APPENDIX A NOTICE OF DEVIATION Duquesne Light Company Docket No. 50-412 Beaver Valley Power Station, Unit 2 License No. NPF As a result of the inspection conducted-on May 5 - June 22,1990, and in ac-cordance with the General Statement of Policy and Procedure for NRC Enforcement Actions, 10 CFR Part 2, Appendix C, (Enforcement Policy) (1989), the following deviation was identified:

In response to question 480.13 of the Unit 2 Final Safety Analysis Report, the licensee committed to_have procedural controls to ensure that the reactor cavity transfer canal drain line flange would be removed during all operations other than refueling.

Contrary to the above, the licensee failed to meet their commitment in that, on and before. June 22, 1990, no procedural controls existed to ensure.that the flange would be removed during power operation.

Per 10 CFR 2, Appendix C, Section V.H.(2), we request that you. provide us, within 30 days, a written explanation or statement describing: 1)' corrective steps taken (or planned), 2) the results achieved, and 3) the date when corrective action will be completed for the above deviation.

F EXECUTIVE

SUMMARY

. Plant Operations- The inspector reviewed two events, The first event was a Unit 1 overpower event; no deficiencies were identified. In the second event,:

i a deviation from a commitment to the NRC was identified which involved the discovery of'an installed flange on the reactor-cavity-transfer canal drain line. Minor-deficiencies were identified during a walkdown of the Unit 1-Emergency Diesel Generators. Housekeeping at both units was good.

Radiological Protection Routine review of this area identified .ru) noteworthy observations.

Surveillance and Maintenance A non-cited violation was identified concerning :p failure to adequately test the valve stroke times of certain containment isolation valves. The inspector found the Unit 1 appearance improvement program to be good;- however, the inspector identified some minor concerns in regard to this effort.

Emergency Preparedness Routi,ne review of this area identified no. noteworthy observations.

Security Routine review of this area identified no noteworthy observations.

Engineering and Technical Support The inspector reviewed licensee activities associated with reliability improvements for the Unit 1 Emergency Diesel Generator Air Start System and resolution of Recirculation . Spray Heat Exchanger '

River Water Expansion Joints. Licensee corrective action and engineering evalu-ations for these' activities were adequate.

Safety Assessment / Quality Verification The inspector found corrective actions for a previously identified violation to be not yet complete.
