CAUSE: TURBINE / REACTOR TRIP POSSIBLY DUE TO A STEAM LEAK (NEAR THE TURBINE TRIP' DEVICE-CONTROLS) WHICH MAY.HAVE AFFECTED MECHANICAL OR ELECTRICAL COMPONENTS.

CORRECTIVE ACTION: DEFECTIVE WELD REPAIRED AND EQUIPMENT INSPECTED.

CAUSE: HIGH VIBRATION ON #3 MAIN TURBINE BEARING. 3 CORRECTIVE ACTION: MANUALLY TRIPPED TURBINE.

! 89 L CAUSE: REACTOR TRIP OCCURRED DURING SURVEILLANCE OF CHANNEL N44 DUE TO POWER LOSS TO N43 AND THE FAILURE OF CONTROL ROOM OPERATORS TO NOTICE THAT THE WRONG BISTABLE HAD TRIPPED.

CORRECTIVE ACTION: EXTENSIVE TROUBLESHOOTING OF N43 WAS PERFORMED; HOWEVER, THE CAUSE OF POWER LOSS COULD NOT BE DETERNINED.

CAUSE: REACTOR TRIP DUE TO STEAM GENERATOR #2 LO-LO LEVEL. INTERCEPT. VALVES DID NOT. t OPEN WHEN TURBINE WAS BROUGHT UP TO SPEED. -THE BOP OPERATOR' FAILED TO ENSURE.

THAT THE VALVES OPENED.(AS REQUIRED BY PROCEDURE 13800-2)..

l CORRECTIVE ACTION: ADJUSTMENTS TO CORRECT THE CLOSING BIAS VOLTAGE ON THE CIRCUIT CARDS: -)

FOR THE INTERCEPT VALVES WERE MADE'AND THE B0P OPERATOR HAS BEEN COUNSELED.. j I '89 ,

CAUSE: TURBINE MANUALLY TRIPPED TO REPAIR TURBINE CONTROLS.
