APPENDIX A NOTICE-0F V10LATION Niagara Mohawk Power Corporation Docket No. 50-220 Nine Mile Point Nuclear Power Station, Unit 1 License No. DPR-3 As a result of the inspection conducted on January 1 - February 7,1982, and in accordance with the NRC Enforcement Policy (10 CFR Part 2 Appendix C) (47 FR 9987) (March 9,1982), the following violation was identified:

10 CFR 50, Appendix B, states in part, "VI. Document Control-Measure Shall be establish =d to control the issuance of documents such as instructions, procedures ar d drawings, including changes which 3rescribe all activities affecting qua:ity. These measures shall assure tlat documents, including changes, are r' viewed for adequacy..."

Contrary to the above, examination of drawings and procedures demonstrated that measures were not effective in that the installation of the drywell to torus differential pressure instrumentation and its isolation valves were not reflected in the documents listed below:

P&ID #C-18014-C, Reactor Containment Inert Gas Purge and Fill System",

Revision 15, dated December 6, 1980.

ISP-IC-21, " Pre-Startup Valve Lineup Check", Revision 4, dated June 29, 1981.

ISP-IC-23, " Integrated Leak Rate Test of the Primary Containment",

Revision 9, dated June 8, 1981.

This is a Severity Level V Violation (Supplement I).

Pursuant to provisions of 10 CFR 2.201, Niagara Mohawk Corporation is hereby re-quired to submit to this office within thirty days of the date of this Notice, a written statement or explanation in reply, including:

(1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation. Where good cause is shown, consideration will be given to extending your response time.
