v HIGHLIGHTS OF WEEKLY INFORMATION REPORT WEEK ENDING JUNE 1, 1990 Brunswick, Units 1 and 2 Both units are in cold shutdown pending resolution of the operator's qualifi-cation issue. Licensee management met with NRR and RII staff in Atlanta on May 24, 1990, to discuss their upgraded training plans for their licensed

operators and the root cause assessment regarding deficiencies identified during operator requalification examinations at Brunswick.

Presently, the licensee is conducting remedial training (2 shifts a day). The staff will send examiners to the plant on June 2 to va idate the test model.

All licensed operators will be reexamined when the licensee is ready, currently estimated as June 9 and'10, 1990.

l The licensee has formed a team to assess root causes of the training deficien-cies and will report their findings and corrective actions to the NRC staff by June 30. 1990.

GAO Survey of Implementation Activities for the low-level Radioactive Waste PoHey Amendments Act (LLRWPAA) i EDO, NMSS and GPA staff members met with GA0 representatives on May 30, 1990, regarding a GAO survey of Com)act State and Federal agency activities to implement provisions of the L.RWPAA. The survey was requested by the Senate Comittee on Governmental' Affairs. GA0 staff has identified twelve specific issues they plan to initially cover in the survey to be completed by the end of July. Based on the survey, they may review specific areas in further detail. Representative issues identified by GA0 include the. nature and extent of NRC. technical assistance provided to States and Compacts, criteria the' States and Compacts are using to select sites, types of disposal designs being considered and their environmental / health and safety impact, problems encountered by the States in developing new sites, ability of States and Compacts-to have new sites available by 1993 and the impact on generators if sites are not available, liability issues, storage of low-level waste (LLW) at utility sites disposal'of greater than Class C waste and the potential for the Federal government to override States and Compacts with more stringent recuirements on LLW disposal.

. United Nuclear Corporation (UNC). Naval Products - Physical Security Upgrades UNC requested relief from certain of the physical security upgrades currently being implemented at Category I facilities. The staff has authorized UNC to discontinue the implementation of certain upgrade-items on the basis that all unencassulated highly enriched uranium will be removed from the site by the

I Seabrook Over the Memorial Day weekend, operators at Seabrook Station completed a series of turbine torsional tests. From the results of these tests, engineers have determined that the recent modifications to the turbine to eliminate the unde-sirable vibrational problem were successful. Operators proceeded to prepare to synchronize the turbine generator to the grid.

At about 8:00 pm on May 29, 1990, Seabrook Station was connected to the New

' England Power Grid (reactor power level was about 15%). This was the.first time in Seabrook Station's history that nuclear-generated electricity was pro-duced and introduced to the grid.

At about 8:00 am on May 30. 1990 -the reactor power level was increased to 18%

i and it will remain at this level for four hours to allow the turbine generator to reach thermal equilibrium. Then the turbine generator will be disconnected from the grid to run a turbine overspeed test. Following the successful com-c pletion of this test, the turbine generator will be synchronized to the grid and reactor power will be' increased to 30% and the full power ascension test program will resume.

Meeting With EPRI On ALWR Source Term

.DREP staff members met with the EPRI staff on May 31, 1990, to discuss various aspects of the source term for the ALWR, Members of NRR, as well

-as RES, attended.

Public Meeting Of NRC Staff and BWR Owners' Group On Motor-0perated Valves

.On May 24, 1990, the NRC staff held a public meeting with the Boiling Water Reactor (BWR) Owners' Group to discuss ongoing efforts to improve the perfor-L mance of motor-operated valves (MOVs) at nuclear power plants.

At the meeting, JUNE 1, 1990 ENCLOSURE A

the BWR Owners' Group provided a status report on its program to assist BWR licensees in~ responding to NRC Generic Letter 89-10. " Safety-Related Motor-Operated Valve Testing and Surveillance." The program involves the establish-ment of the design-basis differential pressure and flow conditions for MOVs in safety-related systems. The staff discussed its ongoing review of the results

.j if the NRC research program associated with the resolution of Generic Safety Issue (GI) 87, " Failure of HPCI Steam Line Without Isolation." A draft infor-mation notice on the research results has been prepared and was distributed at the meeting. As part of its review of the researci results, the staff conducted a survey of the MOVs within the scope of GI-87 at six BWR units. The survey results and conclusions were provided at the meeting.

Based on the survey, the-staff believes that a number of these valves may be marginal in their perfor-mance. The BWR Owners' Group has agreed to assist the staff in obtaining Sc, similar information on the MOVs within the scope of GI-87 at the other BWR 1

plants. The staff is preparing a letter to formalize this request for assis-tance from.the BWR Owners' Group.

Rosemount Transmitter Concern On May 23-24, 1990, a meeting was held with Rosemount at Eden Prairie, MN to discuss the current status of Rosemount transmitters regarding the loss of fill oil concern. A tour of the plant included the enhanced testing facilities for detecting fill oi'l problems in Model 1153 and 1154 transmitters.

SAFEGUARDS AND TRANSPORTATION International Action Plan Working Group (APWG) Meeting The APWG, which met on May 29, 1990, is comprised of representatives from the Department of State, Department of Energy, Arms Control and Disarmament-Agency and the Nuclear Regulatory Comission. Topics discussed. included the status of outstanding documentation from international safeguards meetings; the schedule for future bilateral safeguards meetings with France, Federal Republic of Germany, EURATOM,_ Japan and the United Kingdom; safeguards and nuclear material reporting issues identified by the EURATOM Inspectorate during the February 1990 bilateral safeguards meeting; and funding from the U.S. Support Program to the International Atomic Energy Agency for consulting services.

Interagency Meating on the Fature of Large Scale Reprocessing (LASCAR)_.

Safeguards Project l

-On-May 29, 1990, representati'es from the Department of Energy, Department of State, Arms-Control and Disarmament Agency and the Nuclear Regulatory Commission met to discuss the J.S. agenda for the upcoming LASCAR Plenary Meeting in Avignon, France in oune. An evaluation of the reports-from the Working Groups-addressing technical considerations impacting on the application-p of safeguards _in the main process and product areas of a reprocessing plant will be the principal task of the Plenary Meeting. The future of LASCAR, including possible arrangements for the 1991 Plenary Meeting being held in

.the U.S., will also be discussed.

L International Atomic Energy Agency (IAEA) Safeguards Inspection On May.29, 1990, the IAEA initiated a short_ notice random inspection at t.5e General Electric Fuel Fabrication Plant in Wilmington, North Carolina.

Inspection activities included an audit of the facility ledger, sampling of

. uranium dioxide pellets packaged for shipment and scanning of fuel rods.

Office of Management and Budget (OMB) Clearance for the Nuclear Regulatory Commission (NRC)/ Department of Energy (DOE) Form 741 Staff has received OMB clearance for NRC/ DOE Form 741, the Nuclear Material

. Transaction Report, which is utilized by all licensees to report nuclear material transactions to the Nuclear Materials Management and Safeguards

-System (NMMSS).

In granting clearance approval, OMB requested NRC to prepare

-an addendum to NUREG/BR-0006, Revision #3, licensee instructions for completing Form 741.. The clearance for the form expires July 31, 1992 and OMB has requested that a revised NUREG be printed and distributed before that date.

On May 21-24, 1990, Transportation Branch staff conducted an inspectica at Transnuclear, Inc.,-facilities in Hawthorne, New York.

L Office (YMPO) audit of Lawrence Livermore National Laboratory (LLNL). LLNL is the DOE /YMP0 contractor for the development of a waste package.

-The NRC team consisted of QA and technical staff who evaluated both the LLNL QA 3rogram and the effectiveness of the DOE audit. The DOE auditors reviewed wor ( in four technical areas: waste package performance assessment, metal barrier selection and testing, waste form, and container fabrication process development. Work has been reduced under LLNL's QA program due to a funding 1

cut in December 1989 followed by a subsequent slow down in completing readiness reviews prior to starting activities which fall under the scope of work fur LLNL.

The DOE audit was performed in a professional manner both from the perspective of the audit team and the laboratory.

It was noted that LLNL had adequate QA controls in place, people were trained in QA requirements, and there was satisfactory implementation in the programmatic areas that were evaluated by the audit team. The DOE audit team identified 10 preliminary findings. The findings (six deficiencies and four observations) were not significant.

In general, LLNL's QA Program is adequate and effective to the extent it has been i

implemented.

West Valley Demonstration Project The Department of Energy (DOE) is negotiating a compliance agreement and consent order with the Environmental Protection Agency (EPA) that would cover JUNE 1, 1990 ENCLOSURE B

DOE activities at the West Valley Demonstration Project. The New York State

. Energy Research and Development Authority (NYSERDA), DOE's partner in the West Valley Demonstration Project and the owner of the West Valley property, would be a party to these agreements as would the New York State Department of Environmental Conservation, the state agency authorized by EPA to regulate

. hazardous wastes. The purpose of the agreement under negotiation would be to establish a process for implementing plans and schedules for the management of radioactive mixed waste consistent with the requirements of the Resource Conservation and Recovery Act and the New York State Environmental Quality Review Act. Office for Analysis and Evaluation of Operational Data Items of Interest Week Ending June 1, 1990 f

Division of Operational Assessment (DOA)

Planning and preparation of the Zion Diagnostic Evaluation Team was completed during the period May 29-June 1, 1990.

The team will arrive onsite on June 4 for the entrance meeting and to begin the initial evaluation efforts. The AEOD Deputy Director and Region Administrator will join the team for the entrance meeting with the licensee. The it;itial onsite evaluation period is scheduled for June'4-15.

The NRC Staff Actions Requests resulting from the Amersham Incident Investigation was issued on May 25, 1990.

On May 30-31, an Electrical Inspector Counterpart Meeting coordinated by Region II was held at the TTC. Representatives from NRR, RES and each region participated. The TTC arranged for a guest speaker who made a presentation on surge protection in power plants. Other topics discussed included degraded grid voltage, fuse / breaker coordination, nuclear plant aging research, D.C.

system problems, diesel generator problems, breaker sizing, Emergency Data System (EDS) inspections and lessons learned from the pilot Electrical Distribution System Functional Inspections (EDSFI).

Initial distribution of the Vogtle Incident Investigation Report was made within the NRC May 31, 1990 in preparation for a commission briefing June 8, 1990.

Division of Safety Programs (DSP)

Technical Review Re) ort, AE0D/90-07, on " Reversed Sensing Lines Connections" was issued-this wee (.

This study was initiated to review an event which involved reversed connections of sensing lines to a containment hydrogen / oxygen analyzer.

The root cause of this event was determined to be mislabeling of the analyzer inlet / outlet ports. Nincteen additional events of reversed sensing line connections were identified by data searches. The use of inadequate design

-documents, deficient startup procedures, and weak verification methods were determined to be the primary causes for these events. This review determined that the frequency of occurrences is relatively small and the consequences of the events resulted in limited safety significance. No further NRC action was recommended.

State Programs National Energy Strategy Sub-Group Staff Meeting Spiros Droggitis, State Programs, participated in an Economic Policy Council (EPC) National Energy Strategy (NES) Sub-Group Staff meeting on May 31,1990 at the Department of Energy Headquarters in Washington, DC.

The principle objectives of the meeting were to circulate a list of narrowed options from the NES Interim Report, issue guidance on analysis of the options, organize the analytical efforts into Sub-Groups, identify DOE leads for each Sub-Group and exchange information on points of contacts.

Second Building Status GSA provided additional portions of a revision to the developer's proposed lease addendum during the week, but as of June 1, we had not received the complete GSA revision on which we are expected to comment. We understand that the remaining portions will be provided to us by June 4.

As soon as we have received GSA's proposed revisions and commented on them, GSA will schedule a meeting with the developer to discuss the revisions.

Enforcement Conference with Georgetown University Medical Center (GUMC)

An enforcement conference will be conducted with GUMC in the Region I office on May 31, 1990. The enf orcement conference will focus on the 2

a> parent loss of control which led to a 2.1 curie iridium-192 brachy-tierapy source being picked up and shipped without authorization and proper shipping papers. Within three days of realization of the loss, the carrier was identified and the source was located at the carrier's warehouse in Des Plaines, IL. The source was surveyed and properly prepared for shipment by a consultant, then returned by the shipper to GUf1C on May 7, 1990.

Seabro,ok The reactor was at 8% power on May 29, 1990 and turbine torsional testing was completed at 5:45 a.m.

Preliminary review indicated that the turbine resonance problem has been resolved. NHY planned to resume turbine operation in the evening, shifting from the startup bypass valves to the feed-reg valves for the first time, and synchronizing the generator to the grid.

The generator was connected to the New England grid at 8:04 p.m., May 29; Seabrook generated electricity from reactor heat for the first time.

Steam generator feedwater regulating valve FRV-C was unstable in automatic and was placed in manual control.

No other problems were noted. At 7:00 a.m. on May 30th, about 180 MWe was being generated during a slow escalation to 216 MWe (18% of rated MWe).

After four hours at rated electrical output, turbine overspeed testing began at 7:00 p.m., May 30. Mechanical trip setpoint repeatability was not acceptable. After the overspeed test, power was reduced below 5% (to Mode 2) because of high steam generator (SG) conductivity (greater than 7 umho/cm) and chlorides (which approached the 100 ppb admin guideline). SG chemistry problems are expected as coatings and contaminants are flushed out by higher power flows.

Corrective actions are SG blowdown, shifting suc' ion to the condensate storage tank, and cycling the hotwell through the temporary, trailer-mounted demineralizers provided for the power ascension program. Mode 1 testing is expected to resume this weekend after SG cleanup and turbine mechanical trip maintenance.

Indian Point 2 IP2 steam generator contained several ID cracks in heat effect zone of welds; cracks were propagating from pits. Licensee was requested to extend his inspection to include visual %spection of lower shell assy, looking for pitting. Six zone areas wete visually inspected including a JUNE 1, 1990 ENCLOSURE N

longitudinal seam. Using upper shell area as reference standard for visual inspection, the lower shell assy does not have evidence of pitting l

problem.

Haddam Neck Several hundred ID indications within the pressurizer cladding next to heater area have been detected and evaluated as cracks propagating thru the cladding, but not into base material (carbon steel)

Additionally, three indications required further examination by the licensee and were dispositioned on 5/30/90, with cracks penetrating into base material 1/4", two of the indications has lengths of 3/16" and one l

with a length of 1/8". Licennee will provide the NRC an analysis of the acceptability of these conditions.

Davis Memorial Hospital Enforcement Conference 1

On May 30, representatives from the Davis Memorial Hospital were in the Region II Office to attend an Enforcement Conference regarding findings of the NRC inspection conducted on February 7, 1990. The licensee acknow-1 edged that inadequate management attention had been given to the program.

The licensee reviewed the corrective action they have taken or planned for the violations. The hospital CEO stated that he closed the nuclear medicine department because of his concern for patient safety and would keep it closed until the deficiencies in the program were corrected. The CEO indicated that management changes in the nuclear medicine department i

was being considwed.

Comissioner 'tisits On May 31 - June 1 Commissioner Curtiss, visited the Carolina Power &

Light Company's H. B. Robinson and Brunswick Nuclear Power Stations. The Commissioner was accompanied by the Acting Director, Division of Reactor Safety and a Division of Reactor Projects Section Chief 4.

Virginia Electric ani! Power Company On June 1, the Regional Administrator, accompanied by selected Region II and NRR staff, was at the Surry Nuclear Power Station to present the results of the Surry SALP to representatives of the Virginia Electric and Power Company.

On May 29, the licensee reported a potential overexposure of a radio-grapher to the NRC. Region II diverted an inspector to the licensee's job site to evaluate the incident. Based on the reenactment of the incident and the results from the radiographer's TLD, the inspector concluded that an overexposure did not occur, and that the radiographer's whole body exposure was less than one rem, t

On May 31, 1990, B. S. Mallett, Chief, Nuclear Materials Safety Branch, and other regional staff conducted a management meeting to d scuss the violations found during a recent inspection and the licensee's corrective actions. The inspection identified six isolated violations which indicated that management controls were not fully adequate. Further enforcement action is not being considered in this case, because the deficiencies do not appear to be programmatic.

On May 29, 1990, Charles E. Norelius, Director, Division of Radiation Safety and Safeguards, and selected regional staff conducted an Enforcement Conference to discuss (1) violations identified during a recent inspection.

(2) the licensee's corrective action, and (3) NRC enforcement options.

The problems centered around two therapeutic misadministrations that occurred during the use of a Microselectron high dose rate remote afterloading brachytherapy device. The problems included inadequate verification of treatment plans and use of the device by an unqualified Escalated enforcement for these problems will be proposed by person.

Problems were identified in attentiveness and responsiveness to control room indicators, conduct of critical operations during shift turnovers and supervision-of licensed operators by the senior licensed operator in the control room. Escalated enforcement will be recommended by Region III as a result of the problems disclosed through this event.

Power and Industry;ia"; State Comittee on Supervision of Nuclear Safety; Organization "Energ Bureau of Fuels and Energy Complex; Ministry of Foreign Affairst and the Central Comittee of the Communist Party. The visit is sponsored through the Ninth Joint Committee Meeting of the US/ USSR Agreement on the Peaceful Uses cf Atomic Energy.

Zion Nuclear Power Station The Zion Diagn(stic Evaluation Team inspection, led by Mr. A. F. Gibson, Director, Division of Reactor Safety. Region II, begins June 4, 1990, at the Zion Station. The team will evaluate the status of plant performance in the areas of engineering design and technical support, operations and quality training, maintenance, surveillance and testing, chemistry,lso consider programs, and management and organization. well as at the Zion site, and review the past performance of the Zion l

Station staff.

Braidwood Nuclear Power Station l

On May 31, 1990, a delegation from the Soviet Union's Rovno nuclear plant toured the licensee's facility, including the Production Training Center j

near the Braidwood Station. The members of the delegation from the Rovno i

plant are Vladimir Korovkin, Plant Manager and Boris Tur, Head of Maintenance, with Olga Romanova, interpreter. The visit is the first under a protocol agreement siined as a result of a visit by Byron l

personnel to the Rovno plant n October 1989. The purpose of the visit l

is to exchange technical information that would be beneficial for both plants. The delegation will visit Byron Station from June 4 through June 12, 1990.

High Mountain Service. Inc. Enforcement Conference An enforcement conference was held on May 31, 1990, with High Mountain Service, Inc., at the Region IV Uranium Recovery Field Office in Denver.

Colorado, to review the findings of a routine, unannounced inspection conducted April 18 and May 9-10, 1990. The main areas of concern for this conference was the failure of a radiographer to survey a radiographic exposure device after each exposure and the failure to recharge pocket dosimeters at the start of each shift.

Newman Memorial Hospital Enforcement Conference An enforcement conference was held on June 1, 1990, at the Region IV office with Newman Memorial Hospital to review the findings related to an inspection conducted at the licensee's facility on January 25-26 and May 10, 1990. The conference focused on the lack of management oversight by both the radiation safety committee and radiation safety officer resulting__in a number of violations identified during this inspection.
