APPENDIX A -
NOTICE OF VIOLATION Florida Power and Light Co.
Docket No. 50-389 St. Lucie 2 License No. CPPR-144-As a result of' the inspection conducted on November 2-5 and 9,1981, and in accordance with the Interim Enforcement Policy,.45 FR 66754 (October 7,1980),
the following violation was identified.
10 CFR 50, Appendix B, Criterion V, as implemented by Topical Report TQARI-76A, requires the licensee to accomplish quality-related activities in accordance with prescribed procedural requirements.
Contrary to the above, on November 3, 1981, the licensee failed to accomplish quality-related welding activities in accordance with the prescribed procedural requirements in that they used welding amperages below the minimum specified for welding reactor coolant pressure boundary joint CH-0147-002.
The welding procedure designated for.this joint, procedure WPS-43A, Rey, 3, specified a minimum welding amperage of - 65 amps when welding with 3/32 inch filler metal. The licensee welded the root and hot pass of the joint with 3/32 inch diameter filler metal using 50 and 60 amps.
respectively.
This is a Severity Level V Violation (Supplement II.E).
Pursuant to the provisions of 10 CFR 2.201, you are hereby required to submit to this off!ce within thirty days of the date of this Notice,.a written statement or explanation in reply, including: (1) admission or denial of the alleged viola-tions; (2) the reasons for the violations if admitted; (3) the corrective steps which have been taken and the results achieved; (4) corrective steps which will be taken to avoid further violations; and (5) the date when full compliance will be achieved. Consideration may be given to extending your response time -for good cause shown.
Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation.
