Appendix A NOTICE OF VIOLATION Department of Health and License No. 34-07167-01 Human Services As a result of the inspection conducted on July 16-17, 1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the following violations were identified:
1.
Condition 8.C. of your license limits the amount of carbon-14 that you may possess at any one time to a total of 6 millicuries.
Contrary to this requirement, on July 16, 1981, you possessed at least 27 millicuries of carbon-14, an amount in excess of your possession limit.
This is a Severity Level V violation (Supplement VII).
2.
Condition 19 of your license requires that you conduct a quarterly physical inventory to account for all byproduct material received and possessed under the license.
Contrary to this requirement, as of July 16, 1981, you failed to conduct quarterly physical insentories to account for all byproduct material received and possessed since this requirement became effec-tive on May 25, 1978.
This is a Severity Level V violation (Supplement VII).
3.
Condition 15 of your license requires that you test your chromatograph detectors containing nickel-63 for leakage and/or contamination at intervals not to exceed six months.
Condition 18 of your license requires that you test your sealed sources containing byproduct material for leakage and/or contamina-tion at intervals not to exceed six months.
Contrary to the above requirement as of July 17, 1981, you failed to leak test several chromatograph detectors containing nickel-63 and scaled sources containing cesium-137 and cobalt-60 within six month ictervals as shown in your " Inventory of Sealed Sources of Radio-isotopes" which was submitted to the NRC with your letter dated September 3, 1981.
This is a Severity Level V violation (Supplement VII).

Appendix A Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written statement or explanation in reply, including for each item of noncompliance:
(1) cor-rective action taken and the results achieved; (2) corrective action to be taken to avoid further noncompliance; and (3) the date when full compliance will be achieved. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation. Consideration may be given to extending your response time for good cause shown.
