RII:EHG 50-389/81-22 Violation:

Inadequate Corrective Action for Welding i'rocedure Violations 10 CFR 50, Appendix B, Criterion V, as implemented by Topical Report TQARI-76A, requires the licensee to accomplish quality-related activities in accordance with prescribed procedural requirements.

Contrary to the above, on November 3,1981, the licensee failed to accomplish quality-related welding activities in accordance with the prescribed procedural requirements in that they used welding amperages below the minimum specified for welding reactor coolant pressure boundary joint CH-0147-002. The welding procedure designated for this joint, procedure WPS-43A, Rev.1, specified a minimum welding amperage of 65 amps when welding with 3/32 inch filler metal.

The licensee welded the root and hot pass of the joint with 3/32 inch diameter fiiler metal using 50 and 60 amps respectively.

Response

1.

We concur with the finding as specified on November 3,1981.

The Welding Procedure was violated in using lower amperage than specified in the Welding procedure Specification, on Joint CH-0147-002 as well as on two previous findings.

2.

The underlying cause in each case was that the welder was in error in assuming the correct amperage for WPS-43A R-3 and failed to review the requirements prior to making the repair weld.

3.

To increase the effectiveness of corrective action, we have recalled all Weld Repair Reports and placed a copy of the WPS in the jacket for the welders to review prior to making the weld.

4.

Corrective steps taken are:

a.

Welding Supervisors to monitor welds with ammeter tongs on a daily basis and report any violation of the WPS.

b.

Attach a Weld Procedure Specification with each Weld Repair Report for review of the essential and non-essential variables prior to making the weld.

c.

Discrepant Field Condition 0957 was written to request radiographic examination of the subject weld which was found to be acceptable.

5.

Full compliance has been achieved.
