Report Nos. 50-338/81-19 and 50-339/81-16

This refers to the special inspection conducted by Mr. D. J. Burke and others of

this office during the period July 3-15, 1981, of activities authorized by NRC

License Nos. NPF-4 and NPF-7 for the North Anna facility.

Our findings were

discussed with Mr. W. R. Cartwright, Station Manager, at the conclusion of the

inspection.

Areas examined during this inspection included the events associated with the

Unit 2 main transformer failure and resulting fire of July 3,1981.

Within these

areas, the inspection consisted of selective examinations of procedures and

representative records, interviews with personnel, and observations by the

inspectors.

During the inspection, it was found that certain activities under your license

appear to violate NRC requirements.

This item and references to pertinent

requirements are listed in the Notice of Violation enclosed herewith as

Appendix A.

Elements to be included in your response are delineated in

Appendix A.

In addition to the need for corrective action regarding the specific violation,

we understand that you are considering improvements associated with this event.

We will review your actions and plans during subsequent inspections. Addi-

tionally, we are forwarding a number of potentially generic issues identified in

this report to our licensing staff for fsrther e"aluation.

We will inform you

of the results of this evaluation at a !ater date.

in accordance with Section 2.790(d) of the NRC's " Rules of Practice," Part 2,

Title 10, Code of Federal Regulations, activities involving safeguards and

security measures are exempt from public disclosure; therefore, the Supplement to

the enclosed inspection report will not be placed ia the Puclic Document Room. A

copy of this letter and the enclosed inspection report with the exception of the

Supplement will be placed in the NRC's Public Document Room.

If this report

contains any information other than that in the Supplement that you believe to be

exempt from disclosure under 10 CFR 9.5(a)(4), it is necessary that you:

(a)

notify this office by telephone within ten days from the date of this letter of

your intention to file a request for withholding; and (b) submit within

twenty-five days from the date of this letter a written application to this

office to withhold such information.

If your receipt of this letter has been

delayed such that less than seven days are available for your review, please

notify this office promptly so that a new due date may be established. Con-

sistent with section 2.790(b)(1), such application must be accompanied by an

or part thereof sought to be withheld, and a full statement of the reasons on the

basis of which it is claimed that the information should be withheld from public

discl osure.

This section further requires the statement to address with

specificity the considerations listed in 10 CFR 2.790(b)(4). The information

If we do not hear from you in this regard within the
