Question 6:

Does the NRC staff consider it necessary for the purpose of its investigation to have lists of nonconformance and deficiency reports that specify weld number, nature of deficiency, etc.? If so, has NRC staff obtained lists of I

nonconformance reports, etc. containing information such as weld number and nature of deficiency report?

Response 6:

The independent regulatory review team has attempted to utilize every available documentation control tool to assist in selecting a valid and intel _ligent sample of records for review of the pipe welding & NDE issues raised by Congress. Although not a regulatory requirement, the team feels it is helpful to have lists of nonconformance and deficiency reports that specify, when L

appropriate, specific weld numbers and the natura of the deficiency, in that such information is useful and efficient to the dedication of inspection re-sources to those deficiencies of interest.

For example, the team had requested any information an Pullman-Higgins nonconformance reports (NCRs) that would identify deficiencies related to specific subject areas of inaccessible welds or " accepted-as-is" welds. As a result of this request, during the week of April ~ 9,1990, the licensee provided to the team a listing by NCR number and copies of NCRs which were identified to pertain to these subject areas.

These copies had been produced from the microfilm of the applicable NCRs.

The team was also provided a copy of a computer printout, dated March 27, 1990, of Pullman-Higgins nonconformance reports listing one-line entries of NCR numbers, dates and annotated subject descriptions.

This computer printout represented initial attempts by the licensee to search the available computer files for Pullman-Higgins NCR information and to determine whether specific keyword searches (e.g., " inaccessible," " accept-as-is", or " welds") would pro-vide meaningful and complete data. The licensee determined that a listing developed from keyword searches would be incomplete because the accuracy of the search is dependent upon whether the desired keyword had been placed in the computer file fields describing each nonconformance report.

Because of other inspection activities, the team did not really use the com-puter listing, dated March 27, 1990 although initial scanning of the printout was started.. Subsequently, the team learned of Dr. Myers' desire for more com-plete information and the resulting computer run, dated May 1, 1990, listing all nonconformance reports and deficiency reports generated by Pullman-Higgins.

A copy of this computer listing, which (it is understood) was offered to Dr. Myers,- was also provided to the team on May 2, 1990.

This later listing, which was considered more " user friendly," is being used by the team to select a sample of Pullman-Higgins NCRs for evaluation.

Such evaluation is in addi-tion to the team's review of those NCRs provided by the licensee with respect to the " accepted-as-is" welds or welds identified as " inaccessible," as discussed above, and any other NCRs provided in relation to specific weld packages and other records being reviewed by the team.
