APPENDIX A NOTICE OF VIOUTION Carolina Power and Light Company Docket Nos. 50-325 and 50-324 Brunswick 1 and 2 License Nos. DPR-71 and DPR-62 As a result of the inspection conducted on September 21-25, 1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980),
the following violations were identified.
A.
Technical Specification 6.8.1 requires that written procedures be estab-lished, implemented and maintained covering the activities and procedures recommended in Appendix A to Regulatory Guide 1.33, November 1972.
Appendix A to this Regulatory Guide lists procedures for control of radio-activity in the liquid radioactive waste system.
Contrary to the above, liquid radwaste system Procedure OG-8 C-81-13 was not.
adequately established and implemented on August 9,1981, in that it did not contain sufficient precautions, prerequisites, directions and acceptance criteria to control the transfer of spent resin sludge from the waste collector tank to the Unit 2 phase separator without a release of radio-activity.
As a result, the temporary flexible transfer line became overpressurized and burst releasing highly radioactive spent resin sludge to the general area of the -3 level of the radwaste building and contaminating five workers. One worker required offsite medical assistance.
This is a Severity Level V Violation (Supplement IV).
B.
Technical Specification 6.8.3 requires that temporary changes to required procedures be reviewed by the Plant Nuclear Safety Committee (PNSC) and approved by the General Manager within 14 days of implementation.
Contrary to the above, a temporary change to liquid radwaste processing system procedure OG-8 C-81-9 implemented on July 27, 1981, was not reviewed by the PNSC nor approved by the General Manager as of September 25, 1981, although it remained in effect through August 1981.
This is a Severity Level VI Violation (Supplement IV).
Pursuant to the provisions of 10 CFR 2.201, you are hereby required to submit to this~ office within thirty days of the date of this Notice, a written statement or explanation in reply, including: (1) admission or denial of the alleged viola-tions; (2) the reasons for the violations if admitted; (3) the corrective steps which have been taken and the results achieved; (4) correc.ive steps which will be taken to avoid further violations; and (5) the date when full compliance will be achieved. Consideration may be given to extending your response time for good cause shown. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation.
