Date: PROPOSED ACTION AND/OR PISOLUTION: Reform area of void and pressure grout with Embeco 636. Conduit will be placed in void through the form (see attached Figure II. Det. A.) for bleeding trapped air. The grouting operation will continue until grout g flows from the bleeder. The form will rarain in place the. duration of the cure period, then removed and the bleeders trinmed flush to the call. 'Ihe vibrator should be placed in the concrete rapidly and should penetrate at least six inches into the preceding layer if there is one. 2. The vibrator should be held stationary for about 5 to 15 seconds. 3. The vibrator should not be inserted within two feet of any unconfined edge. 4. In thin slabs the vibrator should be sloped toward the horizontal . as necessary so that it is fully embedded. 5. '1he distance between insertions should be such that the areas visibly affected by the vibrator overlap by a few inches. 6. When concrete piles up (for example at the end of the pump line or a trenie) this pile can be leveled by inserting the viarator into the pile to " melt" it down. But concrete should not be noved horizontally with the vibrator. 7. If concrete must be noved horizontally a shovel should be used. AII

The Weld Data Report ~may, at th'e discretion of the Morrison Construction Company, be utilized when controlling activities other than stated above.

2.0 REFERENCES

The Quality Assurance Program, Section 7 - Process Control, Field Fabrication and Erection. 3.0 RESPONSIBILITY The Project Engineer, Welding Supervisor, Quality Control Supervisor, and P! ping . Superintendent are responsible for implementing the designated functions described in this procedure. 4.0 INSTRUCTION 4.1 The Weld Data Report (MCCO Fonn PC-13) is utilized as a traveler or is included in a traveler package as a Process Control Checklist. 4.2 The approved Mini Spec will be used to complete the basic instructbn requirements on the Weld Data Report. LaSalle County Station A,,no m y Specification, applicable codes, approved welding procedures, and the Quality Assurance Program. 4.3 The Weld Data Report consists of eight identical, separate, colored sheets, each sheet separated by a carbon insert. Caution should be exercised in handling or marking, as each indication will transpose on all sheets. 4.4 A Weld Data Report will be issued for each production weld of nominal pipe size 21" and larger for piping systems governed by Section ill of the ASME Boiler & Pressure, Vessel Code. 4.5 The Quality Control Review Log (MCCO Form PC-58) will be utilized and maintained \\, in conjunction with the Weld Data Report to monitor the work. 5.0 PREPARATION OF WELD DATA REPCRT 5.1 The designated Project' Engineering Department personnel, utilizing the approved Mini-Spec,will complete the assigned responsibilities as detailed on Sections A,G, and I of the Weld Data Report, and sign the completed sections. Similarly~, columns 1 through 5, and 11 through 14 when applicable, on the Quality Control Review Lcg will be completed by the designated personnel of the Project Engineering Department. The revision numbers for the Mini Spec and Field Procedure should be included in Section A of the Weld Data Report. Revision numbers to the drawing, shall be indicated on the Weld Data Report at the time of fabrication. The Project Engineering Department will then transfer to the Welding Department the Weld Data Report and the Quality Control Review Log, MCCO PC-58.

Column 6 of the Quality Centrol Review log shall be completed by the Welding Department. Section B of the Weld Data Report and Column 6 of the Quality Control Review Log should indicate the revision of the d welding procedure to be used. The Welding Department will then transfer to the Quality Control Department the Weld Data Report and the Quality Control Review g Log, MCCO PC-58. [ 5.3 The Quality Control inspector shall review the completed sections cf the Weld Data (. Report, and shall enter the Quality Control and Authorized I.n spector's inspection / Hold Points, when applicable, in Section J of the Weld Data Report. When the review of the Weld Data Report for compliance with Section til of the ASME Boiler & Pressure Vessel Code and the D' sign Specification is satisfactory, the Quality Control Inspector e shall also review columns 1 through 6 of the Quality Control Review Log with the con-tents of the Weld Data Report at this time. 5.4 Should any revisions or corrections.be made to the Weld Data Report, these changes shall be made and initialed by the person responsible for the affected section of the Weld Data Report. Quality Control shall review all changes and revisions to the Weld Data Reports for correctness and compliance. These revisions and/or changes shall be made available to the Authorized. inspector. The Quality Control Welding Inspector shall visually examine the final weld, indicate his acceptance for final NDE on Section H, and visual examination of final weld on Section J of the l I ' Weld Data Report. 6.8 8 Copy 2 of the Weld Data Report will be removed from the protective envelope when 1 the weld is verified as complete, and transferred, via the Document Station, to the Quality Control Department.
