NOTICE OF VIOLATION (NRC INSPECTION REPORT NOS. 50-361/93-17 AND
50-362/93-17)
This letter refers to the inspection conducted by Mr. C. Myers of this office,
and Ms. C. Yates of the Office of Nuclear Reactor Regulation, during the
period of August 16 through 20, 1993, of activities authorized by NRC License
Nos. NPF-10 and NPF-15, and to the discussion of our findings held with
members of your staff on August 20, 1993. Additional in-office review of
documents continued through August 27, 1993. The inspection evaluated the
effectiveness of your activities to assure the operability and reliability of
motor operated valves (MOVs) in safety-related systems.
Specific areas examined during this inspection are described,in the enclosed
inspection report. Within these areas, the inspection consisted of selective
examinations of procedures and representative records, and interviews with
personnel.
Based on the results of the inspection, certain of your activities appeared to
be in violation of NRC requirements, as specified in the enclosed Notice of
Violation (Notice). The violation concerns a failure to document test
deficiencies and to take timely and effective corrective actions.
Your MOV program was found to be implemented consistent with your commitments

sampled during the inspection.

However, the report discusses a concern associated with auxiliary feedwater
valve 3HV-4705. After completing your GL 89-10 program to assure the

capability of the M0V to operate under worst case design basis conditions, the
MOV subsecuently failed to open during routine surveillance testing. You
,
identifiec maintenance related deficiencies as the root cause for that failure
(
of the valve to open under no flow conditions in May,1993.
