This refers to the routine safety inspection conducted by Mr. S. R. Lasuk of this office on July 16-17, 1981, of activities at 4676 Columbia Parkway in Cincinnati, Ohio authorized by NRC Byproduct Material License No. 34-07167-01 and to the discussion of our findings with you at that time and to the need for addition information in order to complete the inspection. This also refers to the discussion of additional findings in telephone conversat;ons between you and Mr. Lasuk on October 16 and October 23, 1981. The latter findings were based on the leak test information which you submitted to this office with your letter dated September 3, 1981.
The inspection was an examination of activities conducted under your license as they relate to radiation safety and to compliance with the Commission's rules and regulations and with the conditions of your license. The inspec-tion consisted of a selective examination of procedures and representative records, observations, independent measurements, and interviews with personnel.
The inspection also included a review of the actions described in your letter dated April 10, 1978, regarding the apparent items of noncompliance found during our January 25, 1978 inspection. We have no further questions regarding these matters.
During this inspection, certain of your activities appeared to be in non-compliance with NRC requirements, as specified in enclosed Appendix A.
