This refers to the routine safety inspection (50-293/90-05) conducted by Messrs.
J. Macdonald, A. Cerne, and Mrs. C Carpenter of this office on January 16 to
March 8, 1990 at the Pilgrim Nuclear Power Station, Plymouth, Massachusetts.

Areas examined during this inspection are described in the NRC Region I Inspec-
tion Report which is enclosed with this letter.

It appears that one of your activities involving testing instrument line flow
[
check valves was not conducted in accordance with NRC requirements as set forth
in the enclosed Notice of Violation, Appendix A. You are required to respond to
this letter following the instructions in Appendix A.
In your response you
l
should address the apparent discrepancy in the manufacturer's flow test results
1
versus those specified in procurement documents; and the repetitive failures of.
your staff to address the Technical Specification Surveillance requirement.
Additionally, we have reviewed your February 26, 1990 reply to the Notice of
Violation enclosed with our letter dated .lanuary 19, 1990 regarding failure to
properly implement equipment tagout procedures.
The corrective actions stated
in your letter appropriately address this issue.
The long term effectiveness
of these actions will be evaluated during the current outage and during routine
inspections.
