T1m failed header accenblics cach connict of two valvec rounted vertically in a dead-end,12-inch diancter pfpc projecting at a 90' horizontal angle from the main etcan line. The two headers are counted at 1800 to cach other on the opposite sides of the main steam line. The headero were denigned to ecct Soction 1 of tho AS:'I Code by the Lechtel Corporation.

For your reactors which are licensed to cperate, it is rcquestod that you provide this office t71 thin ten days, in writing, as to whether headers as described above are installed in the stcam lincs of your facility.
