Subject: Inspection 50-293/82-12
This refers to the routine safety inspection conducted by Mr. J. Johnson of
this office on April 5 - May 9,1982, at the Pilgrim Nuclear Power Station,
Plymouth, Massachusetts, of activities authorized by HRC License flo. DPR-35
and to the discussions of our findings held by Mr. Johnson with Mr. R. Machon
and other members of your staff periodically during the inspection and at the
conclusion of the inspection.
Areas examined during this inspection are described in the NRC Region I In-
spection report enclosed with this letter. Within these areas, the inspection
consisted of selective examinations of procedures and representative records,
interviews with personnel, and observations by the inspector.
Based cn the results of this inspection, it appears that one of your activities
was not conducted in full compliance with NRC requirements, as set forth in
the Notice of Violation, enclosed herewith as Appendix A.
This item of non-
compliance has been categorized into the level described in 10 CFR 2. Appendix
C and the Federal Register Notice 47 FR 9987 dated March 9,1982. You are
required to respond to this letter; and, in preparing your response, you
should follow the instructions in Appendix A.
In your response, please include those actions taken or planned to ensure that
actions specified by the Technical Specifications in the event of inoperable
equipment are followed.
Include cases where the loss cf one component may
affect several sections of the T.S. and where the loss may involve instrument
channels and logic systems.
The item described in the attached Notice of Violation Appendix A, involving
reactor vessel water level instrumentation, is classified as a Severity Level
IV violation. As indicated in Supplement I of the NRC Enforcement Policy
(10 CFR 2, Appendix C), significant violations of this type are normally
classified at Severity Level III. However, after careful consideration of
the factors involved in this instance including plant and facility status
and compensatory monitoring measures we have exercised our judgement under
the NRC Enforcement Policy and have classified this violation as a Severity

Level IV. Similar violations of this type in the future may result in
additional enforcement action. Furthemom, we also note that the violation
was identified by you and reported in LER F" 011/01T; however, we are con-
cerned about the occurrence of this event and the need for appropriate
training of the operating staff to prevent recurrence. Accordingly, we will
schedule an enforcement conference with you in the near future in order to
discuss our concerns in this regard.
In accordance with 10 CFR 2.790(a), a copy of this letter and the enclosures
will be placed in the NRC Public Document Room, unless you notify this office,
'
by telephone, within ten days of the date of this letter and submit written
application to withhold ir.fomation contained therein within thirty days of
the date of this letter. Such application must be consistent with the require-
mentsof2.790(b)(1).
