Gentlemen:

Subject: Report Nos. 50-335/81-28 and 50-389/81-22

This refers to the routine safety inspection conducted by Mr. E. H. Girard of

this office on November 2-5 and 9,

1981, of activities authorized by NRC

Operation License No. OPR-67, and Construction Permit No. CPPR-144 for the St.

Lucie facility.

Our preliminary findings were discussed with Mr. B. J. Escue,

Site Manager, at the conclusion of the inspection.

Areas examined during the inspection and our findings are discussed in the

enclosed inspection report.

Within these areas, the inspection consisted of

selective examinations of procedures and representative records, interviews with

personnel, and observations by the inspector.

During the inspection, it was found that certain activities under your license

appear to violate NRC requirements.

This item and references to pertinent

requirements are listed in the Notice of Violation enclosed herewith as

Appendix A.

Elements to be included in your response are delineated in

Appendix A.

The violation identified in Appendix A is similar to two violations disclosed

in previous inspections.

In an April inspection (Item 389/81-05-02) several

examples of welders failing to follow welding procedures were identified. Again

in August (Item 389/81-16-01) additional examples of welders not following pro-

cedures were identified.

It would seem that the actions you have previously

taken to prevent recurrences of this violation have been ineffective.

requested that you give this matter your particular attention since repetitive

violations are subject to escalated enforcement action by NRC.

In addition to

your response to Appendix A, please inform us of your evaluation of the under-

lying causes of these violations and how you propose to remedy these causes.

In accordance with 10 CFR 2.790(a), a copy of this letter and the enclosures

will be placed in the NRC's Public Document Room unless you notify this of fice,

by telephone, within ten days of the date of this letter and submit written

application to withhold information contained therein within thirty days of the

date of this letter. Such application must be consistent with the requirements

of 2.790(b)(1).
