INVESTIGATION PLAN AND SCHEDULE REGARDING GAO FINDINGS DISCLOSED DURING A REVIEW OF RIII INSPECTION AND SITE CONSTRUCTION ACTIVITIES AT LA SALLE COUNTY STATION Below is an outline of the investigation plan and schedule developed to respond to GAO findings disclosed during their review of RIII inspection activities at La Salle. Also included, as part of this plan, is an investigation into the statements taken from site construction personnel during confidential interviews with GAO representatives. 1. Investigation Plan 1.1 Review GAO findings disclosed during their site check of RIII inspection activities to become thoroughly familiar with the findings and conclusions. 1.2 Review statements obtained during GA0 confidential interviews with site construction personnel. -.o 1.3 Review SAR commitments, as necessary, to become familiar with technical and quality requirements. 1.4 Discuss inspection details with each Construction Branch l inspector who performed inspections related to the GAO findings. Obtain all pertinent information available regarding the inspection. 1.5 Conduct offsite discussions with the site construction l 1 personnel who made statements to the GA0 representatives. Obtain details and specific information regarding their concerns. Perform a site inspection to resolve each individual concern to the maximtm extent possible. 1.6 Conduct a detailed investigation into the GAO findings relative to RIII inspection activities. \\ Statements Concerning Quality f Ef plan t Construction The following statements concerning the quality of plant ecastruc-tion were made by individuals employed at the LaSalle facility during confidential interviess with GAO representatives. All of these indivi-duals indicated that they did not want their identity disclosed publicly or to site management. To protect the confidentiality of these indivi-dunic--as GAO has pledged--you are requested to contact them off-site should you require to discuss their statements with them. . Name: f( Phone: Code: 33-C/56 Sta tem en t: Incorrect vibrktion.cf conc' rete Concrete is over-vibrated-at the site. I have never seen concrete vibrated as it is at this job. Con-crete is vibrated until all the accregate has set-tied at the bottom and foam has formed on the top. This results in some areas of the concrete being sc! c. Some areas are alnest like chalk shich can be scrared away with a small bolt or nail. Evi-dence of incorrect vibration is most noticeable where wall pours joins one another. Those areas that have been detected have been repaired, how-ever, it is doubtful if all like areas have been detected. l Concretecsamples are.taken prior to vibrations.- 1 If such ' tests were_mndc1before and af ter vibra-tion the-results _would_be. surprising. -Thie-would / be surprisinc. Example: On an open butt weld the welder. has to be qualified for open butt ac well as backing plate. Sta tem ent: Some cable pulls in the service building gg g have been pulled beyond the-27d*'11mit' without being pulled through a special sleeve or out into a junction box. Cable is often pulled right through a junction box. He has seen junction boxes and conduit pulled off the sall because of the strain on cables shen trying to go through core than three 90* bends at once.

-3.- Frocedures. The foremen don't appear to know what welding procedurcs apply. When asked whether a certain cable tray should be full welded or tacked up, the foremen sometimes tell him to do the work as he thinks best and sometime tell him to look at the v.ay other trays are done. He has worked on trays located at elevation 735,1_in_es' 254 264 27.on J.be colu=n line, seperating the auxillary building and the turbine building. Of ten the foremen say sloppy work is okay because it is in some place hard to see. The foremen say they will never see it; it is okay. Tu e,,, 'htngor. am was positioned such that it hit the cable tray nuts. The fore =an bent the hangar am and told him to weld it rather than moving the arm to fit bety.een the nuts. It v.as next to the wall so they will never see it. In February, 1977, while eworking for Morrison, the iitater--intale-valves--for-enit-#2 ^werQt66-large forjthe pipe-so-the-12ndiameter-pipes were-split back-and then spread to acco=odate the valve. The v. elds tc repair the pipe of ten cracked because of the cold weather 1 Som etim es the actual cracking of the pipe welds would be loud enough to shake the building. There were so many repair welds that the pipe now _looks like dried cocked spaghetti.
