June 29, 1982 PRELIMINARY NOTIFICATION OF EVENT OR UNUSUAL OCCURRENCE PNO-II-82-72A This preliminary notification constitutes EARLY notice of events of POSSIBLE safety or public interest significance.

The information is as initially received without verifi-cation or evaluation, and is basically all that is known by the Region II staff on this date.

SUBJECT:

EXTRACTION STEN 4 LINE BREAK This PN updates PNO-II-82-72, issued on June 28, 1982.

The break which initiated the event occurred in the outside radius of an elbow of a 24-inch line which branches off the 42-inch high pressure turbine exhaust.

The failed material appeared to be due to thinning from steam erosion.

The failure was in the base metal rather than a weld area. The break measures approximately four square feet.

Both the reactor and the turbine were manually tripped, although the automatic turbine trip functioned following the reactor trip. The reactor cooldown proceeded without incident. An electrical equipment panel severely damaged by the steam line break did not affect safety-related equipment or other functions critical to reactor control.

Preliminary indications are that the cooldown did not exceed technical specification limits.

Three Duke employees were injured, having suffered steam burns. One was treated on site, and the other two were admitted to a hospital overnight and were released today.

Although the event had no radiological implications on or off site, appropriate pre-cautionary actions were taken.

The licensee today is beginning to examine comparable steam lines at Oconee Unit 1, which is currently operating.

Unit 3 is down for refueling and modifications.

Region II is monitoring the licensee's corrective actions and will keep other NRC offices informed.

Media interest has occurred. Duke issued a press release, and both the licensee and the NRC have responded to press inquiries.

The State of South Carolina has been informed.
