I APPENDIX A NOTICE OF VIOLATION Florida Power Corporation Docket No. 50-302 Crystal River 3 License No. DPR-72 As a result of the inspection conducted on March 4-26, 1982, and in accordance with the NRC Enforcement Policy, 47 FR 9987 (ibrch 9,1982), the following violations were identified.
A.
Technical Specification 4.6.1.2.d requires type C leakrate testing to be conducted on containment isolation valves at intervals no greater than 24 months.
Contrary to the above, as of Ibrch 22, 1982, leakrate testing on containment isolation valves (SAV-23, SAV-122 and LRV-46), was not conducted within the required 24 month interval.
This is a Severity Level IV Violation (Supplement I.).
B.
Technical Specification 6.8.1 t equires that applicable procedures recom-mended in Appendix "A" of Regulatory Guide 1.33, 1972, be established and implemented.
Procedure AI-500, Conduct of Operations, paragraph 4.1 requires the signature or initials of the person performing a step-by-step operating procedure at each step of the procedure.
Contrary to the above, during the period of March 7 and 8,1982, the person performing step-by-step operating procedure OP-403, Chemical Addition System, did not sign or initial any steps within the procedure.
This is a Severity Level V Violation (Supplement I.).
Pursuant to the provisions of 10 CFR 2.201, you are hereby required to submit to this office within thirty days of the date of this Notice, a written statement or explaration in reply, including:
(1) admission or denial of the alleged viola-tions; (2) the reasons for the violations if admitted; (3) the corrective steps which have been taken and the results achieved; (4) corrective steps which will be taken to avoid further violations; and (5) the date when full compliance will be achieved.
Consideration may be given to extending your response time for good cause shown.
