Millstone Unit 2 l

On August 7, 1993, an event investigation team was established by the Millstone 2 director to identify the root cause of the leakage from letdown system isolation valve failure and to review the subsequent actions including work control, procedural compliance, adequacy and the results of valve disassembly, detailed metallurgical testing of the valve studs and body and management oversight of the repair effort. The final report of this investigation team's effort is scheduled for September 17, 1993.

On August 20, 1993, a Millstone Unit No. 2 Independent Review Team was established to perform an assessment, on an advisory basis to the Chief Nuclear Officer, John Opeka, to determine whether the basis for current operation of Millstone Unit No. 2 is sound. Additionally, the review will focus on management actions, root cause evaluations and corrective actions taken to determine their appropriateness and whether they will assure lasting 1

correction to the identified problems.

Finally, the review will consider j

lessons learned for purposes of sharing with the entire Northeast Utilities nuclear organization and the industry as appropriate.

Review categories include Design Control, Configuration Control, Management Involvement and Event Investigation Triggers. The team is scheduled to complete the review by September 3, 1993. At the completion of the review, the team will prepare a report identifying findings and recommendations as appropriate. Additionally, during the course of the review, significant findings and observations will be communicated to management.

i Effective September 1, 1993, Jack Keenan has been removed as director of Millstone 2 to report directly to John Opeka, Executive Vice President, to assist in the Engineering Integration Activity for the utility.

Effective September 12, 1993, Gary Bouchard will assume the position as director of Millstone 2.

In the interim Stephen Scace, Vice President, Millstone Station, will be the acting director for Millstone 2.

Gary Bouchard was the former director of Haddam Neck and currently is the director of Quality Services.

i The licensee plans to fill the position of director of Quality Services with a reverse loan from INP0. No name has been identified yet.

Russian Visit On August 31, 1993, Mr. Victor Chernomyrdin, Prime Minister of the Russian Federation, visited the St. Lucie Plant. The Prime Minister was accompanied by 19 other Russian dignitaries and support personnel.

Ivan Selin, NRC Chairman, William White, Deputy Secretary of Energy, Stewart Ebneter, Region II Administrator, along with the Florida Power and Light i

(FP&L) top corporate and plant management were on site to welcome and guide i

the Russian visitors through a tour of the plant. The approximately 1-hour tour included the control room and other essential parts of the plant. After the tour, which ended with a demonstration of the St. Lucie simulator, there was a short photo opportunity for the national and local media and press.

River Bend Station Plant Startup At 11:48 p.m. on August 31, 1993, River Bend was taken critical in the restart from a plant scram that occurred on August 10, 1993. The reactor tripped due to an inadvertent shorting of two terminals by an I&C technician while performing maintenance. On August 14, 1993, while shut down, the Site Vice President issued a Stop Work Directive due to a " declining trend in human performance." The Stop Work Directive was lifted on August 19, 1993, and a requirement to have three consecutive days of error-free work was imposed as a condition for startup. That condition was satisfied on August 25, 1993. To minimize thermal stresses on a leaking fuel pin, the plant had an extended cool down and did not reach Mode 4 (Cold Shutdown) until 10:30 p.m. on August 26, 1993.

Because it appeared that the plant would enter Cold Shutdown, the staff addressed the issue of whether the licensee would be required to install the reactor vessel level modification requested by Bulletin 93-03 prior to resta-t. After consultation with the Reactor Systems Branch, the licensee was informed that they would not be required to install the modification prior to startup.

The licensee's respo7se to Bulletin 93-03 is still being reviewed by the staff.

i ABWR Proof and Review Technical Specifications Complete On August 31, 1993, the last of the proof-and-review version of the ABWR technical specifications were issued to GE, in accordance with the established review schedule.

During that review, the following major issues need to be resolved:

Treatment of non-safety systems (e.g., the combustion turbine generator); while this version of the ABWR technical specifications includes requirements for ATWS, comparable requirements for station blackout have not been included.

Consideration of the ABWR probabilistic analysis results for possible changes to the Completion Times and Surveillance Intervals to accommodate design-specific vulnerabilities.

Office of Nuclear Material Safety and Safeguards Items of Interest Week Ending September 3, 1993 Hurricane Impact on General Electric In response to the posting of a hurricane watch in the Wilmington, North Carolina area on August 30, 1993, General Electric initiated actions to secure their low enriched uranium fuel conversion plant. Activities included removing extraneous materials from the roofs of buildings, moving refurbished shipping containers inside, banding together and covering non-refurbished shipping containers, verifying operability of emergency diesel generators, equalizing the levels in the nitrate lagoons and neutralizing the lagoon contents. With regard to the status of process operations, the conversion lines were shutdown, and activity in the Uranium Recovery Unit was limited to rad waste processing.

Some powder and pellet operations that could be quickly shutdown were allowed to continue to operate.

Environmental Protection Aaency Issues Record of Decision for DuPont Site in Newoort. Delaware On August 26, 1993, the Environmental Protection Agency (EPA) issued a Record of Decision (ROD) for the DuPont Superfund site in Newport, Delaware.

the planned remedial action at this site involves thorium wastes that were previously disposed onsite in accordance with an Atomic Energy Commission license, which was terminated in 1968. The Superfund remediation action is primarily driven by concerns about heavy metal and organic contamination over j

large areas at the site. The Nuclear Regulatory Commission had independently identified the potential for buried radioactive waste at the site as part of i

its review of formerly licensed sites. The staff has reviewed and commented on the draft R0D and will initiate contact with DuPont, in coordination with d

EPA, to ensure proper resolution of the thorium waste.

Meetina with State of Utah on Envirocare On August 31, 1993, representatives of the Division of Low-Level Waste Management and Office of State Programs met with the Director of the Utah Bureau of Radiation Control.

The staff is completing its review of Envirocare's application for a license to conmercially dispose of lle.(2) byproduct material at its Clive, Utah site. Utah has licensed Envirocare to dispose of low-level waste and NORM at that site. Two primary subjects discussed were NRC's draft license conditions for Envirocare and the possibility of Utah providing assistance to the NRC inspection program at Envirecare. Utah was also receptive to the concept of providing inspection assistance to fiRC at the Envirocare site.

Appalachian Compact Users of Radioisotopes (ACURI) meeting on August 27, 1993.

The current status of several ongoing and future NRC activities was reviewed, SEPTEMBER 3, 1993 ENCLOSURE B i

2 including: the on-site storage rulemaking and comments filed in response to the notice of proposed rulemaking; the uniform manifest rulemaking and public meeting held in Bethesda on June 15th; the joint NRC/ EPA efforts to develop additional mixed waste guidance dealing with testing and storage; the advance notice of proposed rulemaking regarding private land ownership for low-level waste (LLW) disposal sites, and guidance being developed on LLW performance assessment. The principal area of questioning from ACURI members was related to NRC licensing requirements for storage of LLW and their concerns about the size and increase of NRC license fees as well as the need for all ACURI members to actively support the Compact and Commonwealth of Pennsylvania in the siting of a LLW disposal facility. A "LLW Disposal Facility Resolution of Support" document has been prepared by ACURI and was circulated at the meeting for signature by individual ACURI members.

Workshoo On The Hich-Level Waste Repository Proaram On August 24-25 Headquarters staff from the Division of High-level Waste Management and the On-site Representatives attended a workshop sponsored by the Affected Units of Local Government (AULG) in Pahrump, Nevada, on the U.S.

Department of Energy's (DOE's) high-level waste repository program. The purpose was to provide a forum by which the AULG could " explore the range of issues and viewpoints regarding DOE's program." The workshop was attended by representatives of various Federal and other governmental agencies involved in the program.

The Division of High-Level Waste Management staff gave a presentation on the Nuclear Regulatory Commission and its role in the high-level waste program. Other presentations included discussions on: 1) the Environmental Protection Agency standards for the disposal of high-level waste; 2) DOE's Alternative Licensing Strategy; 3) the status of the Nuclear Waste Negotiator's search for a volunteer host for the Monitored Retrievable Storage Facility; 4) the Nuclear Waste Technical Review Board's role; 5) the General Accounting Office's recent findings and recommendations; and 6) industry and public utility commission viewpoints.

DOE /NRC Technical Exchance On Substantially Complete Containment And Waste Packaae Desian Concepts On August 24, 1993, representatives of the U.S. Department of Energy (00E),

Nuclear Regulatory Commission, and State of Nevada participated in a technical exchange to discuss the status of DOE's waste package design effort. Other topics included interpretation of substantially complete containment (SCC) and its use as a guide in design, and DOE's plans for addressing site characterization open items related to waste package design concepts. Staff participants presented an overview of NRC's approach to reducing the uncertainty in SCC and staff from the Center for Nuclear Waste Regulatory Analyses (CNWRA) presented a discussion of activities related to the engineered barrier system program at the CNWRA.

Office of Nuclear Regulatory Research Items of Interest Week Ending September 3, 1993 Aareement with United Kinodom Atomic Enerav Authority (AEA) Relatino to Reliability and Probablistic Risk Analysis This agreement defines the terms and conditions by which the USNRC and the AEA will continue cooperation in the area of reliability and risk assessment research. The AEA has agreed to provide reports on results of studies conducted under this agreement including software reliability, human factors, and human reli 45111ty analysis, and dependent failure analysis. The NRC will reimburse the !EA during the 3-year term of the agreement. The agreement has been sent to the EDO for signature and, thereafter, will be sent to the AEA for their signature.

Office for Analysis and Evaluation of Operational Data Items of Interest Week Ending September 3, 1993 Diaanostic Evaluation and incident Investiaation Branch (DEIIB)

During the period August 30-September 3 the Quad Cities Diagnostic Evaluation Team (DET) conducted its second week of onsite observations, interviews and document reviews at the Quad Cities Station and the licensee's (Commonwealth Edison Company) corporate offices. On September 3 the DET conducted an interim exit meeting with the licensee on the preliminary observations of the team. The DET returns to the AE00 offices in Bethesda, Maryland on September 7 to begin 2 weeks of in office evaluation and planning in preparation for the third and final week of onsite evaluation activities scheduled for the week of September 20.

Incident Response Branch (IRB)

On October 1, IRB staff met with the Pennsylvania Emergency Management Agency in Harrisburg, Pennsylvania to discuss work being performed to incorporate the Federal Radiological Monitoring and Assessment Center (FRMAC) into Pennsylvania's Emergency Plans.

It is anticipated that revision to the Pennsylvania Plans will be forthcoming sometime this year.

On August 30-31, in conjunction with Regions I and II, IRB monitored the course of Hurricane Emily. Although the Headquarters Operations Center was not formally activated, emergency communicaticas resources were ready on a standby capacity, if needed, and coordination with the Federal Emergency Management Agency (FEMA) was effected.

Division of Safety Proorams (DFDJ The AE0D Annual Report - Power Reactors, NUREG-1272, Vol. 7, No.1, which presents an overview of the operating experience of the nuclear power industry from the NRC perspective, has been issued. The report also includes the principal findings and issues identified in AEOD studies over the past year, and summarizes information from such sources as licensee events, diagnostic evaluations, and reports to the NRC's Operations Center.

The materials part, NUREG-1272, Vol. 7, No. 2, is in preparation.

r Office of Administration Items of Interest Week Ending September 3, 1993 Contract Awards NRC's Contract entitled, " Purchase of Microcomputer Systems", was issued to the Small Business Administration for execution on 8/23/93. The subcontractor under this effort, Pulsar Data Systems, Inc., will deliver to the NRC Headquarters and all regions, completely assembled Dell Computer Corporation 486 microcomputer workstations with keyboard, mouse, and a 14 inch SVGA color display system monitor.

Every system provided shall have the latest version of MS DOS and Windows installed and operational on the hard drive. This contract also provides for on-site warranty at all places of delivery. Prior to final acceptance by the NRC, to ensure that all systems are fully operational, the contractor will also conduct a 24-hour " burn-in" on each system following system integrat ion and configuration.

A competitive contract was awarded to Wheaton Medical Clinic, Ltd. on August 24, 1993, entitled " Health Services for Region III Employees."

The contractor will provide medical services by a licensed practicing physician for approximately 250 NRC personnel for a two-year base period of performance. The contract amount is $149,650 for the base period of performance, $79,925 for Option Year 1, and 582,200 for Option Year 2.

The Division of Contracts and Property Management has recently made fcur awards in cor:jurction with the FY 1993 Small Business Innovation Research Program. Two awards were made to Shonka Research Associates, Inc., for " Continued Development of a Contamination Monitor Using Visual Identification" and "Special Monitor Detector Development for Respirator Contamination." The third award was made to Radiation Safety Engineering, Inc., for a project entitled "A Simple Method for Solubility Testing of Radionuclides on Air Filters." The fourth award was made to ENSCO, Inc., for " Seismic Monitoring Systems for Local and Regional Network."

Crime Prevention Presentation (Panel Discussion 1 The Division of Security will host Crime Prevention Presentations for NRC employees on October 6, 1993 at 10:00 A.M. at OWFN, and on October 13, 1993 at 2:00 P.M. at the Phillips Building. The panel will consist of representatives 4

from the Montgomery County Police Department, the General Services Administration Federal Protective Service, C&P Security, and the Metro Transit Authority Police.

The presentation will last approximately one hour.

1 Office of Enforcement Items of Interest Week Ending September 3, 1993 Sianificant Enforcement Actions Yhis is to inform the Commission that a Notice of Violation and Proposed Imposition of Civil Penalty in the amount of $200,000 was issued on August 31, 1993 to the Cleveland Electric Illuminating Company (Perry, Unit 1).

This action was based on the licensee's failure to correct cleanliness problems in the suppression pool and drywell which resulted in the clogging of the RHR system suction strainers as well as on the licensee's inadequate corrective actions once those problems were initially identified. The resulting corrective action violation which includes three examples has been classified at Severity level III. The base civil penalty of $50,000 has been escalated a total of 300 percent.

Escalation of 50 percent was warranted for NRC identification, 50 percent mitigation was warranted for the licensee's ultimate corrective actione,100 percent escalation was warranted for the licensee's past poor performance, 100 percent escalation was warranted for the prior opportunity to identify the probleri earlier provided by an NRC information notice, and an additional 100 percent escalation was warranted for the duration of the problem which extends back to 1987.

A Notice of Violation and Proposed Imposition of Civil Penalty in the amount of 55000 was issued on September 1, 1993, to Nondestructive Inspection Service, Inc., Hurricane, WVA. This action was based on a Severity Level III violation that involved the failure to conduct an adequate survey during radiographic operations at a temporary job site.

The base civil penalty for this Severity Level III violation is $5,000.

Escalation of 50 percent was applied for the factor of identification because the violation was identified by the NRC. Mitigation of 50 was warranted for corrective action. No further adjustments were warranted. Three other violations were cited and categorized at Severity Level IV for violations related to transportation requirements.

j A Notice of Violation and Proposed Imposition of Civil Penalty in the amount 3

of $12,000 was issued on or about September 1,1993, to Edwards Pipeline Testing, Inc., Tulsa, OK. This action was based on repetitive willful failures to perform quarterly audits on radiography personnel.

The base civil penalty of $8,000 for a Severity Level II violation was escalated 50% for poor corrective action.

Civil Penalties Paid

)

Commonwealth Edison Company (Quad Cities) paid the civil penalty in the amount of $100,000 which was issued on July 30, 1993.

This action was based on two violations involving the inoperablity of emergency diesel generators which have each been classified as Severity Level III violations.

In the first, the shared emergency diesel generator was inoperable under certain conditions, due to a design deficiency in some associated logic circuitry.

In the second, the Unit 2 emergency generator was determined to be inoperable because the i

associated cooling water pump was incapable of performing its function.

In

each case, the escalation and mitigation factors were considered.

In the first case, the base civil penalty of $50,000 was escalated a total of 50 SEPTEMBER 3, 1993 ENCLOSURE K

percent to $75,000 after partially offsetting escalation for prior opportunities to identify with mitigation for licensee identification.

In the second case, the base civil penalty was mitigated 50 percent to $25,000 based upon mitigation for prior good licensee performance. (EA 93-127)

New York Power Authority (Indian Point 3) paid the civil penalties in the amount of $300,000 which was issued July 21, 1993. The action was based on three Severity Level III violations related to the failure to design and maintain the ATWS mitigation system in a reliable manner, violations of procedures during mid-loop conditions, and violations of various technical specifications. Office of Consolidation Items of Interest Week Ending September 3,,

1993 Second Buildina Status During the week, OMN1 reached 50 percent completion on the interior of Two White Flint North (TWFN). Dynalectric, a sub-contractor to OMNI, began installing telecomunications conduit through the P2 level of the One White Flint North garage. The eight 4" conduits will house voice and data comunications lines linking the two buildings.

On August 31, the Director met with NTEU stewards and OP and OGC staff to discuss the status of the TWFN project and related issues of interest to the union.

On September 1, the Director attended a meeting of the TWFN " Move Team" in i

Bethesda to discuss the construction and occupancy schedule.

On September 1, David Kraisler of Scottsdale, Arizona, began assembling his art creation entitled "Architype Circular" on the TWFN Plaza lawn. The large ferro-cement and Cor-Ten steel structure was comissioned by Lerner Enterprises after consultation with the NRC Comissioners in keeping with an agreement made by White Flint North limited Partnership with the Montgomery County Planning Board j

to provide public art as a part of the two-building complex.

Subconmittee of Water Resources Meetina Maria Lopez-Otin, Federal Liaison, attended a special meeting of the Subcommittee on Water Resources (SWR) on August 19, 1993. The meeting was held to respond to Dr. Jack Gibbon's, Science Advisor to the President, request for SWR participation in a Presidential initiative regarding, among other things, flood plains and the reconstruction of the Mississippi Valley.

The SWR will feed its input to the White House working group. Of interest to the NRC would be any initiative that would affect the water flow in areas where there is a nuclear power plant. Ms. Lopez-Otin has apprised the appropriate NRC staff of this potential development.

Response To Hurricane Emily On August 30, 1993, in response to Hurricane Emily, Robert Trojanowski, Region II State Liaison Officer, reported to the State Emergency Operation Center (E0C), Raleigh, North Carolina. On August 31, 1993, after any threat to the Brunswick facility had passed, Mr. Trojanowski reported to the State E00, Richmond, Virginia, where he remained until the hurricane posed no further threat to the east coast -- September 1, 1993. Overall, communication and i

coordination between Mr. Trojanowski, the Regional Base Team, State agencies (Emergency Management, Radiological Health, and others as appropriate), and representatives of Virginia Power were excellent.

In retrospect, since Emily produced no major damage, this turned out to be an excellent exercise and test of the Regional Hurricane procedures.

Both Governor Hunt, North Carolina, and Governor Wilder, Virginia, expressed their sincere appreciation for the NRC's interest and rapid response to Hurricane Emily.

Meetina with Utah Officials on Atlas Site John Surmeier, Acting Assistant Director for State Agreements Program, Ramon Hall, Director, Uranium Recovery Field Office, and Charles Hackney, Region IV State Liaison Officer, met with William Sinclair, Director, Division of Radiation Control, Utah Department of Environmental Quality, and other State officials on September 1,1993 in Salt Lake City, Utah. The purpose of the meeting was to discuss State concerns with the proposed reclamation plan for the Atlas Minerals Corporation eranium mill tailings site in Moab, Utah.

The State believes that a new environmental impact statement is warranted to examine issues related to the impacts of capping the tailings in place or moving the tailings for offsite remediation.

Meetina with Conaressional Staff On September 2, 1993, the Chairman's Executive Assistant, the Assistant General Counsel for Hearings and Enforcement, and the Director of the Office of State Programs met with two staff members, Dan Adamson and Dean Tousley, of the House of Representatives Natural Resources Committee.

Issues discussed included NRC's assessment of the California Agreement State Program (with emphasis on the State's capability to conduct the low-level waste regulatory SEPTEMBER 3, 1993 ENCLOSURE M

program), the status of the adequacy and compatibility of all Agreement State g

programs, and briefly, matters associated with the August 2 Synar hearing and the appointment of a new Office of State Programs Director. The staff members i

requested that in the future they receive copies of NRC correspondence addressing licensing of the Ward Valley site; NRC agreed to do so.

Office of International Programs Items of Interest Week Ending September 3,1993 Foreton Visitors On Tuesday August 30 Chairman Selin, along with senior officials from the Department of Energy and of the Florida Power and Light Company, hosted Russian Premier Viktor Chernomyrdin, Minister of Atomic Energy Mikhailov, Deputy Premier Shokhin, Environnental Minister Yablokov and Russian Ambassador Lukin at the St. Lucie Nuclear Power Plant in Florida. The purpose of the visit was to demonstrate American nuclear safety and training technology to the political leaders of Russia.

It was part of an intensive program designed to heighten the Premier's sensitivity to certain impediments in our cooperation program in high technology, and to seek resolution of these difficulties at the highest political level. The visit was a follow-on to the Vancouver summit meeting between Presidents Clinton and Yeltsin which proposed a joint commission on energy and space to be chaired by Vice President Gore and Premier Chernomyrdin. The commission will establish a dialogue between the two governments at the political level for expanding fruitful cooperation in energy and space technology. A key issue discussed at the first meeting in Washington (September 1 and 2,1993) was nuclear power plant safety.

NRC participated in the Commission meeting and worked with the Departments of State and Energy preparing policy papers in the nuclear safety area.

These papers set out arguments to persuade the Russians to introduce risk reduction measures in (and to eventually close) their least safe plants; give greater autonomy and authority to the regulatory body Gosatomnadzor (GAN); improve operational training through the use of simulators; develop emergency operating procedures; complete arrangements for liability protection to enable U.S. industry to provide safety assistance, and gradually replace their aging fleet of RBMK (Chernobyl-type) reactors with modern PWR's equipped with containment and augmented safety systems on the model of most western reactors.

On Thursday, Mr. Yoichi Koga, Director of International Affairs on Nuclear Power Safety, ANRE/MITI met with Carlton Stoiber to discuss the international nuclear safety convention and associated topics.

Following this meeting, Mr.

Kooa met with Ronald Hauber and other OIP staff to discuss MITI-NRC relations and the renewal of the NRC-MITI-STA Arrangement.

Babcock & Wilcox (B&W) Anollo Site Decommissionino On August 31, 1993, an exit meeting was held with R. V. Carlson, President, B&W Nuclear Environmental Services, Inc. and members of his staff, to discuss the findings of the latest NRC inspection of the decommissioning of the Apollo Site, and operations at the Parks Township facility.

In attendance were Thomas T. Martin, Region I Regional Administrator, members of his staff, and representatives of NMSS. This meeting was open to the public. At the end of this meeting, NRC staff fielded questions from the public.

As a result of the inspection, three apparent violations were noted which involved failure to comply with license conditions or the approved decommissioning plan. This included 1) Failure to maintain the approved organizational structure; 2) Failure of the Manager of Pennsylvania Nuclear Services Operations to meet the required minimum qualifications; and 3) Failure to maintain covers over contaminated soil piles. Other topics included revisions in the final sampling and remediation protocol, and status of the decommissioning project.

Georgia Power Company's Plant Hatch facility to present Operator License Certificates to newly licensed operators.

Duke Power Coreany On September 2, an Augmented Inspection Team (AIT) was dispatched to the McGuire nuclear power plant to review the circumstances associated with the failure of a steam generator secondary side drain line inside the containment of Unit 2.

One person suffered burns on the leg from the steam.

He was treated in a local haspital and released.

The NRC has issued a press release.

Tennessee Valley Authority On September 2, the Director, Division of Reactor Projects, the Associate Director for Projects, NRR, and members of their staffs, were at the Sequoyah site attending the ORAT exit meeting.

On September 3, the Regional Administrator and members of the Region 11 staff were at the Watts Bar site attending the (open) PAC-AQ exit meeting.

On September 2, the Director, Division of Reactor Safety, was at the Farley facility attending the Service Water Exit meeting.

Hurricane Emily Update Region II began monitoring Tropical Storm Emily on Saturday, August 28, 1993, when it was about 800 miles southeast of the Brunswick site.

Region II emergency response personnel maintained communications throughout the weekend with Region 11 management, the HQ Operations Officers and Region I emergency preparedness staff. During the weekend, i

Region II entered its hurricane response procedure which contains a checklist of actions to be taken to respond to hurricanes. The Resident Inspectors at the Brunswick and Surry sites were polled to assure their availability for responding to the hurricane. Both sites had available at least three Resident Inspectors. The storm reached hurricane force when it was more that 500 miles southeast of Brunswick and traveling at about 10 miles per hour.

On Monday morning, August 30, the State Liaison Officer was dispatched to the state Emergency Operations Center in Raleigh, North Carolina and the Region II Emergency Response Vehicle was dispatched to the vicinity of the Brunswick site. The response team in the vehicle consisted of a Section Chief from the Division of Reactor Projects, a license examiner and a SEPTEMBER 3, 1993 ENCLOSURE R

radiation protection inspector. In addition to the satellite pagers worn i

by the team, the response vehicle contained two cellular telephones, a i

high frequency radio and a portable satellite telephone. The vehicle also contained the necessary food, water and equipment to make the team self 5

sufficient for at least five days. Periodic communications with the team j

in the response vehicle and with the State Liaison Officer were maintained throughout their response efforts. High frequency radio co=unications i

between the NRC installed radio at the Brunswick site and the Region II IRC were successful. Similarly, radio comunications between the response vehicle in the vicinity of the Brunswick site and the IRC were successful.

During the day on August 30, the Region continued to monitor reactor and fuel facility storm preparations. At 16:40 on August 30, 1993, Region II entered the MONITORING mode. Hurricane Emily continued to move toward the 1

Cape Hatteras area. At 06:10 August 31, 1993, Surry declared a i

Notification of UNUSUAL EVENT due to the declaration of a hurricane warning for the site vicinity. At that time, an open ENS telephone line was established between the Surry site, NRC HQ Operations Center and the Region II Incident Response Center. With the hurricane less likely to strike the Brunswick area, the Region II emergency response vehicle was dispatched to the Surry site. Also, during the morning of August 31, 1993, the State Liaison Officer moved to the Virginia Emergency Operations Center in Richmond. At 13:15 hours, the response team reported they were j

within 10 minutes of the Surry site. At about 13:25, the State Liaison j

Officer arrived in Richmond. At 14:30, as a test, a satellite telephone link was established between the response team and the Region II IRC from j

the parking lot of the Surry site.

l On September 1,1993, at 05:00 hours, the hurricane warning for the Surry j

area was discontinued and at 05:15, the Surry site terminated their j

UNUSUAL EVENT. At 09:30 hours, the Region II IRC returned to NORMAL mode.

Russian Prime Minister Visit to St. Lucie On Tuesday, August 31, the Prime Minister of the Russian Federation, Viktor Chernomyrdin, visited the St. Lucie Nuclear Power Plant.

Secretary of Energy Hazel O' Leary. The visit to St. Lucie was hosted by i

NRC Chairman Ivan Selin; James L. Broadhead, Chairman and Chief Executive i

Officer of Florida Power and Light Company (FPL); and Jerome Goldberg, 1

Presidert of the Nuclear Division of FPL. The visit included a plant tour and a press conference. The press conference was covered by local TV and l

newspapers.

The Prime Minister's visit was in conjunction with the first meeting of j

the United States / Russian Federation Joint Comission on Technol.)gical Cooperation in Energy and Space. President Clinton and President Yeltsin I

agreed to establish the Commission at their Vancouver Sumit on April 4, l

1993 and designated Vice-President Gore and Premier Chernomyrdin jointly j

to chair the body. The Joint Commission, which will begin its meetings on j

Wednesday, September 1 in Washington, is intended to provide an impetus for enhanced cooperation between the two nations in the commercial, scientific and industrial applications of energy and space technology.

D. C. Cook On September 2 and 3, 1993, the Regional Administrator and members of his staff visited the Donald C. Cook nuclear plant to perform walkdowns, interview personnel, assess current performance and trends, and meet with the Vice President, Nuclear for American Electric Power.

Prairie Island On September 1,1993, the Deputy Regional Administrator and other members of his staff held an Enforcement Conference with Northern States Power Company to discuss the circumstances surrounding the removal of a guard at the Prairie Island Nuclear Plant. A Department of Labor Administrative Law Judge had recently ruled that the removal constituted employee discrimination as defined in 10 CFR 50.7.

Telechonic Enforcement Conference An enforcement conference was held August 31, 1993, to review the findings related to an inspection conducted on July 14-16, 1993. The conference focused on the licensee's lack of maragement oversight of the radiation safety program which resulted in apparent violations of NRC requirements.

Particular attention was focused on the licensee's failure to implement the quality management program.

Rocky Mountain Gamma Knife Center Update By letter dated August 13, 1993, the Colorado Department of Health I

notified the Region IV State Agreements Program Officer of an incident which occurred at Rocky Mountain Gamma Knife Center (RMGKC), in Denver, Colorado, on July 8, 1993. The incident involved an error in administering radiation to a patient who was undergoing gamma stereotactic surgery for an arteriovenous malformation.

RMGCK reported that as a result of an error which occurred while data was entered into a computerized treatment planning system, radiation was administered to an area of the patient's brain which was not intended for irradiation, and the patient received a dose of approximately 2.5 Gray to the brainstem.

Based on initial reports received from RMGKC, it appeared that while treatment parameters were entered at the treatment planning console, an error message appeared on the console screen.

RMGKC staff continued to enter treatment parameters and experienced additional difficulty with data entry. The Colorado Department of Health has requested additional information from RMGKC regarding use of the treatment planning system, and will continue its investigation of this incident.

In addition, Colorado Department of Health and the Food and Drug Administration (FDA) have been in contact with the manufacturer of the computer treatment planning system. The manufacturer has conducted an investigation at RMGKC and is expected to submit a report to the Colorado Department of Health and FDA regarding the cause of the error.

Region IV, Office of Nuclear Materials Safety and Safeguards, and Office of State Programs staffs will continue to follow the FDA's and Colorado Department of Health's investigation of this case.
