable system. Service to most offices had been restored by the next day.

Pilot Course for Local Area Network Training The " Orientation to the Local Area Network (LAN)" pilot class was presented on October 9, 2990, in the NRC's ITS Training Lab. Comments received from those who participated were very positive. The Wordperfect 5.1 pilet class will be given on October 16, 1990.
This is a half-day, hands-on course designed to train end-users experienced with Wordperfect 5.0 in the new features of 5.1.

PRA Technoloav Transfer Proaram Activities The PRA Advisory Committee met on October 16 to review course materials and course scheduling for 1991. On October 17, Lillian van Santen and John Bickel, INEL PRA Program Manager met with representatives of NMSS and NRR to analyze future training needs.
Job Fairs Produce Results The job fairs attended by NRC personnel in various locations in the country during the week of October 9-12 produced 32 applications from the following locations: Carnegie Mellon University, University of Pittsburgh, Rutgers University, and Alabama A&M.

A Notice of Violation and Proposed Imposition of Civil Penalties in the amount of $2500 was issued on October 16, 1990 to Alt & Witzig Engineering, Inc., Indianapolis, Indiana.
The action was based on:
- 1) failure to secure moisture / density gauges left unattended in the licensee's vehicles, and 2) a number of violations of NRC requirements for safe transportation of licensed material that, considered in the aggregate, are indicative of a lack of management control. The base civil penalty was escalated because NRC identified the violations, the licensee had prior notice, and there were multiple examples of the violations.
Partial mitigation was allowed because the licensee took prompt corrective action once NRC identified the violations.

The action was based on a Severity' Level 11 violation for the licensee's-failure to promptly report the declaration of a Site Area Emergency to state and local authorities. A Severity Level IV violation was also cited 1
for the failure to develop adequate procedures to ensure timely closing of the containment equipment hatch. The base amount of the civil penalty was mitigated by 50% due to the licensee's prompt and extencive corrective L

The action was based on a therapeutic misadministration that resulted from failure to check a patient's planning chart prior to administration. Additional violations of reporting requirements as to-therapeutic misadministrations are cited without civil penalty. The base civil penalty was mitigated for identification and reporting and corrective action, but was escalated for past performance, with a net mitigation of 251.

General Motors Corporation, Saginaw, Michigan, paid the civil penalty in the amount of $875.
The action followed the licensee's reporting the loss of a level gauge from its Grey Iron Plant at Saginaw, Michigan.
The gauge contained a 17 millicurie cesium-137 sealed source.
An NRC inspection identified a number of additional violations, including inadequate training of staff, absence of warning labels cn gauges, and the removal of the device by unauthorized persons.

Indian Point Unit 3 On October 4, 1990, at Indian Point Unit 3,.the New York Power Authority (NYPA) detected that two fuel assemblies were lodged in and lifted with the upper core plate, during refueling operations. NYPA's plan for the recovery of theses assemblies was reviewed by a multidisciplinary NRC on-site team headed by the Region I, DRS, Engineering Branch Chief.
The Regional Administrator authorized implementation of the plan following this review, in accordance with a Confirmatory Action Letter. The recovery -
began on October 16, 1990, during the next to last horizontal move of the core plate and fuel assemblies, one of the assemblies was dislodged. This assembly fell vertically approximately ten (10) feet into its catcher-basket. The containment was evacuated following the drop as a precaution.
There was no indication of fuel assembly damage (i.e. no increase in radiation levels or reactor coolant activity).
Following reentry into the containment the second assembly was lowered into its catcher basket. On October 17, 1990 this assembly was removed from the core plate using a hydraulic jack. The core plate was then transferred to it normal stand in the refueling cavity. As of the morning of October 18, 1990, the assemblies remained in their catchers in the refueling pool. NYPA is pursuing use of the normal spent fuel handling tool to transfer the assemblies to the spent fuel pool. The NRC on-site team' continues to-monitor these activities.

. meeting with members of the Town of Tuxedo Board of Supervisors, two local.
L homeowners associations and the local water company to discuss the plans L
for decomissioning the Cintichem reactor and. hot laboratory facilities.
'NRR and' Region I sent representatives to the meeting to discuss the decommissioning process from the regulatory perspective. The meeting-attendees expressed appreciation to the company for the opportunity to have their questions addressed.

Duke Power Company On October 15, a licensee employee working in containment at the McGuire facility cut his leg with a grinder.
The employee was thought to be contaminated and was promptly transported to a local hospital.
Subse-quently, the employee was found not to be contaminated and the injury was not considered major.
On October 16, Connissioner Curtiss, accompanied by the Director, Division of Reactor Safety, was at the Duke Power Company's Catawba Nuclear Plant for site familiarization.

Carolina Nwer and Light Company 0n October 16, representatives from the Carolina Power and Light Company were in the Region 11 Office to attend an Enforcement Conference regarding events.of August 19, 1990 at their Brunswick facility.
During the current reactor vessel internals inspection at H. B. Robinson, crack indications were identified on 38 out of 106 control rod guide tube support split pins. The' split pin ultrasonic test inspection was prompted by an earlier split pin failure (March 1989) which resulted in Robinson shutting down to retrieve the loose part from the steam generator.
Presently, the licensee and Westinghouse are evaluating resolution options with respect.to the'38 affected pins, 3..
Veterans Hospital

Fermi 2 Nuclear Plant A Detroit Edison employee from Corporate Technical Services suffered fatal injuries while performing balancing and vibration monitoring tests on a reactor building supply fan in the turbine building test area.
The employee apparently slipped or trip (ped into the running fan during the test whilesafety) ro reaching under a barrier to diagnostic vibration equipment.
Preliminary investigation results by the j
licensee and responsible local law enforcement agencies concluded that the fatality was an industrial accident.

Reactor Coolant Pump (RCP) Flange Leakage at Waterford 3 (W3)
This updates the staff note issued on October 12, 1990, regarding the flange leakage on RCP 2A at W3.
On October 12, 1990, a Confirmation of Action Letter was issued to require the following actions to be completed prior to W3 entering Mode 2:
(1) NRC inspection of the ultrasonic testing (UT) technique for determining degradation of RCP studs; (2) NRC inspection of the leakage monitoring technique for detecting RCP flange leakage; and (3) conduct a conference call with licensee and NRC personnel regarding the results of the licensee's bounding failure analysis for potential RCP stud degradation due to buric acid corrosion.
Onsite inspections performed by NRC personnel on October 13, 1990, concluded that the-UT and leakage monitoring techniques utilized by the licensee were satisfactory. The results of the licensee's bounding.
analysis for stud degradation was discussed during a telephone. conference, between Region IV, NRR, and licensee personnel, on October 13, 1990. The results indicated that a reduction of stud diameter due to wastage by boric acid' corrosion.fould cause stud elongation due to stresses from reactor coolant system pressure. The elongation of the studs would result in increased leakage across the RCP flange inner gasket. The licensee's detection of the' increased leakage would provide sufficient time.to shut down the plant prior to failure of the RCP studs. The licensee continually monitors the RCP flange for an increase in leakage using two video cameras.

Prior to entering Mode 2, an inspection of the RCP flange was.

The licensee will continue-to monitor for leakage-from the flange through the end of the current fuel cycle, which is projected to be March 1991.

Transportation Event - Spill of Low Level Sludge or Liquid On October 13, 1990, the Region IV Duty Officer was notified by the l
Headquarters Duty Officer. of a spill of material initially thought.to be uranium oxide in Albuquerque, New Mexico.
It was reported that approximately-5' gallons of liquid had leaked from one 55 gallon: drum on a shipment of two' semi-loads of this material.
The local fire department res)onded, cleaned up the spill with an absorbent material, and surveyed tie area.
The trucks were then released and proceeded to their destination near Grants, New Mexico.

the material was not uranium oxide, but was a barium sulfate sludge from their water treatment-facility, being shipped to Quivira for disposal.
