' This tran'smits the May 16 through June 19, 1990, routine inspection findings.
of Messrs. R. Freudenberger, J. Trapp, R. Fuhrmeister and W. Olsen at the Maine
Yankee Atomic Power Plant, Wiscasset, Maine. Those findings were besed on
observations of activities, interviews, and document reviews, and have been
discussed with Mr. Blackmore of your staff.
Enclosure 1 is a Notice of Violation based on the. inadvertent drainage of the
reactor coolant system pressurizer on June 4,1990.
In this case, it appears
that the operators _had disparate indications of pressurizer level and resolution
of-the_ disparity was practicable but not accomplished before additional steps
were1 ta ke n ._ - It also appears that the operators' reduced pressurizer level by'a
means not authorized under existing plant conditions, and that procedure inade-
quacy was a factor in this' error. These considerations resultedlin our decision
to not process-this matter as a possible non-cited violation, notwithstanding
-
.
the -fact thatJa significant margin and several barriers remained to assure ade-
quate core cooling.
In this case, we especially: desire that your actions to
prevent recurrence of.similar personnel and procedural errors be formally sub-
mitted in response to_the enclosed Notice of Violation.
