Inspection 50-220/82-01
This refers to the routine, safety inspection conducted by Mr. S. Hudson of this
office on January 1 to February 7,1982, at Nine Mile Point, Unit 1 Scriba, New
York, of activities authorized by NRC License No. DPR-63 and to the discussions
of our findings held by Mr. Hudson with Mr. T. Roman of your staff at the con-
clusion of the inspection.
Areas examined during this inspection are described in the NRC Region I Inspec-
tion Report which is enclosed with this letter. Within these areas, the inspec-
tion consisted of selective examinations of procedures and representative records,
interviews with personnel, and observations by the inspector.
Our inspector also verified the steps you had taken to correct the violations
brought to your attention in letters dated September 12, 1979, October 15, 1979,
and September 10, 1980. We have no further questions regarding the steps you
took to correct the items addressed in the October 15, 1979 and September 10,
1980 letters. With regard to items A, B, and D of the September 12, 1979 letter,
these items were not fully corrected.
Items A, B, and D were identified during previous inspections of your licensed
activities on July 20, 1979, and invol ed an incorrect system valve lineup, failure
to label valves and include them in the valve check off list, and incorrect piping
and instrument drawings. These violations were documented in the enclosure to
our letter dated September 12, 1979.
Your letter to this office dated October 2,
1979 stated that your corrective action would be completed by January 1,1980.

However, based on our January 1 to February 7,1982 inspection, it appears that
your corrective action system was not effective in that these items were not fully
!
corrected. As a result, in response to this letter, you should provide a descrip-
tion of the actions taken to ensure that identified violations will be completely
corrected and will not recur.

Based on the results of this inspection, it appears that one of your activities
was not conducted in full compliance with NRC requirements, as set forth in the
Notice of Violatiun, enclosed herewith as Appendix A.
This violation has been
categorized by severity level in accordance with the NRC Enforcement Policy (10 CFR 2, Appendix C) published in the Federal Register Notice (47 FR 9987).
You
are required to respond to this letter and in prepar' g your response, you should
follow the instructions in Appendix A.
An additional apparent violation of NRC requirements relating to the design and
operation of the Containment Atmosphere Dilution System is discussed in the in-
spection report enclosed with this letter. We are considering this item for
appropriate enforcement action. We will be addressing this matter under separate
correspondence at a later time. However, as part of our review of this matter,
we want to discuss with you circumstances surrounding this apparent violation.
You will be contacted subsequently regarding a mutually acceptable date and time
for a management conference.
In accordance with 10 CFR 2.790(a), a copy of this letter and the enclosures will
be placed in the NRC Public Document Room unless you notify this office, by tele-
phone, within ten days of the date of this letter and submit a written application
to withhold information contained therein within thirty days of the date of this
letter. Such application must be consistent with the requirements of 2.790(b)(1).
The telephone notification of your intent to request withholding, or any request
for an extension of the 10 day period which you believe necessary, should be made
to the Supervisor, Files, Mail and Records, USNRC Region I, at (215) 337-5223.
The responses directed by this letter and the accompanying Notice are not subject
to the clearance procedures of the Office of Management and Budget as required by
the Paperwork Reduction Act of 1980 PL 96-511.
