This letter refers to the- routine safety inspection conducted'by M. Banerjee,

E. Collins, D. Lew and W.7 Pasciak for the period February!18 through March 17,

1990,'at the;0yster Creek Nuclear Generating Station.-- The inspection consisted

of document reviews, personnel interviews and observations ofc activities. .The

i

enclosed report documents the inspection results.

Inspectors discussed the-

.'

findings with you and members'of your staff after the inspection.

Paragraphs 1.2 and 1.5 discuss operational events where plant equipment was.

degraded or misaligned.

In the first event, a fire protection system trouble

t

alarm was not addressed in a timely manner.

In the;second event,-inadequate.

'

.-procedures and operator errors allowed a startup to be conducted with the rod.

worth minimizer bypassed. These-events show a' continuing need to emphasize to

plant personnel the importance of prompt' identification and correction of de-

ficient conditions, including procedural weaknesses.

Paragraphs 2.1 and 2.2_of this report discuss radiological control events which

highlight some important-weaknesses. -In the first event, maintenance was

'

.

performed without completing a formal ALARA review.

Poor coordination between

j

the radiological controls group and the maintenance group resulted.in a lack of

understanding of the scope of the. work and an incomplete ALARA review.

In the

second event, radioactive contamination was identified in a'normally nonradio-

'

active system.

However, no safety evaluation was performed or corrective

actions initiated to control or monitor potential release points until an

('

unplanned release of a small amount of radioactive liquid offsite. Both events

show the importance of timely and effective implementation of your performance

improvement plans in the area of radiological controls.

Based on the results of this inspection, four of-your activities were not con-

ducted in full compliance with NRC requirements, as set forth in the Notices

of Violation enclosed herewith as Appendix A.

The violations have been cate-

gorizei by severity level in accordance with the " General Statement of Policy

and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (Enforce-

ment Policy). You are required to respond to this letter and in preparing your

response you should follow the instructions in Appendix A.
